Violation: FOR THE 2/17-2/24 CRUISE THE VESSEL REPORTED 36 HOURS PRIOR TO ARRIVAL IN THE US FROM A FOREIGN PORT. FOR THE 1/27-2/3 CRUISE THE VESSEL DID NOT MAKE A 4 HOUR PRIOR TO ARRIVAL REPORT WHEN ONE ADDITIONAL CASE DEVELOPED AFTER THE 24 HOUR PRIOR TO ARRIVAL REPORT WAS MADE.
Recommendation: 4.2.1.1.1 The master, the medical staff, or other designated staff of a vessel destined for a U.S. port from a foreign port shall submit at least one standardized gastrointestinal illness report based on the number of reportable cases in the gastrointestinal illness log to the VSP no less than 24 hours, but not more than 30 hours before the vessel's expected arrival at the U.S. port. 4.2.1.1.2 If the number of cases changes after submission of the initial report, an updated report shall be submitted no less than 4 hours before the vessel's arrival at the U.S. port.

Violation: THE LOG DID NOT INCLUDE THE TIME PERIOD THAT SPAS WERE OUT OF SERVICE WHEN HALOGEN LEVELS WERE TOO LOW, AND THE CORRECTIVE ACTION TAKEN.
Recommendation: 6.2.2.2.6 Logs and charts shall contain notations of any unusual water events with the swimming pool operation and corrective actions taken.

Violation: EMPLOYEE OBSERVED WASHING HANDS OFF IN A DUMP SINK WHERE UTENSILS WERE RINSED BETWEEN DRINKS.
Recommendation: 7.2.3.1.2 Food employees shall clean their hands and exposed portions of their arms with a cleaning compound in a handwashing sink by vigorously rubbing together the surfaces of their lathered hands and arms for at least 20 seconds and thoroughly rinsing with clean water. Employees shall pay particular attention to the areas underneath the fingernails and between the fingers. Handwashing should be conducted only in designated hand washing facilities.

Violation: TOPPINGS USED FOR PIZZAS AND INDIVIDUAL PIZZA SLICES WERE NOT INCLUDED IN THE PLAN FOR TIME CONTROL FOR FOOD PROTECTION.
Recommendation: 7.3.5.3.5 If time only, rather than time in conjunction with temperature, is used as the public health control for a working supply of potentially hazardous food before cooking, or for ready-to-eat potentially hazardous food that is displayed or held for service for immediate consumption: (1) The food shall be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control; (2) The food shall be cooked and served, served if ready-to-eat, or discarded, within 4 hours from the time when the food is removed from temperature control; and (3) The food in unmarked containers or packages or marked to exceed a 4 hour limit shall be discarded.

Violation: THE TIME AS A CONTROL PLAN SHOULD BE MADE CLEARER. AN ADDITIONAL STEP OF BLAST CHILLING FOODS IS CONDUCTED FOR COLD SALAD, COLD CUTS AND CHEESES. THIS STEP WAS NOT INDICATED ON THE PLAN(S). THE BLAST CHILLER LOG NOTED ENTRIES OF 120F, 100F AND 130F FOR HOT POTENTIALLY FOODS PLACED INTO THE BLAST CHILLER. THE COOLING OF THESE FOODS FROM `140F TO THESE TEMPERATURES WAS NOT MONITORED. ALL OF THE FOODS WERE NOTED TO HAVE COOLED TO BELOW 41F WITHIN 2 HOURS.
Recommendation: Clarify the steps followed on the vessel and place them in the time as a control plan. Begin monitoring the cooling process once the foods have reached 140F.

Violation: GROUND BEEF WAS STORED OVER STEAKS IN THE THAW ROOM.
Recommendation: 7.3.3.2.1.1 Food shall be protected from cross-contamination by: (2) Separating types of raw animal foods from each other such as beef, fish, lamb, pork, and poultry during storage, preparation, holding, and display by using separate equipment for each type, or arranging each type of food in equipment so that cross-contamination of one type with another is prevented, and preparing each type of food at different times or in separate areas.

Violation: BANANAS AND ONIONS WERE STORED IN THE PROVISIONS CORRIDOR. THERE WERE SEVERAL PIPES OVERHEAD WITH PRESS FIT PIPING THAT COULD NOT IMMEDIATELY BE IDENTIFIED.
Recommendation: 7.3.3.5.1 Food shall be protected from contamination by storing the food: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

Recommendation: Either place food items in an area not subject to potential contamination via overhead waste water piping, or utilize sleeve welded piping or solid stainless steel pipelines in areas above food storage.

Recommendation: 7.4.1.1.2 Materials that are used in the construction of multiuse utensils and food-contact surfaces of equipment shall be: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.

Violation: NUMEROUS SOILED ITEMS WERE NOTED IN CLEAN STORAGE. THIS WAS NOTED BOTH IN THE CLEAN ROOM AND THE CLEAN STORAGE RACKS IN THE DISHWASH ROOM. IN ADDITION, SOILED WATER WAS NOTED IN SEVERAL UPRIGHT GLASSES IN STORAGE RACKS. SOME OTHER GLASSES WERE ALSO NOTED TO BE UPRIGHT. THESE GLASSES DID NOT FIT IN THE RACKS WHEN INVERTED. IT APPEARED THAT THE GLASSES WERE PASSED THROUGH THE GLASSWASH UNIT UPRIGHT. THESE GLASSES WERE NOTED TO BE SOILED.

Recommendation: 9.2.1.2.3 Vessels shall have a plan to inspect and maintain heating, ventilation, and air conditioning systems in accordance with manufactures recommendations and industry standards. All standing water should be removed from air handling rooms.
