Site: Potable Water-Empress Forward Passenger Laundrette
Violation: The vent for the right backflow prevention device was pointing up. Additionally, the end of the lines connected to the vents for both of the backflow prevention devices could not be located.
Recommendation: Ensure that the vents point down. Locate the end of the vent drain lines in an area where they can be inspected.

Site: Potable Water-
Violation: There were water lines that were not potable water lines that were striped blue in the engine room. These lines were technical water lines, low temperature cooling water for the generator, preheating lines and precooling lines. Hot potable water lines were striped red and blue and there were air conditioning system water lines that had red and blue stripes.
Recommendation: Ensure that potable water piping and fittings are painted or stripped auxiliary blue, or in accordance with ISO 14726, at 5 m (15 feet) intervals on each side of partitions, decks, and bulkheads in all spaces, except where the decor would be marred by such markings. Do not stripe non-potable water lines with the same colors as potable water lines.

Violation: The column for the machinery (plumbing) connection in the cross-connection control program did not always list the connection. For some the information in this column was "backflow" or "air-gap". It was not clear from the listing what was connected to the potable water supply.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Violation: There were several small diameter potable water lines that were not striped blue. The lines were: water lines to sinks in boson's and joiner's workshops, and the hose connection in crew whirlpool machinery station,

Site: Potable Water-WTD #6 and Pump Room #1
Violation: There were two high pressure washing units (with pumps) connected to the potable water in the engine room. One of the lines was not fitted with a backflow prevention device while the other line was fitted with a non-continuous pressure-type backflow prevention device. Per the engineers, these two connections had been made by contractors the night before. The contractors were on the vessel to clean the grey water tank. The engineers stated that they would provide a connection to the technical water system for these two high pressure washing units.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer. Ensure that connections to the potable water system are protected with the appropriate backflow prevention device. Do not use atmospheric vacuum breakers or hose-bib vacuum breakers for connections that are subject to backpressure and continuous pressure Either identify connections for contractors to use or ensure that a knowledgeable crew member is present when connections to the potable water system are being made.

Violation: The potable water hose connection at the distribution system chlorine dosing station was not on the cross-connection control list. This was corrected.
Recommendation: Ensure that all of the connections to the potable water system are listed.

Violation: A backflow prevention device was not provided on the potable water line supplying the second from the left shower hose.

Violation: A review of the bunkering log showed that there were a few days when the final free chlorine residual test was conducted more than 1 hour prior to the stop of bunkering. Additionally, it was not clear if the stop time in the log corresponded with water flow to the tanks or disconnection of the hoses. There were several days when the last reading was recorded as exactly one hour prior to the stop time for bunkering.
Recommendation: Ensure that free residual halogen monitoring is performed at least hourly during the bunkering of potable water and performed at least once every 4 hours during the onboard production of potable water.

Site: Recreational Water Facilities-
Violation: The drain covers for the aft pool and crew pool were not stamped in accordance with ASME/ANSI A112.19.8M. There was no letter for drain covers.
Recommendation: Ensure that anti-entrapment drain covers or other drains that prevent entrapment hazards as specified in U.S. Consumer Product Safety Publication 363-009801 (dual drains/channel drains) are provided on swimming pools and whirlpool spas. Ensure that drain covers have either the plumbing/engineering approving organization stamp and flow rate affixed to the cover, or that a letter is maintained aboard the vessel certifying that the cover meets the safety requirements outlined in AMSE/ANSI A112.19.8M or an equivalent standard.

Violation: The records for interviews of close contact of ill crew members showed either dates that were all the same for the interviews or dates where the initial interview was after the 24-hour interview and on the same date as the 48-hour interview. When the contact record was opened during the inspection the initial interview date field was populated with the date of the inspection.
Recommendation: Ensure that the correct date and times are recorded for the interviews of close contacts of ill crew members.

(2) That are scaled only in Fahrenheit are designed to be accurate to 3F in the intended range of use.
