Site: Medical-Medical - GI Case Reports
Violation: For the voyage ending 17 November 2007, one passenger case that was listed in the GI log, was not reported on the 24-hour report. When asked why the case was not included in the 24-hour report, the doctor indicated that the onset of illness occurred before the voyage (08 November 2007). The case reported to the medical infirmary on 11 November 2007, the start of the voyage. The case met the definition of a reportable case.
Recommendation: Ensure that the master, the medical staff, or other designated staff of a vessel destined for a U.S. port from a foreign port submits at least one standardized gastrointestinal illness report based on the number of reportable cases in the gastrointestinal illness log to the VSP no less than 24 hours, but not more than 36 hours before the vessel's expected arrival at the U.S. port.

Site: Medical-Medical - Microbiologic Monitoring
Violation: The ship staff is collecting and analyzing potables water samples in ports (bunkering), storage tanks and at various points in the distribution system. The ship staff is using the Colisure test kit. However, some of the test results indicate the sample was positive or negative without reference as to whether the sample results were for E. coli.
Recommendation: Ensure that a minimum of four potable water samples per month are collected and analyzed for the presence of Escherichia coli and are collected randomly from locations on the forward, aft, upper, and lower decks of the vessel. Ensure that sample sites are changed each month.

Site: Medical-Medical - Asymptomatic Contacts of GI Cases
Violation: There was no documentation of the 48-hour follow-up of the asymptomatic contacts of crew GI case-patients. In accordance with corporate policy, the ship is isolating close contacts of GI case-patients for 24-hours. The initial and 24-hour follow-ups are documented in the medical records of the asymptomatic close contacts.
Recommendation: Ensure that medical staff conduct daily verbal interviews with asymptomatic crew until 48 hours after onset of the ill crew members symptoms.

Site: Other-Deck 10 Concierge Lounge
Violation: The self-service pitcher of skim milk and full cream milk had a small green dot sticker affixed as part of the time as a public heatlh control program. The dot color chart indicated that the green sticker was to be used from 1130 - 3:30 or 12- 4 pm. A red sticker was to be used from either 0730-1130 or 0800-1200. The time of the inspection was 10:55 am. Staff immediately discarded the two pitchers.

Site: Buffet-Windjammer - Port/Aft Fruit Buffet Station
Violation: There was no staff monitor present at the port aft fruit buffet, and there was an open access for passengers to enter the buffet from the service side, which was not shielded.

Violation: The self-service tongs were 12 inch length for most buffet stations, but for those with the larger square food containers this was too short and tongs were found with handles contacting foods like honeydew melons and bread/rolls.

Site: Housekeeping-Housekeeping - General
Violation: Most of the ice buckets used in passenger cabins had open holes in the bottom exterior surface, where plugs were missing. This issue was being addressed by corporate RCI, and the ship has replaced 456 of these cabin ice bins to date. Staff stated that this is in process and the remainder are expected throughout the year.

Site: Galley-Main Galley - Deck 5 - Tourant Station
Violation: The gasket on the lid of the tilting pan was torn, making cleaning difficult. This item was corrected during the inspection.
Recommendation: Repair or replace the torn gasket.

Violation: The lower gasket on the inside of the door to the milk dispenser was damaged and separating, leaving an area where spilled milk could collect, making cleaning difficult.
Recommendation: Repair the lower gasket on the milk dispenser.

Site: Galley-Crew Galley
Violation: Two of four flat-top cooking surfaces were warped, exposing the technical elements, leaving gaps were food and debris could collect, making cleaning difficult. The control panels to the damaged units were tagged out of service. The ship is awaiting spare parts from Europe.
Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Galley-Main Galley - Deck 5 - Dishwash Area
Violation: The final rinse pressure gauge on the in-use glasswasher registered 10 psi.
Recommendation: Adjust the final rinse pressure to between 15-25 psi or replace the final rinse pressure gauge.

Violation: The sanitizer bucket concentration was measured at less than 50 ppm. This item was corrected during the inspection.
Recommendation: Ensure that a chlorine solution used as a sanitizing solution has a concentration between 50 mg/L (ppm) and 200 mg/L (ppm).

Site: Buffet-Windjammer - Soup, Milk, and Yogurt Buffet Station
Violation: There was a soiled cutting board found stored inside the undercounter technical compartment at the closed end of the buffet station. The board was removed immediately for washing.

Violation: Four previously cleaned packaged sugar containers were found soiled and stored ready for use. This item was corrected during the inspection.

Violation: There was heavy soil of old beverage residue on the insulation material surrounding the beverage tubing in the undercounter technical compartment at the starboard beverage station. Additionally, there was some old juice residue accumulating on the lower technical compartment directly below the beverage tubing bundle penetration above.

Violation: The deck to bulkhead aft juncture was not coved in the port or starboard side scrapping stations in the Windjammer dining room.

Violation: Recessed or missing grout was noted between some deck tiles in this area.

Violation: A potable water leak was noted in the technical compartment beneath the steam table. This item was corrected during the inspection.

Site: Galley-Main Galley - Deck 4 - Dishwasher Area
Violation: An excessive amount of condensate was noted above the center of the conveyor dishwasher. Steam was observed escaping from the side of the conveyor dishwasher, but there did not appear to be adequate exhaust ventilation in the area.
Recommendation: Ensure that ventilation hood systems and devices are sufficient in number and capacity to prevent grease or condensation from collecting on bulkheads and deckheads.

Site: Galley-Windjammer Galley - Cold Pantry
Violation: A house fly was observed on the counter and on container lids of cut fruit and melon in the cold pantry. This was also noted on the previous inspection of 13 January 2007.
