Violation: The sample collection containers for bacterial analysis had expired on 03/2007.
Recommendation: Replace the bacterial sample collection containers.

Site: Potable Water-Potable Water - Tanks
Violation: There was a cleaning, partial re-surfacing, and disinfection of potable water tanks 1 port and 1 starboard on 20 June 2007. The written record of the activity did not state that the coating system used was applied, cured, and dried according to the coating manufacturer's specifications. VSP reviewed the approval of the coating products.
Recommendation: Ensure that interior coatings on potable water tanks are approved for potable water contact, and all manufacturer's recommendations for application, drying, or curing are followed. Ensure written documentaion for the coating used and recommendations followed are avaialble for review during inspections.

Site: Potable Water-Cross-Connection Control Program
Violation: There were a number of backflow preventers listed and inspected in July for the various passenger launderette's on decks 1, 4-7, and crew launderettes on decks A and B. The backflow preventers were not continuous pressure models. In inspections of the launderettes, the continuous pressure preventers were installed on the hot and cold potable water pipes in the deckheads above, but were neither listed or inspected as the others were.
Recommendation: Ensure that the appropriate backflow preventer is identified and listed for inspection. Ensure the listed preventers are inspected.

Site: Potable Water-Cross-Connection Control Program
Violation: The Wilkins backflow preventers installed in the deckhead of the deck A forward crew launderette were oriented with the intermediate vent pointed upwards. Additionally, the cold water backflow preventer was leaking continuously as observed at the drain pipe connected at the sink drain.
Recommendation: Ensure that backflow prevention devices are periodically inspected and any failed units are replaced.

Violation: The passenger launderette on deck 8 forward had a cold water pipe backflow preventer above the deckhead leaking continuously into the sink drain adjacent.
Recommendation: Ensure that backflow prevention devices are periodically inspected and any failed units are replaced. Ensure that backflow preventers are maintained in good repair.

Violation: A crew member who works in the pastry reported to medical on June 29th, his last symptom was recorded as June 30th and he was released from isolation on July 1st, only 24 hours following his last symptom.
Recommendation: Ensure that symptomatic food employees meeting the case definition for AGE are isolated in cabin or designated, restricted area until symptom-free for a minimum of 48 hours.

Site: Galley-Deck 1 Appetizer Preparation
Violation: The pipe penetrations were not sealed on the right side of the technical compartments for undercounter refrigeration units 4811.011.092 and 4811.011.094. The open area makes cleaning difficult and allows for insect harborage.
Recommendation: Seal the pipe penetrations.

Violation: There were two round holes in the wall of the milk compartment of the WMF coffee machine, exposing the foam insulation and making cleaning difficult.

Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Violation: Under the speed rail near the midship pool, the center back area of the floor of the technical space was soiled with a black material.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: The handwash station in this pantry was posted out of order, with a sign stating "no spare parts onboard". The requisition order #008796 was also on the sign. There was clean dishware on the clean racks and a full ice bin, indicating the pantry was still in use by staff.
Recommendation: Ensure that handwashing facilities are kept clean and in good repair.
