Violation: ON THE FOLLOWING CRUISES, GASTROINTESTINAL ILLNESS REPORTS WERE NOT MADE IN A TIMELY MANNER: CRUISE BEGINNING 6/22/03, THE 4 HOUR REPORT WAS SENT AT 7:42 AM ON 6/29/03, WHILE THE SHIP ARRIVED INTO PORT AT 6:00 AM; CRUISE BEGINNING 6/29/03, THE 24 HOUR AND 4 HOUR REPORTS WERE SENT AT 4:17 AM ON 7/7/03, WHILE THE SHIP ARRIVED INTO PORT AT 7:00 AM ON 7/6/03; CRUISE BEGINNING 7/13/03, THE 24 HOUR REPORT WAS SENT ON 7/19/03 AT 6:00 PM, WHILE THE SHIP ARRIVED INTO PORT ON 7/20/03 AT 6:00 AM. THE CDC AUTOMATED REPORTING SYSTEM WAS EXPERIENCING INTERMITTENT ONLINE PROBLEMS BUT THE VESSEL DID NOT HAVE A SYSTEM IN PLACE TO ENSURE THAT THE REPORTS WERE RECEIVED WITHIN THE SPECIFIC TIME REQUIREMENTS. AS WAS STATED BY THE NURSE, THE COMPUTER IS PROGRAMMED TO SEND THE 24 HOUR REPORTS AT 6:00 AM ONE DAY PRIOR TO ARRIVAL AND THE 4 HOUR REPORTS, IF NECESSARY, AT 2:00 AM THE DAY OF ARRIVAL. SHE ALSO STATED THAT SOMETIMES THERE IS A PROBLEM WITH THE COMPUTERS OR THE SATELLITE THAT WOULD PREVENT THE REPORTS FROM BEING SENT ON TIME.
Recommendation: 4.2.1.1.1 The master, the medical staff, or other designated staff of a vessel destined for a U.S. port from a foreign port shall submit at least one standardized gastrointestinal illness report based on the number of reportable cases in the gastrointestinal illness log to the VSP no less than 24 hours, but not more than 30 hours before the vessel's expected arrival at the U.S. port.

Recommendation: 6.2.2.1.1 A free residual halogen of >1.0 and <3.0 mg/L (ppm) shall be maintained in recirculated swimming pools.

Violation: THE PH WAS RECORDED AT 1.2-1.4 FOR THE COMPLETE TIME THAT THE SPA WAS OPEN ON 4/18/03. DURING THE CLOSING CONFERENCE IT WAS DETERMINED THAT THE HOUSEKEEPING AND ENGINEERING DEPARTMENTS BOTH KEEP RECORDS OF THE POOLS AND SPAS. THE HOUSEKEEPING RECORD FOR 4/18/03 SHOWED THE PH LEVEL WITHIN RANGE BUT SHOWED THE FREE CHLORINE RESIDUAL AT 5 PPM.
Recommendation: 6.3.1.2.2 The whirlpool spa water shall be maintained with a Ph between 7.2 and 7.8. Recommend that if two separate set of records are maintained that they show similar results or investigate the reason they do not. Ensure that the pool/spa free chlorine/bromine and ph values recorded are correct and accurate.

Recommendation: 7.4.2.1.1 Food contact surfaces of equipment and utensils shall be designed and constructed to be durable and to retain their characteristic qualities under normal use conditions. 7.4.2.2.1 Multiuse food-contact surfaces shall be: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches. 7.4.2.5.1 Food-contact surfaces of food equipment shall comply with American National Standards Institute (ANSI) or other internationally accredited food-equipment sanitation standards for materials, design, and construction.

Violation: THE HOBART DISHWASH UNIT DID NOT HAVE A PRESSURE GAUGE INSTALLED ON THE FINAL RINSE LINE. THE PRESSURE GAUGE WAS INSTALLED DURING THE INSPECTION BUT WAS NOT VISIBLE.
Recommendation: 7.5.2.2.7 Warewashing machines that provide a fresh hot water sanitizing rinse: (1) Shall be equipped with a pressure gauge or similar device such as a transducer that measures and displays the water pressure in the supply line immediately before entering the warewashing machine; and (2) If the flow pressure measuring device is upstream of the fresh hot water sanitizing rinse control valve, the device shall be mounted in a 6.4 mm (1/4 inch) Iron Pipe Size (IPS) valve. 7.5.6.1.3 The flow pressure of the fresh hot water sanitizing rinse in a warewashing machine may not be less than 100 kilopascals (15 pounds per square inch) or more than 170 kilopascals (25 pounds per square inch) as measured in the water line immediately downstream or upstream from the fresh hot water sanitizing rinse control valve.

Violation: THE TOYS ARE DISINFECTED DAILY WITH # 15 BUT ARE NOT CLEANED BEFORE THEY ARE DISINFECTED. TOYS ARE WASHED, RINSED AND DISINFECTED ONCE PER WEEK.
Recommendation: 10.3.1.2.2 Toys used in the child-activity center shall be cleaned and disinfected daily. Ensure that toys are cleaned prior to disinfecting. Disinfectants can not penetrate debris, therefore in order to disinfect the toys they would need to be cleaned first.
