Violation: THE MEDICAL STAFF WERE UNAWARE THAT ANY PASSENGER OR CREW MEMBER ISSUED ANTIDIARRHEAL MEDICATIONS MUST BE INCLUDED IN THE GASTROINTESTINAL ILLNESS SURVEILLANCE LOG.
Recommendation: 4.1.2.1.1 A standardized gastrointestinal illness surveillance log for each cruise shall be maintained daily by the master of the vessel, the medical staff, or other designated staff. 4.1.2.1.3 The log shall list: (1) All reportable cases of gastrointestinal illness; (2) All passengers and crew members who are dispensed antidiarrheal medication from the master of the vessel, the medical staff, or other designated staff.

Site: Other-POTABLE WATER CHEMICAL TANKS
Violation: THE POTABLE WATER SUPPLIED HOSE USED TO FILL THE CHEMICAL TANKS IN THE ENGINE ROOM WAS NOT PROTECTED WITH A BACKFLOW PREVENTION DEVICE. THIS HOSE HAD A SPRAY GUN ATTACHED TO THE END, THEREFORE A CONTINUOUS PRESSURE BACKFLOW PREVENTION DEVICE WOULD HAVE TO BE USED.
Recommendation: 5.7.1.1.3 The vessel shall provide a comprehensive cross- connection control program that provides safe connections to the potable water system through air gaps or appropriate backflow devices at the following locations, if present: (1) Potable water supply lines to swimming pools, whirlpool spas, hot tubs, bathtubs, showers, and similar facilities; (2) Photographic laboratory developing machines and utility sinks; (3) Beauty and barber shop spray-rinse hoses; (4) Potable water faucets where hoses are connected or can be connected by threaded or quick-connect outlets such as those serving tanks containing chlorine and other chemicals, and deck taps; (5) Garbage grinders and pulpers; (6) Mechanical warewashing machines; (7) Hospital and laundry equipment; (8) Air conditioning expansion tanks; (9) Boiler feed water tanks; (10) Fire systems; (11) Toilets; (12) Potable water, bilge, and sanitary pumps that require priming; (13) Freshwater or saltwater ballast systems; (14) Bilge or other waste water locations; (15) International shore connection; and (16) Any other connection between potable and non-potable water systems.

Violation: POTABLE WATER PIPING WAS NOT PAINTED LIGHT BLUE OR STRIPPED WITHIN 15 FEET (5 METERS) AT FITTINGS, AND ON EACH SIDE OF PARTITIONS, DECKS, AND BULKHEADS IN THE POOL PUMP ROOM.
Recommendation: 5.3.2.1.1 Potable water piping shall be painted light blue or striped with 15 cm (6 inches) light blue bands or a light blue stripe at fittings on each side of partitions, decks, and bulkheads and at intervals not to exceed 5 m (15 feet) in all spaces, except where the decor would be marred by such markings.

Violation: WITH SEVERAL BATHERS IN THE FORWARD POOL THE FREE CHLORINE CONCENTRATION WAS MEASURED AT 0.24 PPM. THE FORWARD WHIRLPOOL SPA WAS OPEN FOR USE AND HAD A FREE CHLORINE CONCENTRATION OF 2.18 PPM.
Recommendation: 6.1.1.2.3 No bathers shall be allowed to use the pool before a free residual halogen of 1.0 mg/L (ppm) is achieved. 6.2.2.1.1 A free residual halogen of >1.0 and <3.0 mg/L (ppm) shall be maintained in recirculated swimming pools. 6.3.2.1.1 Whirlpool spas shall maintain a free residual chlorine of >3.0 mg/L (ppm) and <10 mg/L (ppm), or a free residual bromine of >4.0 mg/L (ppm) and <10 mg/L (ppm).

Violation: TWO SIGNS WERE POSTED AT THE SPLASH POOL, STATING THAT "INFANTS ARE REQUIRED TO WEAR PROTECTIVE SWIM DIAPERS" BEFORE ENTERING THE POOL/SPAS. THE SIGNS WERE REMOVED DURING THE INSPECTION.
Recommendation: 6.4.1.3.1 Children in diapers or who are not toilet trained are not permitted in the public swimming pools and whirlpool spas.

Violation: THE VARIOUS SIZED PACKAGED MILK PRODUCTS FROM THE ST. THOMAS DAIRIES DO NOT COMPLY WITH GRADE A STANDARDS AS SPECIFIED IN LAW. THESE MILK PRODUCTS SHOULD NOT BE USED FOR PASSENGER OR CREW SERVICE. THERE ARE BAG-IN-BOX MILK PRODUCTS FOR USE IN SELF-SERVICE DISPENSERS WHICH CAN BE USED TO FILL DISPENSING MILK PITCHERS AND SERVED TO BOTH PASSENGERS AND CREW. THE CDC HAS CONTACTED THE FOOD AND DRUG ADMINISTRATION FOR ADVICE ON THIS PRODUCT AND IT IS NOT CERTIFIED FOR USE BY FDA FOR CONSUMPTION.
Recommendation: 7.3.2.1.6 Fluid milk and milk products shall be obtained from sources that comply with Grade A standards as specified in law.

Violation: SEVERAL PREPARED PLATES OF CUT HONEYDEW AND CANTALOUPE MELON WERE FOUND IN PASS-THRU REACH-IN REFRIGERATOR #1 AT PRODUCT TEMPERATURES OF 45-50 F. THE MELON PLATES WERE REMOVED IMMEDIATELY AND TAKEN TO THE BLAST CHILLER.
Recommendation: 7.3.5.3.1.1 Potentially hazardous food shall be maintained: (1) At 60C (140F) [roasts 54C (130F)] or above,; or (2) At 5C (41F) or less.

Violation: SALMON IS PURCHASED FROZEN FROM A SUPPLIER FOR USE IN RAW ITEMS SUCH AS SUSHI, BUT THERE WAS NO WRITTEN RECORD ONBOARD STATING THAT THE SUPPLIER FROZE THE FISH TO A SAFE TEMPERATURE AND TIME ACCORDING TO THE VSP MANUAL SECTION 7.3.4.2.1.
Recommendation: 7.3.4.2.2 If raw, raw-marinated, partially cooked, or marinated-partially cooked fish are served in ready-to-eat form: (1) The person in charge shall record the freezing temperature and time to which the fish are subjected and shall retain the records on the vessel for 90 calendar days beyond the time of service or sale of the fish; or (2) If the fish are frozen by a supplier, a written agreement or statement from the supplier stipulating that the fish supplied are frozen to a safe temperature and a time.

Recommendation: 7.5.6.2.3.1 The sanitizing solutions shall be used with the following concentrations: (1) A chlorine solution shall have a concentration between 50 mg/L (ppm) and 200 mg/L (ppm).

Violation: THE FINAL HOT WATER SANITIZING RINSE TEMPERATURE OF THE IN-USE CONVEYOR GLASSWASH MACHINE WAS 155-160 F AS MEASURED AT THE DISH SURFACE DURING REPEATED TESTS. FOLLOW-UP TESTING OF THE WASH AND RINSE TANKS DEMONSTRATED THAT THE RINSE TANK AND RINSE THERMOMETER REGISTERED 174 F.
Recommendation: 7.5.6.1.2 In a mechanical operation, the temperature of the fresh hot water sanitizing rinse as it enters the manifold may not be more than 90C (194F), or less than: (1) For a stationary rack, single temperature machine, 74C (165F); or (2) For all other machines, 82C (180F). (3) A utensil surface temperature of 71C (160F) as measured by an irreversible registering temperature indicator shall be achieved. (4) The maximum temperature of 90C (194F), does not apply to the high pressure and temperature systems with wand-type, hand-held, spraying devices used for the in-place cleaning and sanitizing of equipment such as meat saws.

Violation: A WET WIPING CLOTH WAS SET ON A COUNTER TOP BETWEEN USES. THE STAFF MEMBER PRESENTED IT AS A SANITIZING CLOTH USED FOR CLEANING AT THE BAR, BUT THERE WAS NO SANITIZING SOLUTION PRESENT AND THE 3 BUCKETS IN THE BAR PANTRY WERE EMPTY.
Recommendation: 7.3.3.4.4.2 Wiping cloths shall be restricted to the following: (2) Cloths used for wiping food spills shall be dry and used for wiping food spills from tableware and single-service articles or wet and cleaned, stored in a chemical sanitizer, and used for wiping spills from food-contact and nonfood-contact surfaces of equipment.

Violation: THE JUNCTURE OF THE DECK AND SERVICE COUNTER WAS NOT COVED. THE BACK BULKHEAD JUNCTURE WITH THE DECK WAS NOT PROPERLY COVED AND DIRT DEBRIS WAS PRESENT ALONG THE SEAM AT THIS JUNCTURE.
Recommendation: 7.7.4.1.3 Bulkhead and deck junctures shall be coved. 7.7.4.1.1 Decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas shall be constructed and maintained for easy cleaning.

Violation: BOTH THE ENTRY TO THE PROVISIONS CORRIDOR ON DECK 2 AND THE ENTRY OF THE MILK/DAIRY WALK-IN REFRIGERATOR HAD A SECTION OF DECK WHICH BUCKLED WHEN WALKED ACROSS. THIS CONDITION COULD CAUSE FRACTURING OF THE DECK IF TROLLEYS OR PALLETS ARE USED IN THESE AREAS. RECOMMENDATION ONLY.
Recommendation: 7.7.4.1.1 Decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas shall be constructed and maintained for easy cleaning.

Violation: THE ARTIFICIAL LIGHT LEVEL ACROSS THE FRONT BAR WAS BELOW THE MINIMUM 220 LUX (20 FOOT CANDLES).
Recommendation: 7.7.5.1.1 The light intensity shall be at least 220 Lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.

Violation: THE AIR CONDITIONING COIL FRAME AND WELDS WERE SEVERELY RUSTED, MAKING THE AREAS DIFFICULT TO CLEAN.
Recommendation: The corrosion/rust on the air conditioning coil makes cleaning this area difficult. 9.2.1.2.1 Air handling units shall be kept clean.

Violation: CHEMICALS WERE STORED BELOW THE DIAPER CHANGING AREA SINK WITH BABY WIPES AND DIAPERS. STAFF STATED THEY DO NOT ACCEPT CHILDREN IN DIAPERS ALTHOUGH THEY STILL HAD THE DIAPER DISPOSAL CONTAINER AND CHANGING TABLE AVAILABLE.
Recommendation: Recommend that the diaper changing table, diaper disposal container, diapers and baby wipes be removed from the child activity center if children in diapers are no longer accepted. Chemicals and paper products should be stored separately to prevent cross-contamination.
