Violation: WATER WAS BUNKERED IN JACKSONVILLE DURING THE MONTHS OF FEBRUARY, MARCH, AND APRIL OF 2004. THE ONLY BACTERIOLOGICAL SAMPLE RESULT FOR THIS PORT WAS FROM OCTOBER 01, 2003.
Recommendation: Ensure that bacteriological reports are not more than 1 month old.

Site: Other-POTABLE WATER
Violation: THE ONLY RECORDS AVAILABLE FOR THE CLEANING AND DISINFECTION OF THE POTABLE WATER TANKS WAS FOR TANKS 4P AND 4S. THESE TANKS WERE CLEANED AND DISINFECTED ON SEPTEMBER 2, 2004.
Recommendation: 5.3.4.1.3 Documentation of the cleaning shall be maintained for 12 months and shall be available to the VSP for review during inspections. 5.3.4.1.1 Potable water tanks and any parts of the potable water distribution system shall be cleaned, disinfected, and flushed with potable water: (1) Before being placed in service; and (2) Before returning to operation after repair, replacement; or (3) Being subjected to any contamination, including entry into a potable water tank. 5.3.4.1.4 Disinfection following potential contamination shall be accomplished by increasing free residual halogen to at least 50 mg/L (ppm) throughout the affected area and maintaining this concentration for 4 hours.

Site: Other-POTABLE WATER
Violation: A DOUBLE CHECK VALVE ASSEMBLY WAS INSTALLED ON THE REVERSE OSMOSIS UNIT. ACCORDING TO THE STAFF, THIS DEVICE WAS FOR THE BACKWASHING OF THIS UNIT. AFTER SOME DISCUSSION, THE STAFF INDICATED THAT THIS DEVICE WAS NOT NEEDED AS THE SYSTEM WAS NOT UNDER PRESSURE. IN ADDITION TO BACKWASHING, CHEMICALS WERE USED TO CLEAN THE REVERSE OSMOSIS SYSTEM. IT WAS NOT CLEAR FROM THE STAFF'S EXPLANATION WHY THIS DEVICE WAS INSTALLED IF IT WAS NOT NEEDED.
Recommendation: If a backflow prevention device is necessary for this unit replace the double check valve assembly with a device that has a vent to the atmosphere. If this device is not necessary provide a clear explanation as to why it is not needed. Drawings included with the explanation would be appreciated.

Violation: THE POTABLE WATER LINES IN THE MAIN LAUNDRY ROOM WERE NOT STRIPED BLUE.
Recommendation: 5.3.2.1.1 Potable water piping shall be painted light blue or striped with 15 cm (6 inches) light blue bands or a light blue stripe at fittings on each side of partitions, decks, and bulkheads and at intervals not to exceed 5 m (15 feet) in all spaces, except where the decor would be marred by such markings.

Recommendation: Replace the missing dataplate. 7.5.2.2.3 A warewashing machine shall be provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.
