Site: Other-Steakhouse Galley
Violation: A sign advising crew members to immediately report to the medical center if they are experiencing diarrhea or vomiting was posted in the corridor containing the time control plans. In addition to vomiting and diarrhea, crew members must immediately report to the medical center if they experience any acute gastroenteritis signs/symptoms, such as fever, abdominal cramps, headache, or muscle aches.

Site: Potable Water-Aft Pool Machinery Room
Violation: The reduced pressure backflow preventer protecting the potable water system from the recreational water filling system had a constant drip from the air relief vent indicating a possible malfunction.

Violation: There was no way to actively measure the recirculation flow rates for the Fwd and Aft swimming pools.
Recommendation: Monitor and adjust the RWF's flow rates as recommended by the manufacturer and to maintain optimum public health protection and water chemistry. Install flow meters.

Site: Recreational Water Facilities-Shock Halogenation
Violation: The level of free halogen was not being recorded at the beginning and end of shock halogenation for all the whirlpools.
Recommendation: Ensure a written or electronic record of the date and time of shock halogenation (concentration in ppm at the start and completion and time) is available for review during inspections.

Site: Recreational Water Facilities-Antientrapment Drain Covers
Violation: The swimming pool drain covers for the Aft, Mid, and Fwd pool did not conform with ASME A112.19.8 standards. The drains were gravity, multiple, and greater than 3 feet apart with no alarm. An invoice was observed indicating that compliant drain covers had been purchased. In addition, the Fwd pool had 4 suction drains in which the drain covers were not in compliance with ASME A112,19.8 requirements. These suction drains did not have either a safety vacuum release system or an automatic pump shut-off system.
Recommendation: For RWFs with gravity drainage and multiple drains (2 or more drains greater than 3 feet apart), provide either a standard design (not compliant with ASME A112.19.8) drain cover and an alarm, or an ASME A112.19.8 compliant antientrapment/antientanglement drain cover. Ensure the alarm is audible and sounds in both a continuously manned space and at the RWF. Ensure this alarm is for all draining: accidental, routine, and emergency. For RWFs with suction fittings and multiple drains (2 or more drains per pump with drains greater than 3 feet apart), provide an ASME A112.19.8 compliant antientrapment/antientanglement drain cover.

Site: Recreational Water Facilities-Safety Signage
Violation: The safety signage for all recreational water facilities except for the Serenity whirlpools, did not meet the requirements of the 2011 VSP Operations Manual. It was noted that new signs have been ordered.
Recommendation: Provide safety signs for all RWFs, except for baby-only water facilities. Ensure the signs, at a minimum, include the following words: (1) do not use these facilities if you are experiencing diarrhea, vomiting, or fever; (2) no children in diapers or who are not toilet trained; (3) shower before entering the facility; and (4) bather load #. For children's RWF signs, include the exact wording "TAKE CHILDREN ON FREQUENT BATHROOM BREAKS" or "TAKE CHILDREN ON FREQUENT TOILET BREAKS." In addition to the safety sign requirements in section 6.7.1.1.1, install a sign at each whirlpool spa and spa pool entrance listing precautions and risks associated with the use of these facilities. Include, at a minimum, cautions against use by the following: (1) individuals who are immunocompromised; (2) individuals on medication or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure; and (3) pregnant women, elderly persons, and children. Additionally, caution against exceeding 15 minutes of exposure.

Violation: An unknown water source dripped continuously from the deckhead onto thawing storage racks containing trays of packaged meat products.
Recommendation: Do not store foods: (7) Under leaking water lines, including leaking automatic fire sprinkler heads, or under lines on which water has condensed. Protect food from contamination that may result from a physical, chemical, biological origin.

Site: Buffet-Staff Mess Beverage Station
Violation: There were slotted fasteners in the food splash area of the water dispenser of the ice/water machine.
Recommendation: Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Site: Galley-Potwash Area
Violation: The gauge for the hot water sanitizing compartment of the three-compartment sink indicated the water temperature was 200F. The water temperature was measured at 180F as measured by the inspector's and staff's thermometer. The crew member working at the sanitizing compartment stated he takes manual readings periodically to ensure the water temperature is accurate.
Recommendation: Ensure water temperature-measuring devices are scaled: (1) in Celsius or dually scaled in Celsius and Fahrenheit are designed to be accurate to within 1.5C (within 3F) in the intended range of use; (2) only in Fahrenheit are designed to be accurate to within 3F in the intended range of use.

Violation: There was a wet wiping cloth on the counter. The area was closed and being cleaned following the breakfast service.
Recommendation: Restrict wiping cloths to the following: (2) Cloths used for wiping food spills are dry and used for wiping food spills from tableware and single service articles or wet and cleaned, stored in a chemical sanitizer, and used for wiping spills from food-contact and nonfood-contact surfaces of equipment.

Violation: The table top with decorative fruit bowl stands was heavily soiled with more than a day's accumulation of dust. The area was not in use at the time of the inspection.

Violation: The light intensity was below 110 lux behind and around many pieces of deck and counter mounted equipment. This was observed at the ice machines in the food areas and housekeeping pantries, behind equipment at the crew mess beverage station, behind the deck ovens, behind the coffee machines in the dining rooms, in the Steakhouse Galley between the dishwash machine and clean storage rack and behind the combination oven.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) behind and around mounted equipment, including counter-mounted equipment.

Violation: The light intensity was below 220 lux in front of the toaster. This was observed on the previous inspection. A light intensity assessment has been conducted in all food areas.
Recommendation: Ensure the light intensity is at least 220 lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas. ***This is a 2005 Operations Manual requirement and must be corrected immediately.

Violation: The two counter located handwash sinks in the 2 to 5 year old room had a maximum temperature of 115F when measured with the inspector's thermometer. These sinks were located outside of the toilet room.
Recommendation: Ensure the maximum water temperature for a handwashing station does not exceed 43C (110F).
