Violation: SHIPPING CONTAINERS AND LABELS WERE NOT AVAILABLE FOR SHIPPING CLINICAL SPECIMENS. THIS IS A NEW VSP 2005 REQUIREMENT.
Recommendation: Ensure the medical staff maintains a supply of at least 10 clinical specimen collection containers for both viral and bacterial agents (10 for each), as well as the proper shipping containers and labels for same and provided in Annex 13.4 of this manual.

Violation: ALTHOUGH THE RECORDS INDICATE THAT MAINTENANCE PERSONNEL ARE PERFORMING THE REQUIRED INSPECTIONS, CLEANING AND MAINTENANCE, THERE WAS NO WRITTEN PLAN. THIS IS A NEW REQUIREMENT OF THE 2005 VSP OPERATIONS MANUAL. THIS ITEM WAS CITED ON THE 08/28/2005 INSPECTION.
Recommendation: Ensure that vessels have a written plan to inspect, clean, and maintain the heating, ventilation, and air conditioning systems in accordance with manufactures recommendations and industry standards. Ensure that the plan is available for review during inspections.
