Site: Potable Water-GARBAGE ROOM
Violation: A WASH DOWN HOSE WITH AN ATTACHED SPRAY NOZZLE WAS SCREWED TO THE HANDWASH STATION BUCKET FILLING TAP IN THE GARBAGE ROOM. A NON-CONTINUOUS PRESSURE BACKFLOW PROTECTION DEVICE WAS INSTALLED ON THE POTABLE WATER SUPPLY LINE.
Recommendation: Install a backflow preventer which is specifically for continuous pressure applications in this location.

Violation: THE POTABLE WATER LINE SUPPLYING THE FEED WATER FOR THE SEWAGE TREATMENT SYSTEM STAGE FILTERS WAS PROTECTED FROM BACKFLOW WITH A NON CONTINUOUS PRESSURE BACKFLOW PREVENTER, BUT THE PIPING SECTION WHERE INSTALLED WAS SUBJECT TO CONTINUOUS PRESSURE, AND THIS PREVENTER IS NOT APPROVED FOR THAT APPLICATION.
Recommendation: Install a backflow preventer which is specifically for continuous pressure applications in this location.

Site: Recreational Water Facilities-LOTUS POOL
Violation: THE LOTUS POOL WAS OPEN FOR USE AND THE FREE CHLORINE RESIDUALS MEASURED ON THREE SEPARATE MANUAL TESTS WERE 0.62, 0.65, AND 0.70 PPM RESPECTIVELY. THE POOL WAS IMMEDIATELY CLOSED AND A RE-TEST MADE LATER REGISTERED OVER 2 PPM.
Recommendation: Ensure that a free residual halogen of >1.0 and <3.0 mg/L (ppm) is maintained in recirculated swimming pools.

Violation: A DENT WAS NOTED ON THE BOTTOM SEAM OF THE ORANGE JUICE CONCENTRATE CAN.
Recommendation: Ensure that food packages are in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Ensure that canned goods with dents on end or side seams are not used.

Violation: TIME ONLY AS A PUBLIC HEALTH CONTROL WAS USED FOR POTENTIALLY HAZARDOUS FOODS THROUGHOUT THE BUFFET AND BUFFET STORAGE. THE SERVICE PERIODS EXCEED 4 HOURS AND LABELS FOR DISCARD WERE PROVIDED ON ALL POTENTIALLY HAZARDOUS FOODS IN STORAGE. WHEN BEEF KABOBS, POTATO WEDGES, WHITEFISH FILLETS, AND BREADED SCALLOP WERE REMOVED FROM TIME CONTROL STORAGE AT THE GRILL STATION, COOKED, AND PUT ON THE SELF-SERVICE BUFFET THE TIME CONTROL LABEL WAS NOT TRANSFERRED WITH THE FOOD TO THAT DISPLAY CASE. ADDITIONALLY, MILK AND CREAMER PITCHERS ON THE BEVERAGE LINE WERE HELD UNDER TIME CONTROL AND LABELED, BUT THE LABEL WAS FOR THE TIME THOSE PRODUCTS WERE PLACED ON TIME CONTROL RATHER THAN THE DISCARD TIME.
Recommendation: If time only, rather than time in conjunction with temperature, is used as the public health control for a working supply of potentially hazardous food before cooking, or for ready-to-eat potentially hazardous food that is displayed or held for service for immediate consumption, ensure that (1) The food is marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control; (2) The food is cooked and served, served if ready-to-eat, or discarded, within 4 hours from the time when the food is removed from temperature control; and (3) The food in unmarked containers or packages or marked to exceed a 4 hour limit is discarded.

Violation: TWO SMALL TRAYS OF CREAM CHEESE WERE MEASURED AT 74F AT 1530. THESE ITEMS WERE NOT LABELED AND WERE NOT ON TEMPERATURE CONTROL OR TIME CONTROL. THE TRAYS WERE SET OUT FOR SERVICE SINCE THE MORNING AND WERE LOCATED ON THE PREPARATION COUNTER. ALL ITEMS WERE DISCARDED DURING THE INSPECTION.

Violation: SEVERAL 1/2 PINT CONTAINERS OF MILK WERE MEASURED AT 50F AT 1530 IN THE REACH-IN UNDERCOUNTER REFRIGERATOR. THESE ITEMS HAD BEEN IN THE UNIT FOR GREATER THAN FOUR HOURS AND WERE ON TEMPERATURE CONTROL. THE MILK WAS DISCARDED.
Recommendation: Except during preparation, cooking, or cooling, or when time is used as the public health control, ensure that potentially hazardous food is maintained: (1) At 60C (140F) [roasts 54C (130F)] or above, or (2) At 5C (41F) or less.

Recommendation: Ensure that equipment for cooling and heating food, and holding cold and hot food, is sufficient in number and capacity to maintain specified potentially hazardous food temperatures. Ensure that food contact equipment is maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines.

Violation: SLIGHT CORROSION WAS NOTED ON THE GRILL SURFACE.
Recommendation: Ensure that multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Recommendation: Ensure that cleaned equipment and utensils, laundered linens, and single-service and single-use articles are stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 150 mm (6 inches) above the deck. Clean the ventilation duct and cover.

Violation: THE WATER TEMPERATURE WAS MEASURED AT 134F AT THE HOT GALLEY HANDWASH STATION. THIS UNIT WAS ADJUSTABLE, HOWEVER THE TEMPERATURE REMAINED THE SAME FROM THE HOT TO COLD SETTING.
Recommendation: Ensure that handwashing sinks are equipped to provide water at a temperature of at least 43C (110F) through a mixing valve or combination faucet. For handwash sinks with electronic sensors, where the user cannot make temperature adjustments, ensure that the temperature provided to the user does not exceed 52C (125F).

Violation: TEN LIGHTS WERE OUT IN THE FROZEN MEAT WALK-IN REFRIGERATOR 4204. THIS WAS CORRECTED DURING THE INSPECTION.
Recommendation: Ensure that the light intensity is at least 110 Lux (10 foot candles) at a distance of 75 centimeters (30 inches) above the deck when in use, in walk-in refrigerator units and dry storage areas, and in other areas and rooms during periods of cleaning.

Violation: THERE WAS VISIBLE SOIL AND ORGANIC MATERIAL NOTED ALONG THE BLUE METAL COAMING AT THE SEMICIRCULAR SECTION OF THE ATRIUM FOUNTAIN. ORGANIC DEBRIS WAS NOTED BOTH ON THE STONE SIDE OF THE BLUE COAMING AND THE OPPOSITE SIDE AT THE DECK LEVEL.
Recommendation: Ensure that decorative fountains and water sprays in HVAC air- distribution systems are maintained free of algae and mold. Clean the atrium fountain base and coaming more thoroughly.
