Violation: THE RECORDS NOTING THE TESTING OF THE REDUCED PRESSURE PRINCIPLE BACKFLOW PREVENTERS WERE NOT AVAILABLE. THERE WAS SOME QUESTION ON WHEN THE BACKFLOW PREVENTER BETWEEN THE POTABLE AND TECHNICAL WATER WAS LAST TESTED.
Recommendation: Ensure that all reduced pressure principle backflow preventers are properly tested at least annually and that the record of the testing is readily available for review.

Recommendation: The ultra-pasteurization label does not necessarily exempt milk/cream products from refrigeration. Further review by VSP and the FDA revealed that if a product has been heat processed to a level where no refrigeration is required the master carton will so designate. Otherwise the milk/cream product must be held at or below 45 F.

Recommendation: Adjust the clean landing tables or install a secondary drain to ensure wastewater doesn't pool along the clean table landing.

Violation: THE UPPER COLD TOP OF THE STACKED BUFFET COUNTER HAD AN INEFFECTIVE SNEEZE SHIELD. SUGGEST REPLACEMENT WITH A SERIES OF PLEXIGLASS DOORS.
Recommendation: Display tables or buffet lines for service of unpackaged foods must be effectively shielded so as to intercept any source of contamination from the consumer. Appropriate food shielding (sneeze guards) should be provided.

Violation: A SPATULA WAS STORED IN CONTACT WITH A DIRTY ELECTRICAL CORD AT THE HOT SERVICE STATION.
Recommendation: Between uses during service, food-dispensing utensils shall be stored in the food with the dispensing utensil handle extended out of the food, stored clean and dry or stored in running water.

Violation: ONE PASSENGER FROM THE 5/26-29 CRUISE NOTIFIED VSP THAT SHE REPORTED TO THE INFIRMARY AND NOTIFIED THE STAFF THAT SHE HAD DIARRHEA. THIS PASSENGER WAS NOT RECORDED IN THE DIARRHEAL ILLNESS LOG AND THE REPORT SUBMITTED TO VSP NOTED 6 ILL WHILE THE ACTUAL LOG DOCUMENTED 7 ILL.
Recommendation: Diarrhea logs in the hospital must be accurately maintained, and results must be reported to the Vessel Sanitation Program in accordance with Section III of the "Vessel Sanitation Program Operations Manual", August 1989.

Violation: A CREW MEMBER WHO IS A FOOD WORKER REPORTED TO THE INFIRMARY WITH DIARRHEAL ILLNESS ON 7/14. HE WAS EXCLUDED FROM WORK AND THE INFORMATION RECORDED IN THE DIARRHEAL ILLNESS LOG. HE RETURNED TO WORK ON 7/15 ALTHOUGH THE CHART INDICATED HE WAS TO BE EXCLUDED FOR AN ADDITIONAL 8 HOURS. THERE WAS NO MEDICAL CLEARANCE RECORDED IN EITHER THE LOG OR HIS MEDICAL RECORD.
Recommendation: Ensure that any food worker excluded from work due to illness is not permitted to return to work unless medically cleared to do so. Ensure that such clearance is properly recorded in both the diarrhea log and the medical record.

Violation: EXCESS LENGTHS OF POWER CORDS AND HOSES TO COUNTER MOUNTED COFFEE AND JUICE MACHINES, BULK MILK DISPENSERS, CREDIT CARD MACHINES, ETC. WERE COILED OR DRAPED OVER COUNTER TOPS MAKING THEM DIFFICULT TO ACCESS AND CLEAN IN THE GALLEYS, DINING ROOMS, MESSES, AND BARS.
Recommendation: Shorten cords and hoses so they are suspended above the counters and make cleaning easier or otherwise bundle and hang them from the back of the equipment.

Recommendation: Decks must be accessible for cleaning and dirty dishware, trays, or food/beverage containers shall not be stored on the deck.

Violation: THERE WAS ONLY A SINGLE HANDWASH BASIN FOR APPROXIMATELY 55-80 WAITER STAFF AND BUS BOYS TO USE AT THE DIRTY DISH DROP-OFF. THE GALAXY SIDE ALSO APPEARED TO BE INSUFFICIENT.
Recommendation: Recommend extending the single basin and fitting 3 sink taps with soap and paper towel dispensers at this drop area and at the galaxy side if necessary.

Recommendation: It is required that decks be maintained so as to be durable and easily cleanable. Cove the seam at the deck to a minimum 9.5 mm radius with a hard material.

Recommendation: It is required that additional lighting be provided to insure at least 20-footcandles of light on all food preparation surfaces.

Violation: EXCESS CONDENSATION WAS NOTED ON THE AIR CONDITIONING SUPPLY VENT MOUNTED ON THE DECKHEAD. THE FINAL RINSE TEMPERATURE OF THE POT WASH SINK WAS 190 F.
Recommendation: Reduce the final rinse sink water temperature to 170 to lower the condensation at the deckhead. If that doesn't work consider improving ventilation.
