Site: Medical-
Violation: THE MEDICAL STAFF WERE NOT ABLE TO LOCATE THE SAMPLE COLLECTION CONTAINERS FOR BACTERIAL TESTING OF STOOL.
Recommendation: 4.3.1.1 Maintain a supply of 10 clinical specimen collection containers for bacterial agents.

Violation: THERE WERE A FEW DAYS WHEN THE ANALYZER CHART RECORDER FOR THE PRODUCED WATER NOTED LESS THAN 2.0 PPM FOR SEVERAL HOURS. THE LEVELS WERE FROM 1.75 PPM-1.95 PPM.
Recommendation: Ensure that potable water is continuously halogenated to at least 2.0 mg/L (ppm) free residual halogen at the time of bunkering or production with an automatic halogenation device.

Violation: THE LINE USED TO ADD WATER TO THE VAP TREATMENT TANK WAS CONNECTED TO A TECHNICAL WATER LINE. THIS TREATMENT TANK IS USED FOR THE EVAPORATOR INCLUDING WHEN POTABLE WATER IS MADE.
Recommendation: Ensure the water line used to add water to the vap treatment tank is from a potable water source.

Violation: THERE WERE SEVERAL SMALL DIAMETER POTABLE WATER LINES THAT WERE NOT STRIPED BLUE IN THE ENGINE ROOM, WORKSHOPS, WHIRLPOOL/POOL ROOMS, AND GARBAGE ROOMS.
Recommendation: Paint or stripe potable water piping and fittings in auxiliary blue, or in accordance with ISO 14726, at intervals not to exceed 5 m (15 feet) in all spaces, except where the decor would be marred by such markings.

Violation: THE VESSEL WAS USING MORE THAN ONE SHADE OF BLUE FOR THE POTABLE WATER LINES. IN ADDITION, SEVERAL LINES WERE STRIPED BLUE AFTER THE REDUCED PRESSURE BACKFLOW PROTECTION DEVICE IN THE POOL/WHIRLPOOL SPACES.
Recommendation: Paint or stripe potable water piping and fittings in auxiliary blue, or in accordance with ISO 14726, at intervals not to exceed 5 m (15 feet) in all spaces, except where the decor would be marred by such markings. Remove blue striping that is installed after the reduced pressure backflow protection device.

Site: Potable Water-BACKFLOW PREVENTION
Violation: THE BACKFLOW PROTECTION DEVICES USED WERE NOT PROVIDED IN THE COMPREHENSIVE CROSS-CONNECTION CONTROL PROGRAM. THIS IS A NEW REQUIREMENT IN THE 2005 VSP MANUAL.
Recommendation: 5.7.1.1.4 List the method/device for all cross-connections and backflow prevention devices on the vessel so that there is a match to the plumbing system component and location. Repeat devices such as toilets can be grouped under a single device type, and air-gaps shall be included in the listing. A log documenting the inspection or maintenance in written or electronic form shall be maintained and be available for review during inspections.

Site: Potable Water-BACKFLOW PREVENTION BEACH BLANKET GALLEY
Violation: THE CENTER CARBONATOR WAS NOT FITTED WITH A VENTED BACKFLOW PROTECTION DEVICE.
Recommendation: Install a vented backflow protection device on the carbonator.

Violation: THE HOT POTABLE WATER LINE IN WHIRLPOOL 3 ROOM WAS STRIPED BLUE AND GREEN. THIS WAS AN INCORRECT DESIGNATION AS THE HOT POTABLE WATER LINES ON THE VESSEL ARE STRIPED BLUE AND RED. THE RECIRCULATION LINE IN THE "NOT A POOL" ROOM WAS STRIPED BLUE.
Recommendation: Remove the green striping from the hot potable water line and stripe hot water lines with the correct designation. Ensure that non-potable water lines are not striped blue.

Site: Recreational Water Facilities-GOOFY'S POOL
Violation: THE SHEPHERD'S HOOK WAS NOT IN A PROMINENT LOCATION.
Recommendation: Ensure that an easy-access shepherd's hook and approved floatation device is provided at a prominent location near each public swimming pool.

Site: Recreational Water Facilities-KID'S POOL-MICKEY'S EAR
Violation: THE DRAIN COVER WAS NOT STAMPED AS REQUIRED TO INDICATE THAT IT IS ANTI-ENTRAPMENT. A LETTER WAS NOT AVAILABLE INDICATING THAT THIS DRAIN IS ANTI-ENTRAPMENT. THIS IS A NEW REQUIREMENT IN THE 2005 VSP MANUAL.
Recommendation: Ensure that anti-entrapment drain covers or other drains that prevent entrapment hazards are provided on swimming pools and whirlpool spas. An approved cover shall have either the plumbing/engineering approving organization stamp and flow rate affixed to the cover, or a letter maintained onboard the vessel certifying that the cover meets the safety requirements outlined in ASME/ANSI A 112.19.8M or an equivalent standard.

Site: Buffet-CREW MESS BEVERAGE STATION
Violation: THE DESIGN OF THE SODA DISPENSER WAS SUCH THAT THE LIP-CONTACT AREA OF THE CUP WOULD BE SUBJECT TO CONTAMINATION IF RE-FILLING OF GLASSES WAS ALLOWED. WHEN QUESTIONED THE STAFF INDICATED THAT RE-FILLING OF GLASSES OCCURRED.
Recommendation: 7.4.2.4.3 Ensure the dispensing equipment actuating lever of self-service beverage dispensing equipment is designed to prevent contact with the lip-contact surface of glasses or cups that are refilled.

Violation: A LOOSE GASKET WAS NOTED ON PASS-THROUGH REFRIGERATION UNIT 340, MAKING CLEANING DIFFICULT.
Recommendation: Replace the loose gasket with a tight fitting new gasket.

Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Violation: THE LIGHT BULBS IN THE ROLL WARMER WERE NOT SHIELDED OR SHATTER-RESISTANT.
Recommendation: Ensure that light bulbs are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, and linens, or unwrapped single-service and single-use articles.
