Violation: IN DEVELOPING THE CORRECTIVE ACTION STATEMENT FOR THIS INSPECTION, CRITICAL-ITEM DEFICIENCIES (DESIGNATED WITH YES IN CRITICAL COLUMN (WORTH 3 - 5 POINTS), WHETHER DEBITED OR NOT, SHOULD INCLUDE STANDARD OPERATING PROCEDURES AND MONITORING PROCEDURES IMPLEMENTED TO PREVENT THE RECURRENCE OF THE CRITICAL DEFICIENCY. Site: Other-POTABLE WATER
Violation: THERE WERE MANY DAYS SINCE THE LAST INSPECTION WHEN THE FAR POINT DISTRIBUTION CHARTS WERE NOT CHANGED EVERY 24 HOURS. ON 3/16/2004, THE SHIP WAS WITHOUT POWER AND FROM 9:00 AM TO 4:30 PM AND THE FREE HALOGEN RESIDUAL WAS NOT RECORDED ON THE CHARTS. MANUAL TESTS WERE NOT PERFORMED DURING THIS TIME. ON 12/17/2003, THE RECORDING PEN RAN OUT OF INK AND THE FREE HALOGEN RESIDUAL WAS NOT RECORDED FOR THE LOWER LOOP. MANUAL TESTS WERE NOT PERFORMED DURING THIS TIME AND THE PEN WAS NOT REPLACED UNTIL THE NEXT DAY.
Recommendation: 5.5.2.2.1 Halogen analyzer-chart recorder charts shall be changed, initialed, and dated daily. Charts shall contain notations of any unusual water events in the potable water system. 5.5.2.2.3 Records from the halogen analyzer-chart recorder shall verify the free residual halogen of 0.2 mg/L (ppm) and 5.0 mg/L (ppm) in the water distribution system for at least 16 hours in each 24-hour period since the last inspection of the vessel.

Site: Other-DECK 5 GALLEY
Violation: THE FOOD CONTACT SURFACES OF THE RESIN-COATED BUFFET CONTAINERS HAD CUTS, GOUGES AND BUBBLES IN THE RESIN. THESE CONTAINERS WERE NO LONGER CLEANABLE.
Recommendation: 7.4.1.1.2 Materials that are used in the construction of multiuse utensils and food-contact surfaces of equipment shall be: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition. 7.4.2.2.1 Multiuse food-contact surfaces shall be: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Violation: THE NON-FOOD CONTACT SURFACES OF EQUIPMENT HAD GAPS, OPEN SEAMS AND PEELING SEALANT, MAKING THEM DIFFICULT TO CLEAN.
Recommendation: 7.4.1.1.9 Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: THE GLASSWASH MACHINE'S DATA PLATE WAS NOT READABLE.
Recommendation: 7.5.2.2.3 A warewashing machine shall be provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Violation: DURING ACTIVE USE THE FINAL RINSE GAUGE ON THE RACK TYPE GLASSWASH MACHINE READ 160 F AND THE TEMPERATURE ON THE DISH READ 174F. THE FINAL RINSE GAUGE SHOULD READ BETWEEN 180-194F.
Recommendation: 7.5.6.1.2 In a mechanical operation, the temperature of the fresh hot water sanitizing rinse as it enters the manifold may not be more than 90C (194F), or less than: (1) For a stationary rack, single temperature machine, 74C (165F); or (2) For all other machines, 82C (180F). (3) A utensil surface temperature of 71C (160F) as measured by an irreversible registering temperature indicator shall be achieved. (4) The maximum temperature of 90C (194F), does not apply to the high pressure and temperature systems with wand-type, hand-held, spraying devices used for the in-place cleaning and sanitizing of equipment such as meat saws.

Violation: THE BACK DECKHEAD OF THE TECHNICAL AREA, BENEATH THE BAR COUNTER WAS COVERED WITH SLOTTED METAL AND WAS NOT EASILY CLEANABLE. A LARGE HOLE WAS NOTED BETWEEN THE DECK AND BULKHEAD IN THE SAME AREA.
Recommendation: 7.7.4.1.1 Decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas shall be constructed and maintained for easy cleaning.
