Death Notifications

Notifications of new firefighter deaths should be submitted directly to the US Fire Administration by completing the Fallen Fire fighter Notification Form available on the USFA website.

Data Corrections

The data for the fatality map application are primarily derived from the USFA National Fallen Fire fighters Memorial Database. Incident data corrections or notification of errors should be sent directly to USFA (see the USFA Contact Us web page). You may also copy us on corrections or errors by using the email address provided above or the form below.
