Violation: THE GASTROINTESTINAL ILLNESS REPORT FOR CRUISE DATES 02/29/2004 THROUGH 03/07/2004 WAS SUBMITTED AS A 4 HOUR REPORT THAT COMBINED THE 24 HOUR REPORT AND THE FOUR HOUR REPORT. THE REPORT WAS SUBMITTED ON 03/06/2004 AT 6:55 PM AND THE SAN JUAN PORT OF CALL WAS ON 03/07/2004 AT 6 AM
Recommendation: 4.2.1.1.1 The master, the medical staff, or other designated staff of a vessel destined for a U.S. port from a foreign port shall submit at least one standardized gastrointestinal illness report based on the number of reportable cases in the gastrointestinal illness log to the VSP no less than 24 hours, but not more than 30 hours before the vessel's expected arrival at the U.S. port.

Violation: THE CHARTS FOR THE PRODUCTION WATER ANALYZER/RECORDER INDICATED THAT FOR SEVERAL RECENT DAYS THE HALOGEN LEVEL WAS NOT AT LEAST 2.0 PPM. HOWEVER, THE MANUAL READINGS THAT WERE DONE EVERY 4 HOURS INDICATED READINGS ABOVE 2.0 PPM.
Recommendation: Ensure that the two separate record keeping logs are accurate and reflect the actual readings during the production of potable water.

Violation: THE CHARTS FOR THE PRODUCTION AND BUNKER ANALYZER/RECORDERS DID NOT INDICATE THAT THEY WERE CALIBRATED AT THE START OF OPERATIONS.
Recommendation: Verify, and if necessary calibrate the halogen analyzers at the beginning of production and bunkering of potable water and record this either on the charts or in a log.

Violation: IN THE BELL BOX ON DECK 4, A TRAY OF 20 CONTAINERS OF YOGURT HAD AN INTERNAL TEMPERATURE OF 45 F. A LARGE PLASTIC CONTAINER OF INDIVIDUAL CREAMERS HAD A TEMPERATURE OF 58 F IN THE REACH-IN REFRIGERATION UNIT. BELL BOX STAFF PICKED UP THE PRODUCTS FROM THE PROVISION REFRIGERATION UNITS ONE HALF HOUR EARLIER.
Recommendation: 7.3.5.2.5.1 Cooling shall be accomplished using one or more of the following methods based on the type of food being cooled: (1) Placing the food in shallow pans; separating the food into smaller or thinner portions; using blast coolers, freezers, or other rapid cooling equipment; stirring the food in a container placed in an ice water bath; using containers that facilitate heat transfer; adding ice as an ingredient; or other effective methods.

Violation: MANY OF THE BARS WITH BEER TOWERS INSTALLED WERE STORING CLOSED AND OPEN BOTTLES OF WINE, FROZEN DRINK MIXES, AND WRAPPED CONDIMENTS IN THE BEER KEG REFRIGERATORS THAT HAD KEGS AND CARBONATION CYLINDERS ALSO IN THE REFRIGERATORS. THIS INCREASES THE POSSIBILITY OF CONTAMINATING THE FOOD CONTAINERS AND THE FOOD STORED IN THESE REFRIGERATORS.
Recommendation: Discontinue storing items other than beer kegs and the required carbonation cylinders together in these refrigerators.

Violation: THE DISPENSING MECHANISM FOR THE SELF-SERVICE ICE AND WATER DISPENSERS WERE NOT DESIGNED TO PREVENT CONTACT WITH THE LIP-CONTACT SURFACE OF GLASSES THAT ARE REFILLED.
Recommendation: Modify the dispensing triggers so the lip-contact surface of glasses are not likely to contact the dispensing mechanism.

Violation: PEELING SOFT SEALANT WAS NOTED ON THE BACK PLATE OF THE SLICER.
Recommendation: 7.4.1.1.2 Materials that are used in the construction of multiuse utensils and food-contact surfaces of equipment shall be: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.

WHEN ASKED IF THE BAG WAS FOOD GRADE THE EMPLOYEE STATED THAT THE BAG WAS A GARBAGE BAG AND THAT THE BREAD WOULD BE USED FOR BREAD CRUMBS. THIS CONTAINER OF BREAD WAS DISCARDED DURING THE INSPECTION.
Recommendation: Ensure that bags used to store food are food grade.

Violation: THE COLANDER USED TO DRAIN FOOD FROM THE SOUP STATION STEAM KETTLE WAS PLACED ON THE DECK AND NOT ON A PLATFORM.
Recommendation: 7.5.7.3.1 Cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

Recommendation: 7.6.2.3.6 Lubricants shall meet the requirements specified in 21 CFR 178.3570 Lubricants with Incidental Food-Contact, if they are used on food-contact surfaces, on bearings and gears located on or within food-contact surfaces, or on bearings and gears that are located so that lubricants may leak, drip, or be forced into food or onto food-contact surfaces.

Violation: THE DECK IN THE CREW MESS BUFFET LINE WAS NOT CLEAN. THE CORNERS/SEAMS OF THE DECK WERE NOT COVED.
Recommendation: 7.7.4.2.1 Decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, shall be cleaned as often as necessary. 7.7.4.1.3 Bulkhead and deck junctures shall be coved.

Violation: THE DECORATIVE BULKHEAD PANEL BEHIND THE COFFEE MACHINES SHOWED EVIDENCE OF WATER DAMAGE, IT WAS CURLING AND SEPARATING FROM THE COUNTER. THERE WAS A DIFFICULT TO CLEAN GAP AT THE JUNCTURE OF THE COUNTER AND THE DECORATIVE BULKHEAD PANEL.
Recommendation: Repair/replace the decorative bulkhead panel. Ensure that when replaced the gap is sealed.

Violation: BOTH THE ENTRY TO THE PROVISIONS CORRIDOR ON DECK 2 AND THE ENTRY OF THE MILK/DAIRY WALK-IN REFRIGERATORS HAD A SECTION OF DECK THAT BUCKLED WHEN WALKED ACROSS. THIS IS A REPEAT VIOLATION FROM THE 11/24/2003 INSPECTION. THE VESSEL IS SCHEDULED FOR DRY DOCK THIS YEAR.
Recommendation: 7.7.4.1.1 Decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas shall be constructed and maintained for easy cleaning.
