Site: Potable Water-DECK A AFT GARBAGE ROOM
Violation: DECK WASH HOSES WERE ATTACHED TO EACH OF TWO HANDWASH STATIONS (ENTRY DOOR AND BIN WASH) IN THE GARBAGE ROOM WITH SPRAY RINSE NOZZLES CONNECTED AT THE END OF THE HOSES. A NON-CONTINUOUS PRESSURE TYPE BACKFLOW PREVENTER WAS INSTALLED AT THE HOSE BIBB FOR EACH OF THESE SINKS.
Recommendation: Install continuous pressure type backflow preventers for the hot and cold potable water supply pipes to both handwash stations in the garbage room.

Site: Potable Water-COMPREHENSIVE BACKFLOW PREVENTION PROGRAM
Violation: THE WRITTEN BACKFLOW PREVENTION PROGRAM DID NOT INCLUDE THE AIR-GAP (NON-MECHANICAL) BACKFLOW PROTECTION USED IN THE POOL AND SPA COMPENSATION TANKS, EVAPORATOR FEED TO THE BOILER TANK, AND OTHER SUCH CONNECTIONS. THE AIR-GAPS WERE NOT DOCUMENTED FOR LOCATION OR INSPECTION.
Recommendation: Update the written cross-connection control program to include identification of all air-gaps and inspection of same.

Site: Food Service General-TIME AS A PUBLIC HEALTH CONTROL
Violation: THE TIME ONLY AS A PUBLIC HEALTH CONTROL PLAN DID NOT SPECIFY THAT CRACKED SHELL EGGS COMBINED FOR OMELETTE SERVICE WERE ON TIME CONTROL WHILE IN REACH-IN REFRIGERATED STORAGE. THE PLAN DID NOT LIST THE BREAKFAST SERVICE TIME PERIOD EITHER.
Recommendation: Revise the written time control plan and ensure it addresses the situations where cracked shell eggs are stored in reach-in refrigeration units, and ensure that the service times are listed for breakfast, lunch and dinner.

Violation: WOODEN PALLETS WERE NOTED STACKED ON TOP OF CARDBOARD CASES OF WATER.
Recommendation: Do not stack pallets on food or drinks in storage.

Violation: THE LABEL ON THE BLAST CHILLER WAS IN DISREPAIR, MAKING CLEANING DIFFICULT.

Site: Buffet-LIDO - ROTISSERIE AND TASTE OF NATIONS PANTRIES
Violation: THE CONTROL LABELS WERE PEELING AND LEAVING A DIFFICULT TO CLEAN SURFACE AT THE HOT FOOD TABLES AT THE BUFFET LINES. A HOLE WAS PRESENT IN THE COUNTER WHERE THE POWER CABLE FOR THE HOT TABLE PENETRATED IN THE ROTISSERIE PANTRY.
Recommendation: Repair or remove peeling control labels from the hot food tables.

Site: Food Service General-SERVING TRAYS
Violation: THE EDGES OF THE SERVING TRAYS WERE IN DISREPAIR, MAKING CLEANING DIFFICULT.
Recommendation: Replace trays as the damage is significant enought that they cannot be easily cleaned.

Site: Galley-POT WASH
Violation: AN ENTIRE STORAGE RACK AND AT LEAST 3-4 DECK STANDS WERE FILLED WITH DIRTY POTS, PANS, AND UTENSILS DIRECTLY OPPOSITE THE POTWASH MACHINE ON THE CLEAN END OF THE POTWASH. A FULL ROLLING BIN OF SOILED CLOTH TABLE LINEN WAS PARKED DIRECTLY BESIDE THE POTWASH MACHINE ON THE CLEAN SIDE. THERE WAS LESS THAN A METER SEPARATION BETWEEN THE VARIOUS SOILED ITEMS AND THE POTWASH MACHINE AND ALL WERE IN THE PATHWAY TO THE CLEAN SHELVES BEHIND.
Recommendation: Sufficient tables and storage shelving should be provided on the dirty end of the pot eash operation, rather than accumulating large amounts of dirty items at the point where clean pans and utensils would be removed from the sinks and machine for storage.

Site: Preparation Room-FISH PREPARATION ROOM A-1-14
Violation: EVERY PLASTIC FOOD CONTAINER STORED BENEATH THE PREPARATION COUNTER ON THE CLEAN SHELF WAS SOILED. STAFF STATED THAT SOILED ITEMS HAD BEEN PLACED ON THE CLEAN SHELVING UNIT.
Recommendation: Ensure that food only contacts surfaces of equipment and utensils that are cleaned and sanitized.

Violation: THE PREVIOUSLY CLEANED LETTUCE WASHER WAS SOILED ON THE INSIDE AND STAGNET WATER WAS NOTED POOLED IN THE DRAIN.

Violation: A LARGE ROASTING PAN ON THE DIRTY SHELF HAD SURFACES WHICH WERE DAMAGED AND PITTED, MAKING CLEANING DIFFICULT. A LARGE METAL BOWL ON THE DIRTY SHELF HAD CUT METAL ALONG THE INNER RIM, MAKING CLEANING DIFFICULT, AND A TALL METAL WHISK WAS IN DISREPAIR AND THE WIRES WERE BOUND WITH A PLASTIC TIE STRAP. ALL THREE ITEMS WERE DISCARDED IMMEDIATELY.
Recommendation: Replace damaged pots, pans, and utensils when they are no longer easy to clean.

Violation: THERE WAS NO TOILET PAPER IN THE CREW MESS FEMALE RESTROOM. THIS WAS CORRECTED IMMEDIATELY.
Recommendation: Maintain a supply of toilet paper in the crew mess female restroom at all times.

Site: Preparation Room-COLD STORE FRESH AND COOKED MEAT
Violation: THE DECK WAS IN DISREPAIR WITH TWO DEEP GOUGES THAT ALLOWED WATER TO STAND IN THE CREVICE. THIS MADE CLEANING DIFFICULT.
Recommendation: Repair the damage deck.

Site: Galley-HOT GALLEY, TASTE OF NATIONS AND ROTISSERIE PANTRIES
Violation: DECKS NEAR COOKING EQUIPMENT, DECK SINKS, AND GUTTERS HAD BROKEN PIECES OF GROUTING AND RECESSED GROUT IN BOTH THE GALLEY HOT SECTION, TASTE OF NATIONS, AND ROTISSERIE PANTRIES.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Violation: ELECTRICAL, WATER AND DETERGENT/SANITIZER CORDS/HOSES WERE NOTED ON THE DECK, MAKING CLEANING DIFFICULT.
Recommendation: Ensure that exposed utility service lines and pipes, including lines for fire detection and protection systems, are installed so they do not obstruct or prevent cleaning.

Violation: LIQUID WAS NOTED DRIPPING FROM THE DECKHEAD IN THE AFT PORT MIDDLE AREA OF WALK-IN REFRIGERATION UNIT # A1-01. FOOD WAS NOT STORED BELOW THE DRIPPING LIQUID.
Recommendation: REpair the deckhead leak in walk-in refrigerator #A1-01.

Site: Food Service General-FOOD WASTE DISPOSAL SYSTEM
Violation: THE TYPHOON WASTE DISPOSAL SYSTEM WAS NOT WORKING DURING THE INSPECTION IN GALLEYS ON DECK 1 AND DECK A AND IN THE VEGETABLE PREPARATION ROOM IN PROVISIONS. STAFF WERE IN THE PROCESS OF REPAIRING THE WASTE SYSTEM DURING THE INSPECTION.
Recommendation: Maintain the waste disposal system in good repair.

Violation: THE VENTILATION SYSTEM OVER THE CLEAN SIDE OF THE POT WASH WAS NOT EFFECTIVE IN REMOVING THE CONDENSATE FROM THE BULKHEAD. THIS WAS NOTED ON THE 08/15/2004 INSPECTION.
Recommendation: Ensure that ventilation hood systems and devices operate effectively to prevent grease and condensate from collecting on the bulkheads and deckheads and remove contaminants generated by equipment located under them.
