Site: Food Service General-Toilet Rooms - Food Areas
Violation: There was no "WASH HANDS AFTER USING TOILET" signs in the restrooms used by the food employees.
Recommendation: Ensure that signs are conspicuously posted on the bulkhead adjacent to the door of the toilet, reading "WASH HANDS AFTER USING TOILET " in a language that the food employees understand.

Site: Galley-Grill - Walk-In Freezer
Violation: There was broken and missing grout all over the left side of the deck.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Violation: There was a gap suggesting a missing fastener and a fastener not inserted correctly in the food splash zone of the right deck oven. This was corrected.
Recommendation: Replace missing fasteners and secure them properly.

Violation: There was a soiled scoop used for the cleaning of the drains placed inside the handwash station. The crew member working on that side of the dishwasher removed the scoop from the handwash station.
Recommendation: Ensure that handwashing facilities are used for no other purpose and are accessible at all times.

Violation: There was a soiled scoop used for the cleaning of the drains placed inside the handwash station.
Recommendation: Ensure that only articles necessary for the food service operation are stored in food preparation, food storage, and warewashing areas.

Violation: There was an accumulation of burned-like debris inside the lighting fixture located above the in-use soup kettles. The staff took out the cover to clean it.

Violation: Some of the sealed dial thermometers at the undercounter refrigerators had inside pooled water and an accumulation of mold.

Violation: The training of the Integrated Pest Management Manager, as part of the pest control personnel, was not documented in the IPM Plan.
Recommendation: Ensure that the training of the pest-control personnel is documented in the Integrated Pest Management Plan.

Site: Medical-Gastrointestinal Illness Reporting - Food Handler
Violation: A male head team waiter developed signs and symptoms of acute gastroenteritis (AGE) that started on 18 March 2011 at 3:00 A.M. The waiter reported to medical on 18 March 2011 at 11:39 A.M. According to the medical staff, the medical department opens at 8:00 A.M. It was unclear to the staff why the waiter did not report to the medical department at 8:00 A.M., or if the waiter worked a shift or partial shift before reporting to medical.
Recommendation: Ensure that employees having gastrointestinal (GI) symptoms do not go to work and do report to medical as soon as having their first GI symptom.

Violation: There was no 72-hour questionnaire for an 'able bodied' crew member that had symptoms of acute gastroenteritis on 26 February 2011.
Recommendation: Ensure that questionnaires containing all of the data elements that appear in the questionnaire found in Annex 13.2 and detailing activities and meal locations for the 72 hours before illness onset are distributed to all passengers and crew members who are gastrointestinal illness cases. Ensure that the completed questionnaires are maintained alongside the gastrointestinal illness surveillance log.

Violation: The reduced pressure zone assembly for the whirlpool compensation tank fill was weeping.
Recommendation: Ensure that backflow preventers are maintained in good repair.

Site: Food Service General-Ice Machines
Violation: Several ice machines in the main galley and ice panties had a fine black/gray powder material in the water return collection trays. In addition, some of the ice machines were soiled along the edges of the cubers.
Recommendation: Ensure that food-contact surfaces of equipment and utensils are clean to sight and touch.
