Site: Medical-GI Logs
Violation: During the three voyages prior to the 15 March 2008 cruise, there were a few cases which met the case definition for gastrointestinal illness, but were recorded as non-reportable. The underlying conditions column listed diagnoses of "indigestion", "indigestion due to escargot" and "motion sickness" (with several episodes of diarrhea).
Recommendation: A reportable case of gastrointestinal illness shall be defined as: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period); or (2) Vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature >38 C (100.4 F)); and (3) Reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member. The log shall list: (1) All reportable cases of gastrointestinal illness; (2) All passengers and crew members who are dispensed antidiarrheal medication from the master of the vessel, the medical staff, or other designated staff.

Violation: The maintenance log did not indicate the chlorine concentration and contact time for potable water storage tank disinfection.
Recommendation: Ensure that documentation of any inspection, cleaning, disinfection (including concentration and contact time of disinfectant), and flushing is maintained for 12 months and is available to the VSP for review during inspections.

Violation: There was no indication in the written documentation for the tank paint that the manufacturer's application, drying and curing recommendations were followed.
Recommendation: Ensure that interior coatings on potable water tanks are approved for potable water contact, and all manufacturer's recommendations for application, drying, or curing are followed. Ensure written documentation for the coating used and recommendations followed are available for review during inspections.

Violation: There was no indication in the records that the system was flushed with potable water until the free residual chlorine was less than or equal to 5 ppm.
Recommendation: Ensure that the disinfected parts of the system are flushed with potable water until the free residual halogen is at or below 5.00 mg/L (ppm).

Recommendation: Ensure that equipment that is fixed because it is not easily movable is installed so that it is: (1) Spaced to allow access for cleaning along the sides, behind, under and above the equipment; (2) Spaced from adjoining equipment, bulkhead, and deckhead at a distance of not more than 0.8 millimeter or 1/32 inch; or (3) Sealed to adjoining equipment or bulkhead, if the equipment is exposed to spillage or seepage.

Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Violation: The IPM records indicated that there was a recurring drain fly infestation at the glass crusher every 2 weeks. The current pesticide application procedure appeared to be ineffective in controlling the drain flies.
Recommendation: Adjust the pest control procedures in the garbage room to effectively control their presence and reoccurrence.

Violation: The previously disinfected Lego pieces had not been separated prior to disinfection and water was noted in between the pieces.
