Site: Medical-MEDICAL - CLINICAL SPECIMEN SHIPPING CONTAINERS
Violation: THE VESSEL DOES NOT HAVE THE REQUIRED OUTER SHIPPING CONTAINERS OR THE HAZARD LABELS REQUIRED BY IATA AND DOT. THIS IS A NEW REQUIREMENT UNDER THE VSP OPERATIONS MANUAL - 2005.
Recommendation: Obtain the outer shipping containers and proper shipping hazards labels for the shipment of diagnostic specimens.

Violation: THE GI LOG USED TO RECORD CASES OF ACUTE GASTROENTERITIS CASES DOES NOT LIST ABDOMINAL CRAMPS, HEADACHES AND MYALGIA AS RECORDABLE SYMPTOMS. THIS IS A NEW REQUIREMENT UNDER THE VSP OPERATIONS MANUAL - 2005.
Recommendation: Contact CAPT Vaughan, Vessel Sanitation Program, to obtain the new GI log template with all recordable symptoms.

Violation: THREE CUTTING BOARDS LOCATED IN THE CLEAN DISHWASH STORAGE AREA WERE PITTED AND SCORED MAKING CLEANING DIFFICULT.
Recommendation: Ensure that surfaces such as cutting blocks and boards that are subject to scratching and scoring are resurfaced if they no longer can be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

Violation: THE WATER CONTACT SURFACE OF THE WATER TRAY IN THE UPPER COMPARTMENT OF ICE MACHINE 4811046080 WAS HEAVILY SOILED WITH A BROWNISH MATERIAL. THE ICE MACHINE WAS CLEANED DURING THE INSPECTION.

Violation: APPROXIMATELY SIX PREVIOUSLY CLEANED LARGE BRUTE PLASTIC CONTAINERS HAD FOOD DEBRIS ON THE INTERIOR AS WELL AS ON THE EXTERIOR HANDLES. THESE CONTAINERS WERE STACKED INSIDE OF EACH OTHER. THE CONTAINERS ARE USED TO TRANSPORT RAW TURKEY AND DUCK.
