Violation: THE DIAGNOSTIC SAMPLE COLLECTION CONTAINERS FOR BACTERIAL ANALYSIS WERE EXPIRED.
Recommendation: Ensure that at least 10 clinical specimen collection containers for both viral and bacteriological agents (10 each), as well as the proper shipping containers and labels for same, are maintained by the medical staff. Ensure that diagnostic sample collection containers for bacteria are replaced before expiration.

Violation: THE FAR LEFT COMBINATION OVEN HAD A SPRAY HOSE WITH NO BACKFLOW PREVENTION DEVICE. THE CREW STATED THE HOSE WAS NOT USED AND IT WAS REMOVED DURING THE INSPECTION.
Recommendation: Remove the hose or install an appropriate backflow prevention device.

Violation: THE VENT HOLE WAS PLUGGED AT THE BACKFLOW PREVENTION DEVICE IN THE MEN'S STEAM ROOM GENERATOR. A BACKFLOW PREVENTION DEVICE WAS NOT PROVIDED ON THE POTABLE WATER LINE SUPPLYING THE WOMEN'S STEAM GENERATOR ROOM.
Recommendation: Remove the plug from the vent hole on the backflow prevention device. Install an appropriate backflow prevention device on the potable water line supplying women's steam room.

Violation: SOME OF THE LINES THAT WERE STRIPED BLUE IN THE WHIRLPOOL AND SWIMMING POOL PUMP ROOMS WERE NOT POTABLE WATER LINES. IN ADDITION, LINES WERE STRIPED BLUE AFTER THE REDUCED PRESSURE BACKFLOW PROTECTION DEVICES.
Recommendation: Remove the blue striping from non-potable water lines and from lines after the reduced pressure backflow prevention devices.

Violation: THERE WAS NO DOCUMENTATION ON THE APPROVAL OR RECOMMENDATIONS FOR THE TANK COATING THAT WAS USED ON POTABLE WATER TANK PW2 CENTER ON 11/20/2005.
Recommendation: Ensure that interior coatings on potable water tanks are approved for potable water contact, and all manufacturer's recommendations for application, drying, or curing are followed. Ensure written documentation for the coating used and recommendations followed are available for review during inspections.

Violation: THE DRAIN COVER WAS NOT STAMPED TO INDICATE THAT IT WAS ANTI-ENTRAPMENT (ANTI-VORTEX) AND THERE WAS NO DOCUMENTATION AVAILABLE TO CERTIFY THAT THE COVER MEETS ANTI-ENTRAPMENT REQUIREMENTS.
Recommendation: Ensure that anti-entrapment drain covers or other drains that prevent entrapment hazards as specified in U.S. Consumer Product Safety Publication 363-009801 (dual drains/channel drains) are provided on swimming pools and whirlpool spas. Ensure that drain covers have either the plumbing/engineering approving organization stamp and flow rate affixed to the cover, or that a letter is maintained aboard the vessel certifying that the cover meets the safety requirements outlined in AMSE/ANSI A112.19.8M or an equivalent standard.

Violation: THE RECORD FOR FECAL ACCIDENTS INDICATED THAT THE POOLS WERE SCRUBBED WITH 200 PPM CHLORINE, RINSED AND REFILLED. THERE WAS NO CONTACT TIME. THE FLOW CHART FOR THE VESSEL'S FECAL ACCIDENT PLAN INDICATED THAT THE CHLORINE CONCENTRATION WILL BE RAISED TO 200 PPM FOR 1 HOUR.
Recommendation: Ensure that a fecal accident response procedure, which meets or exceeds the procedure provided in annex 13.11, is documented and available to the VSP for review during inspections. Ensure that the fecal accident response procedure onboard the vessel is followed.

Violation: ON THE FOLLOWING DAYS IN APRIL 2006, THE RECORD FOR THE SHOCK TREATMENT OF THE AFT STBD WHIRLPOOL INDICATED CLOSED, BUT THE WHIRLPOOL WAS OPEN DURING PART OF EACH DAY: 13, 16, 17, 19, 21, 22, 23, 27.
Recommendation: Ensure that the free residual halogen is increased to at least 10.0 mg/L (ppm) in whirlpool spas and circulated for at least 1 hour at the end of each day.

TEMPERATURES WERE TAKEN AND LOGGED WHEN THE ITEMS WERE PLACED ON TIME CONTROL, BUT THE TRAYS WERE NOT LABELED WITH A 4-HOUR DISCARD TIME. THE HOURS OF OPERATION FOR ROOM SERVICE WAS GREATER THAN 4 HOURS.
Recommendation: If time only, rather than time in conjunction with temperature, is used as the public health control for a working supply of potentially hazardous food before cooking, or for ready-to-eat potentially hazardous food that is displayed or held for service for immediate consumption, ensure that (1) The food is marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control; (2) The food is cooked and served, served if ready-to-eat, or discarded, within 4 hours from the time when the food is removed from temperature control; and (3) The food in unmarked containers or packages or marked to exceed a 4 hour limit is discarded.

THERE WAS ANOTHER HANDWASH SINK IN THIS AREA. PERSONNEL COULD USE THE SINK TO WASH THEIR HANDS AND CONTAMINATE THE FOOD ITEMS STAGED IN THE AREA.
Recommendation: Remove the soap dispenser from the area of the utility sink.

Violation: THE HOOD CLEANING CABINET WAS OVER THE PREPARATION COUNTER/PREPARATION SINK AREA. WHEN THE DOOR OF THE CABINET WAS OPENED LIQUID DRIPPED ON TO THE COUNTER. THIS CABINET IS NOT LEAK-PROOF AND THE CLEANSER WAS NOT IN A SPILL-PROOF CONTAINER.
Recommendation: Ensure the cabinet is leak-proof and that the cleaning chemical is placed in a spill-proof container.

Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Violation: SMALL FLIES WERE NOTED ALONG THE SHUTTER DOOR TRACKS. THIS AREA WAS MOIST AND SOILED WITH FOOD RESIDUE.
Recommendation: Ensure that the presence of insects, rodents, and other pests is effectively controlled to minimize their presence in the food storage, preparation, service areas, warewashing, and utensil storage areas aboard a vessel. Maintain this area clean and dry to remove the water and food source for pests.
