Violation: TWO POTABLE WATER ANALYZER/RECORDER CHARTS AT THE BRIDGE SHOWED FREE CHLORINE RESIDUALS BELOW 0.2 PPM FOR PERIODS OF 4 HOURS WITH NO EXPLANATION OR CORRECTIVE ACTIONS DOCUMENTED.
Recommendation: 5.5.2.2.1 Halogen analyzer-chart recorder charts shall be changed, initialed, and dated daily. Charts shall contain notations of any unusual water events in the potable water system. Low chlorine levels should be somehow explained on the chart, including the solution of the problem.

Violation: A NUMBER OF ANALYZER/RECORDER CHLORINATION CHARTS AT THE BRIDGE HAD RECORDING PERIODS IN EXCESS OF 24 HOURS. ONE SUCH CHART HAD A RECORDING PERIOD OF 28 HOURS PRIOR TO BEING CHANGED.
Recommendation: 5.5.2.1.1 Halogen analyzer-chart recorder charts shall have a range of 0.0 to 5.0 mg/L (ppm) and have a recording period of 24 hours.

Site: Other-MAIN GALLEY
Violation: POACHED EGGS WERE FOUND IN AN ICE BATH WELL WITHIN TEMPERATURE. THESE POACHED EGGS NEED TO BE REHEATED SO THAT ALL PARTS OF THE FOOD REACH A TEMPERATURE OF AT LEAST 74°C (165°F) FOR 15 SECONDS.
Recommendation: 7.3.4.3.3 If reheated in a microwave oven for hot holding, potentially hazardous food shall be reheated so that all parts of the food reach a temperature of at least 74°C (165°F) and the food is rotated or stirred, covered, and allowed to stand covered for 2 minutes after reheating. Time management could be selected as an option for this food product, but a plan for that should be made in writing and it should be available for review during inspections.

Violation: BREAD AND ROLLS WERE PRESENTED TO PASSENGERS ON THE SELF SERVICE BUFFET OUTSIDE OF THE SNEEZE SHIELDS WHERE THEY WERE SUBJECT TO CONTAMINATION BY PASSENGERS.
Recommendation: 7.3.3.6.1 Food on display shall be protected from contamination by the use of packaging; counter, service line, or salad bar food guards; display cases; or other effective means.

Violation: AN UNCOVERED SLICED MEAT TRAY AND AN UNCOVERED SALAD BOWL WAS PLACED ON THE BUFFET LINE FOR PASSENGER USE. THESE WERE NOT PROTECTED BY A SNEEZE GUARD.
Recommendation: 7.3.3.6.1 Food on display shall be protected from contamination by the use of packaging; counter, service line, or salad bar food guards; display cases; or other effective means.

Violation: THE COILS ON THE DEEP FRYER HAVE BOLTS WITH EXPOSED THREAD. SLOTTED FASTENERS WERE NOTED ON THE DEEP FRYER AS WELL. THESE FOOD CONTACT SURFACES WERE NOT EASILY CLEANABLE.
Recommendation: Replace the slot head fasteners with low profile bolt or round head fasteners.

Violation: THERE WAS WATER LOCATED IN THE INSIDE LIP OF THE MIXING COLLAR.
Recommendation: Air-dry the collars prior to replacement and after cleaning.

Violation: SLIGHT CORROSION WAS NOTED IN THE DISPENSING ARM OF THE COFFEE URNS AND ON THE BOLT INSIDE OF THE URN.
Recommendation: Seek a different metal for this dispensing arm which matches the stainless steel container. It appears the dissimilar metals are creating electrolysis and corrosion at the point of connection inside the urns.

Violation: THE THREADS ON THE BOLT INSIDE OF THE COFFEE URN ARE EXPOSED AND ARE NOT EASILY CLEANABLE.
Recommendation: Fasten the dispensing head differently so there is no exposed bolt threads or cut the bolt so the tip is flush with the connecting nut.

Violation: THE CUTTING BOARDS WERE SEVERELY PITTED AND SCORED.
Recommendation: 7.4.5.1.3 Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they no longer can be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

Violation: THE SURFACE AREA UNDERNEATH THE BLADE OF THE MEAT SLICER WAS NOT EASILY CLEANABLE.
Recommendation: 7.4.2.2.1 Multiuse food-contact surfaces shall be: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Violation: THERE WERE A NUMBER OF ALTO SHAAM HOT HOLDING OVENS THAT WERE MISSING AMBIENT TEMPERATURE MEASURING DEVICES.
Recommendation: 7.4.2.4.9.2 Temperature measuring devices shall conform to the following guidelines: (2) Cold or hot holding equipment used for potentially hazardous food shall be designed to include and shall be equipped with at least one integral or affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.

Violation: THERE WAS NO AMBIENT TEMPERATURE MEASURING DEVICE IN THE ALTO SHAAM OVEN LOCATED BEHIND THE HOT SERVICE LINE. HASH BROWN POTATOES AND PANCAKES WERE HELD HOT IN THIS UNIT.
Recommendation: 7.4.2.4.9.1 Temperature measuring devices shall conform to the following guidelines: (1) In a mechanically refrigerated or hot-food storage unit, the sensor of a temperature measuring device shall be located to measure the air temperature in the warmest part of a mechanically refrigerated unit and in the coolest part of a hot-food storage unit.

Violation: THE DECORATIONS OVER THE BUFFET LINE WERE NOT EASILY CLEANABLE. SOME OF THESE WERE HANGING OVER THE SERVING LINE AND COULD CONTAMINATE FOODS DISPLAYED THERE.
Recommendation: Move the decorations completely or display them in a way where they don't fall below the counter level where they could potentially contaminate foods stored beneath.

Violation: SOME OF THE ALTO SHAAM UNITS HAD OPEN SEAMS THAT MAKE CLEANING THESE UNIST DIFFICULT.
Recommendation: 7.4.2.2.6 Nonfood-contact surfaces shall be free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: A NUMBER OF THE LARGER FOOD AND CONDIMENT SERVICE CONTAINERS HAD PEELING EXTERIOR PLASTIC COATING. THE EXPOSED SURFACE WAS DIFFICULT TO CLEAN.
Recommendation: Replace worn items when they become too difficult to clean properly.

Violation: THE BRAIDED FLEXIBLE METAL PIPE BENEATH THE 3RD SINK COMPARTMENT WERE DIFFICULT TO CLEAN AND LOCATED WHERE THEY WERE SUBJECT TO SPLASH. THE METAL WAS CORRODING AND SHOULD BE EITHER REPLACED OR COVERED WITH A SMOOTH, EASY TO CLEAN PRODUCT.
Recommendation: 7.4.1.1.9 Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: THE SPRAY PATTERN IN THE UPPER ARM FINAL RINSE NOZZLES WAS POOR DURING OPERATION OF THE STARBOARD CONVEYOR DISHWASH MACHINE. NOT ALL DISHWARE PASSING THROUGH THIS CHAMBER WAS SUBJECT TO A FULL RINSING SPRAY.
Recommendation: 7.5.4.1.1 Warewashing equipment shall be maintained in good repair and proper adjustment including: (1) Warewashing equipment shall be maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines. (2) Water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use.

Violation: THE FINAL RINSE GAUGE ON THE POT WASH MACHINE WAS NOT FUNCTIONING PROPERLY DURING ACTIVE USE.
Recommendation: 7.5.2.1.1 Water temperature measuring devices: (1) That are scaled in Celsius or dually scaled in Celsius and Fahrenheit shall be designed to be accurate to ±1.5°C in the intended range of use. (2) That are scaled only in Fahrenheit shall designed to be accurate to ±3°F in the intended range of use.

Violation: GREASE RESIDUE WAS NOTED ON THE DEEP FRYER COILS. THIS RESIDUE COULD BE SCRAPED OFF WITH HAND PRESSURE.
Recommendation: 7.5.5.1.1 Food-contact surfaces of equipment and utensils shall be clean to sight and touch.

Violation: THE HAND WASH STATION IS APPROXIMATELY 35 FT. FROM THE MEAT CARVING STATION.
Recommendation: 7.7.1.1.2 The handwashing facility shall be located within 8 m (25 feet) of all parts of the area and should not be located in an adjacent area that requires passage through a closed door.

Site: Other-INTEGRATED PEST MANAGEMENT (IPM)
Violation: THE PRESSURIZED CONTACT INSECTICIDE (565 PLUS XLO) WHICH HAS BEEN USED IN FOOD PREPARATION/SERVICE AREAS APPEARS TO BE A RESTRICTED USE PRODUCT. IT IS UNCLEAR WHETHER THIS PRODUCT MEETS THE REQUIREMENTS SPECIFIED IN 40 CFR 152 SUBPART I CLASSIFICATION OF PESTICIDES. IF IT IS A RESTRICTED-USE PESTICIDE, THE PEST CONTROLLERS TRAINING CERTIFICATE DOES NOT SPECIFY HE HAS BEEN CERTIFIED TO APPLY SUCH PESTICIDES.
Recommendation: A review of the manufacturers data did not prove this product to be formulated from restricted use materials. Please contact the manufacturer and ensure this is the case and the training provided is sufficient for its use.

Violation: THE DECKHEAD IN THE BUFFET AREA WAS SOILED WITH FOOD RESIDUE. STEAM FROM THE BAIN-MARIES WAS CREATING CONDENSATION ON THE DECKHEAD.
Recommendation: 7.7.4.1.1 Decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas shall be constructed and maintained for easy cleaning. If condensation develops on the deckhead use paper towels to wipe off the affected area and seek a ventilation solution for long term.

Violation: THE HOOD WAS PULLING AWAY FROM THE BULKHEAD.

Violation: THE PORTABLE CHILD SEAT WAS REPORTEDLY NOT USED BY THE CHILDREN IN THE TOILET ROOM AND PROVIDERS ARE PROHIBITED FROM ASSISTING IN THAT AREA OF THE TOILET ROOM. RECOMMEND INSTALLATION OF A SECONDARY, PERMANENT SEAT OVER THE EXISTING TOILET SEAT WITH A SIZE FOR SMALL CHILDREN.
Recommendation: 10.2.1.1.1 Child-size toilet and handwashing facilities shall be provided, if toilet rooms are located in a child-activity center.
