Violation: VEGETABLE PATTIES IN THE COMBI-OVEN WERE AT 92 F. NO WRITTEN TIME PLAN WAS AVAILABLE.
Recommendation: 7.3.5.3.1.1 Potentially hazardous food shall be maintained: (1) At 60C (140F) [roasts 54C (130F)] or above,; or (2) At 5C (41F) or less. 7.3.5.3.5 If time only, rather than time in conjunction with temperature, is used as the public health control for a working supply of potentially hazardous food before cooking, or for ready-to-eat potentially hazardous food that is displayed or held for service for immediate consumption: (1) The food shall be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control; (2) The food shall be cooked and served, served if ready-to-eat, or discarded, within 4 hours from the time when the food is removed from temperature control; and (3) The food in unmarked containers or packages or marked to exceed a 4 hour limit shall be discarded.

Recommendation: 7.4.2.4.9.1 Temperature measuring devices shall conform to the following guidelines: (1) In a mechanically refrigerated or hot-food storage unit, the sensor of a temperature measuring device shall be located to measure the air temperature in the warmest part of a mechanically refrigerated unit and in the coolest part of a hot-food storage unit.

Violation: THE DATA PLATES FOR THE LARGE CONVEYOR DISH WASHERS DID NOT CONTAIN THE CONVEYOR SPEED.
Recommendation: 7.5.2.2.3 A warewashing machine shall be provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Violation: ENSURE THAT THERE IS A WRITTEN GUIDANCE ON THE SYMPTOMS OF COMMON CHILDHOOD INFECTIOUS ILLNESSES IN THE CENTER.
Recommendation: 10.4.1.1.2 The child-activity center shall have a written policy on procedures to be followed when a child develops symptoms of an infectious illness while at the center.
