Site: Potable Water-Bacteriological Random Sample Tests
Violation: For the month of April, the bacteriological sample tests were taken from Deck 6 Room Service, Deck 3 Main Galley, Deck 0 Crew Galley and the Pizzeria. For the month of May, the bacteriological sample tests were taken from Deck 6 Room Service, Deck 3 Main Galley, Deck 0 Crew Galley and the Spa.
Recommendation: Ensure that a minimum of four potable water samples per month are collected and analyzed for the presence of Escherichia coli and are collected randomly from locations on the forward, aft, upper, and lower decks of the vessel. Ensure that sample sites are changed each month.

Site: Potable Water-Cross-connection Control Program
Violation: The cross-connection control program did not list the chemical dispensers which were protected by backflow prevention devices.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Violation: There was a threaded tap on the faucet used to fill the left tilting kettle. A backflow prevention device was not installed on the potable water supply line.
Recommendation: Remove the threads or install a continuous pressure backflow prevention device on the potable water line.

Violation: The potable water lines were either painted light blue or striped dark blue. Per staff, both of these colors indicated potable water.
Recommendation: Ensure that potable water piping and fittings are painted or striped auxiliary blue, or in accordance with ISO 14726, at 5 m (15 feet) intervals on each side of partitions, decks, and bulkheads in all spaces, except where the decor would be marred by such markings.

Violation: Hoses were connected to the potable water fill lines for the fresh water fill tanks. There were no backflow prevention devices on these fill lines.
Recommendation: Ensure that backflow preventers are installed when air gaps are impractical or when water under pressure is required.

Violation: There was not an anti-entrapment drain cover for the kid's pool.
Recommendation: Ensure that anti-entrapment drain covers or other drains that prevent entrapment hazards as specified in U.S. Consumer Product Safety Publication 363-009801 (dual drains/channel drains) are provided on swimming pools and whirlpool spas. Ensure that drain covers have either the plumbing/engineering approving organization stamp and flow rate affixed to the cover, or that a letter is maintained aboard the vessel certifying that the cover meets the safety requirements outlined in AMSE/ANSI A112.19.8M or an equivalent standard.

Violation: There were no depth markers at the pool.
Recommendation: Ensure that the depth of each pool is prominently displayed so that it can be seen from the deck and in the pool. Ensure that depth markers are labeled in feet or meters, or both,. Additionally, ensure that depth markers are installed for every 1 m (3 feet) in change of depth.

Violation: The floatation device was not located at a prominent location.
Recommendation: Ensure that an easy-access shepherd's hook and approved floatation device (with a length of line 1/2 the pool width or 16 m (50 feet)) is provided at a prominent location near each public swimming pool.

Violation: The profile strip at the counter/tray rail juncture was loose.

Violation: There was a 12 inch area to the left of the aft beverage station and between the waiter station that was not coved at the deck/bulkhead juncture. This area was heavily soiled with an accumulation of old food and debris.

Site: Housekeeping-Outbreak Prevention and Response Plan
Violation: Per staff, during an outbreak, the public areas were disinfected on an hourly basis. However, the outbreak prevention and response plan did not mention the frequency of disinfection of public areas during an outbreak.
Recommendation: Ensure that when the cumulative proportion of cases of gastrointestinal illness among passengers or crew members is 2%, the infection control response includes cleaning and disinfecting all public areas, including handrails and restrooms, on a continuous basis until the proportion decreases to < 2%.
