Violation: GOOD MANAGEMENT OF GASTROINTESTINAL ILLNESS DATA AND IMPLEMENTATION OF THE ANTIDIARRHEAL MEDICATION LOG BOOK.

Site: Other-WATER ANALYSIS
Violation: THE BACTI-ANALYSIS EQUIPMENT USED ON THIS VESSEL DOES NOT MEET THE REQUIREMENTS OUTLINED IN Chapter 5.6.1.1.2.
Recommendation: A minimum of four potable water samples per month shall be collected and analyzed for the presence of Escherichia coli. Samples shall be collected randomly from locations forward, aft, upper, and lower decks of the vessel. This method must comply with reference 5.6.1.1.2 (Standard Methods for Water Quality).

Violation: A BACKFLOW PREVENTER WAS LEAKING ON THE COUNTER.
Recommendation: The potable water distribution system shall be maintained free of cross-connections with non-potable piping systems and tanks.

Violation: A COMPREHENSIVE BACKFLOW PREVENTION CONTROL PROGRAM WAS NOT IN PLACE.
Recommendation: The vessel shall provide a comprehensive cross- connection control program that provides safe connections to the potable water system through air gaps or appropriate backflow devices at the following locations, if present: (1) Potable water supply lines to swimming pools, whirlpool spas, hot tubs, bathtubs, showers, and similar facilities; (2) Photographic laboratory developing machines and utility sinks; (3) Beauty and barber shop spray-rinse hoses; (4) Potable water faucets where hoses are connected or can be connected by threaded or quick-connect outlets such as those serving tanks containing chlorine and other chemicals, and deck taps; (5) Garbage grinders and pulpers; (6) Mechanical warewashing machines; (7) Hospital and laundry equipment; (8) Air conditioning expansion tanks; (9) Boiler feed water tanks; (10) Fire systems; (11) Toilets; (12) Potable water, bilge, and sanitary pumps that require priming; (13) Freshwater or saltwater ballast systems; (14) Bilge or other waste water locations; (15) International shore connection; and (16) Any other connection between potable and non-potable water systems.

Site: Other-SWIMMING POOLS
Violation: DEPTH MARKERS SHALL BE INSTALLED EVERY METER AND DISPLAYED PROMINENTLY. THERE WILL ALSO BE APPROPRIATE SAFETY EQUIPMENT PRESENT AT ALL POOLS.
Recommendation: Depth markers shall be installed for every 1 m (3 feet) in change of depth and shall be displayed prominently so they can be seen from the deck and from in the pool. Easy access shepherd's hook and approved floatation device shall be provided at a prominent location near each public swimming pool.

Violation: THE SAFETY SIGNS DID NOT INCLUDE LANGUAGE TO WARN IMMUNO-COMPROMISED BATHERS.
Recommendation: A sign shall be installed near the whirlpool spas that lists standard safety precautions and risks, warning against use by particularly susceptible people, such as those who are immunocompromised.

Violation: THERE WAS NO CONSUMER ADVISORY FOR UNDER COOKED FOODS.
Recommendation: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish that is raw, undercooked, or not otherwise processed to eliminate pathogens is offered in a ready-to-eat form or as a raw ingredient in another ready-to-eat food, the passengers shall be informed by vessel newsletter articles, brochures, embarkation television broadcasts, menu advisories, placards, or other written means of the significantly increased risk to certain especially vulnerable consumers eating such foods in raw or undercooked form.

Recommendation: Potentially hazardous food shall be maintained: (1) At 60C (140F) [roasts 54C (130F)] or above,; or (2) At 5C (41F) or less.

Violation: THE PLASTIC SHAFT ON THE VERTICAL CHOPPER WAS DIFFICULT TO CLEAN.
Recommendation: Materials that are used in the construction of multiuse utensils and food-contact surfaces of equipment shall be: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.

Violation: SOME OF THE GLASSWASHERS UPPER SPRAY ARM NOZZLES WERE CLOGGED. THE SPRAY ARM NOZZLE COULD NOT BE REMOVED WITHOUT USING A WRENCH.
Recommendation: Warewashing equipment shall be maintained in good repair and proper adjustment including: (1) Warewashing equipment shall be maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines. (2) Water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use.

Violation: THE FINAL RINSE MANIFOLD TEMPERATURE EXCEEDED 194 DEGREES F.
Recommendation: The temperature of the final rinse should not exceed 180 degrees F. Higher temperatures will not provide adequate sanitizing and produces steam that leads to condensation build-up.

Violation: THE PRESSURE OF THE FINAL RINSE MANIFOLD OF THE GLASS WASHER WAS LESS THAN 5 PSI. THE UPPER MANIFOLD DID NOT HAVE A SPRAY PATTERN.
Recommendation: Pressure measuring devices that display the pressures in the water supply line for the fresh hot water sanitizing rinse shall have increments of 7 kilopascals (1 pounds per square inch) or smaller and shall be accurate to 14 kilopascals ( 2 pounds per square inch) in the 100-170 kilopascals (15-25 pounds per square inch) range.

Violation: A WET RAG WAS NOTED ON THE HANDWASH SINK.
Recommendation: Wiping cloths shall be restricted to the following: (3) Dry or wet cloths that are used with raw animal foods shall be kept separate from cloths used for other purposes, and wet cloths used with raw animal foods shall be kept in a separate sanitizing solution.

Recommendation: Poisonous or toxic materials shall be applied so that: (1) A hazard to employees or other persons is not constituted, and (2) Contamination including toxic residues resulting from drip, drain, fog, splash or spray on food, equipment, utensils, linens, and single-service and single-use articles is prevented. The maximum concentration for cleaning is 100 mg/l (PPM).

Violation: THERE WAS NOT A COMPREHENSIVE INTEGRATED PEST MANAGEMENT PLAN ON THE VESSEL.
Recommendation: Each vessel shall develop an Integrated Pest Management Plan to address effective monitoring and control strategies for pests aboard the vessel.

Violation: THERE WERE NO WRITTEN PROCEDURES ON INFECTIOUS ILLNESSES. THERE WAS ALSO NO CHILD SIZE TOILET PRESENT.
Recommendation: Written guidance on symptoms of common childhood infectious illnesses shall be maintained in the child-activity center. Child-size toilet and handwashing facilities shall be provided, if toilet rooms are located in a child-activity center.
