Site: Galley-Combi-oven #1005
Violation: The backflow prevention device was continuously leaking.
Recommendation: Ensure that backflow preventers are maintained in good repair.

Site: Pantry-Schooner Bar
Violation: The backflow prevention device on the coffee machine was a non-continuous pressure type unit.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Site: Potable Water-
Violation: The list of backflow prevention devices for Facilities indicated that the devices on the washing machines in the crew launderettes were non-continuous pressure. The device appeared to be a continuous pressure device. The same device was installed on the hose at the sink in the photo shop. This hose had a sprayer attached to the end of the hose. For both of these applications, a continuous pressure type backflow prevention device is required due to the downstream valve.
Recommendation: Investigate the type of backflow prevention device that is installed on the washing machines in the crew launderettes and on the hose in the photo shop. Ensure that it is a continuous pressure device and update the list of backflow prevention devices.

Site: Potable Water-Cross-connection Control Records
Violation: The list of non-testable backflow prevention devices for the Engineering Department indicated "Type B9" for each of the locations. The installed devices appeared to be Watts N9.
Recommendation: Ensure that the correct devices are in the list of backflow prevention devices.

Site: Other-Officer's Mess - Buffet Line
Violation: Reach-in refrigerator #1015 was posted out of order for the past 1 week. This is the only refrigeration unit in the area.
Recommendation: Repair or replace the unit to ensure proper cold holding capacity in this area.

Site: Galley-Deck 5
Violation: Crisp bacon was placed in the far right, upper alto-sham unit for hot holding two hours prior to the inspection. The unit's exterior mounted ambient air thermometer indicated an internal temperature of 187F. A second, internal, ambient air thermometer registered 140F. The temperature of the crisp bacon was measure at 95F. The crisp bacon is not a potentially hazardous food.
Recommendation: Ensure that ambient air temperature measuring devices are maintained in good repair and are accurate within the intended range of use.

Violation: Peeling labels were noted on flat-top grill #1009, making cleaning difficult.
Recommendation: Remove the peeling labels.

Violation: In the technical compartment under the speed rail near the handwash station, the old multi-flow lines were cut, leaving multiple opening that were difficult to clean.
Recommendation: Remove the un-unused multi-flow lines and seal the back of the technical compartment once the hoses are removed.

Violation: Utility lines were installed on the floor of the technical space, obstructing cleaning.
Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Buffet-Lido-Starboard Side Forward
Violation: The utility line clamps. located in the technical space, were galvanized metal and heavily corroded. This is the result of utilizing dissimilar metals.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: The sneeze guard over-shelf had excessive sealant, making the surface difficult to clean.
Recommendation: Remove sealant to allow proper cleaning.

Violation: There is a void space behind and below the undercounter Alto-Sham units, creating a space that cannot be cleaned and allows for possible pest harborage.
Recommendation: Ensure that equipment that is fixed because it is not easily movable is installed so that it is: (1) Spaced to allow access for cleaning along the sides, behind, under and above the equipment; (2) Spaced from adjoining equipment, bulkhead, and deckhead at a distance of not more than 0.8 millimeter or 1/32 inch; or (3) Sealed to adjoining equipment or bulkhead, if the equipment is exposed to spillage or seepage.

Violation: During operation, water was spraying from the potwash machine door frame and accumulating on the deck.
Recommendation: Repair/install door seals to prevent discharge from the unit.

Violation: There were recently washed new souvenir soda containers in the clean storage rack. The containers were inverted, but the lids had been attached, impeding air drying.
Recommendation: After cleaning and sanitizing, ensure that equipment and utensils are air-dried or adequately drained before contact with food.

Site: Provisions-Dairy Walk-in Refrigerator #1878
Violation: The top of the starboard door was damaged, making cleaning difficult. The bottom, interior portside door molding was separating, leaving openings, making cleaning difficult.
Recommendation: Repair the door and secure or replace the molding as needed.

Violation: The vertical edge of the counter was separated at the deck juncture, creating a gap.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas, are maintained in good repair.

Violation: The water level tubing between the water bath trays was torn, allowing potable water to flow into the ice bin below. The ice bin was empty.
Recommendation: Replace the tubing.

Violation: According to the pest management logs, the passive and active night time inspections were conducted as early as 3:30 pm.
Recommendation: Ensure that night time inspections are conducted during night time hours.
