Recommendation: In developing the Corrective Action Statement for this inspection, critical-item deficiencies (designated with Yes in Critical Column (worth 3 - 5 points), whether debited or not, should include standard operating procedures and monitoring procedures implemented to prevent the recurrence of the critical deficiency. Violation: THE BUCKET FILL STATION FAUCET AT THE WARD RESTROOM HANDWASH SINK DID NOT HAVE A BACK FLOW PREVENTION DEVICE INSTALLED ON THE POTABLE WATER LINE. THE FAUCET HAD A QUICK RELEASE ATTACHMENT WHERE A HOSE HAD BEEN CONNECTED. DIALYSIS EQUIPMENT WAS NOTED IN THE WARD RESTROOM. THIS EQUIPMENT WAS PERIODICALLY USED FOR PASSENGER DIALYSIS. .
Recommendation: 5.7.1.2.1 Backflow preventers shall be installed when air gaps are impractical or when water under pressure is required. Ensure that the potable water lines suppying the dialysis equipment are protected with the appropriate backflow prevention devices.

Violation: PEELING SOFT SEALANT WAS NOTED AROUND THE SEAM OF THE BIN AND CUBER OF ICE MACHINE 1C0504 MAKING CLEANING DIFFICULT.
Recommendation: 7.4.1.1.2 Materials that are used in the construction of multiuse utensils and food-contact surfaces of equipment shall be: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.

Recommendation: 7.4.2.2.1 Multiuse food-contact surfaces shall be: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Violation: GREASE WAS DRIPPING FROM THE LARGE FLOOR MIXER'S ATTACHMENT PROBE. THE MATERIAL SAFETY DATA SHEET DID NOT STATE THAT THE LUBRICANT WAS FOOD GRADE, ALTHOUGH THIS LUBRICANT IS WHAT THE MANUFACTURER RECOMMENDS.
Recommendation: 7.4.2.4.4 Equipment containing bearings and gears that require lubricants shall be designed and constructed so that the lubricant cannot leak, drip, or be forced into food or onto food-contact surfaces. Ensure that the lubricant used is food grade.

Violation: THE HINGES FOR THE UNDER COUNTER WARMING UNITS WERE IN POOR CONDITION WITH LOOSE SCREWS AND HINGE ATTACHMENTS NOT FULLY FASTENED MAKING THESE AREAS DIFFICIULT TO CLEAN.
Recommendation: 6.3.2.2.3 Nonfood-contact equipment shall be maintained in good repair and proper adjustment including: (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
