Site: Medical-MEDICAL - GASTROINTESTINAL ILLNESS LOGS
Violation: THE SHIP IS NOT ALWAYS INCLUDING ILLNESS ONSET TIMES FOR CASE-PATIENT LISTED ON THE GI LOG. ONE CASE OF GI ILLNESS ON THE PREVIOUS VOYAGE WAS MISCLASSIFIED AS A NON-REPORTABLE CASE. THE CASE HAD 4 EPISODES OF DIARRHEA WITH NO UNDERLYING CONDITION LISTED. THE CASE WAS NOT REPORTED IN THE 24-HOUR OR 4-HOUR REPORTS FOR THAT VOYAGE. THE SHIP IS NOT RECORDING PASSENGERS AND CREW MEMBERS WHO ARE DISPENSED ANTIDIARRHEAL MEDICATION ON THE GI LOGS.
Recommendation: A reportable case of gastrointestinal illness shall be defined as: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period); or (2) Vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature >38 C (100.4 F)); and (3) Reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member. The log shall list: (1) All reportable cases of gastrointestinal illness; (2) All passengers and crew members who are dispensed antidiarrheal medication from the master of the vessel, the medical staff, or other designated staff.
