Site: Potable Water-Cross-Connection Control Program
Violation: The list of backflow prevention devices for the guest and crew cabins did not indicate the connections the devices were installed on. The devices were installed on the shower hoses and toilets.
Recommendation: Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location.

Site: Potable Water-Striping
Violation: The water lines from the mineralizer up to the chlorine injection point for production were striped blue, not blue/gray/blue. This was corrected.
Recommendation: Stripe or paint distillate and permeate lines directed to the potable water system in accordance with ISO 14726 (blue/gray/blue).

Site: Food Service General-Refrigerant Brine Lines and Chilled Water Lines
Violation: The refrigerant brine lines were not uniquely identified in all food service areas. In addition, the chilled water lines on the ice machines in the lido galley and housekeeping pantries were not uniquely identified.
Recommendation: Uniquely identify all refrigerant brine lines in all galleys, pantries, and cold rooms to prevent cross-connections.

Site: Recreational Water Facilities-Antientrapment Drain Covers
Violation: The letter for the field fabricated drain covers did not specify the locations of the RWFs for which the covers were designed.
Recommendation: Ensure the design of custom/shipyard constructed (field fabricated) drain covers and suction fittings is fully specified by a registered design professional in accordance with ASME A112.19.8-2007. Ensure the specifications fully address cover/grate loadings, durability, hair, finger and limb entrapment issues, cover/grate secondary layer of protection, related sump design, and features specific to the RWF. Provide a letter from the shipyard for each custom/shipyard constructed (field fabricated) drain cover fitting. At a minimum, ensure the letter specifies the shipyard, name of the vessel, specifications and dimensions of the drain cover, as well as the exact location of the RWF for which it was designed. Ensure the name of and contact information for the registered design professional and signature are on the letter.

Site: Recreational Water Facilities-Automated Monitoring Sampling Lines
Violation: The sample lines for the analyzer probes on some of the RWFs were not directly from the RWFs or on the return line from each RWF and before the compensation tanks. The vessel was conducting a monitoring study to determine the best location for the sample lines.
Recommendation: Ensure the sample line for the analyzer probe (monitoring) is either directly from the RWF or on the return line from each RWF and before the compensation tank.

Site: Recreational Water Facilities-Safety Signs
Violation: The safety signs for Mickey's Slide and AquaDuck Slide did not meet the requirements of the 2011 VSP Operations Manual. According to the staff, new signs had been ordered.
Recommendation: Provide safety signs for all RWFs, except for baby-only water facilities. Ensure the signs, at a minimum, include the following words: (1) do not use these facilities if you are experiencing diarrhea, vomiting, or fever; (2) no children in diapers or who are not toilet trained; (3) shower before entering the facility; and (4) bather load #.

Site: Bar-Cove Cafe
Violation: The time control plan indicated the operating hours on embarkation day were 11:30 PM to 11:00 PM. Staff stated the opening time was 11:30 AM.
Recommendation: Update the time control plan to indicate the correct operating hours.

Site: Galley-Palo
Violation: A previously rinsed apples inside the walk-in refrigerator still had the distributer's sticker affixed.
Recommendation: Thoroughly rinse raw fruits and vegetables in water to remove soil and other contaminants before being cut, combined with other ingredients, cooked, served, or offered for human consumption in ready-to-eat form.

Site: Galley-Deck 3 Aft Dishwashing
Violation: The conveyor type glasswash machine final rinse spray was slow to activate as the front part of the rack passed under the spray nozzles. This condition was observed during 3 successive evaluations on the same machine. Once activated, the spray pattern thoroughly covered the rack. No glasses were being washed at the time the machine was being evaluated.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines.

Site: Pantry-Deck 10 Forward
Violation: A wet wiping cloth was draped over the side of the container of chemical sanitizing solution. The cloth was placed into the solution.
Recommendation: Restrict wiping cloths to the following: (2) Cloths used for wiping food spills are dry and used for wiping food spills from tableware and single service articles or wet and cleaned, stored in a chemical sanitizer, and used for wiping spills from food-contact and nonfood-contact surfaces of equipment.

Site: Buffet-Cabana's Port Side
Violation: Some of the light fixtures above the passenger side of the service line were soiled with dust.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: The inside corners of some of the previously cleaned grease pan housings on the grills were soiled with grease residue.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Galley-Deck 2 - Pastry Inner Freezer
Violation: Water was seeping from a bulkhead hatch seam to the left of the evaporator and ice had formed on the deck. No food was impacted.
Recommendation: Ensure bulkheads in food storage areas are maintained in good repair.

Site: Housekeeping-Medical Crew and Guest Public Toilets
Violation: There were no signs advising users to use a paper towel to open the door.
Recommendation: Post a sign advising users of toilet facilities to use hand towel, paper towel, or tissue to open the door unless the exit is hands free.

Site: Children Area-
Violation: There were no airtight waste receptacles in the 3-12 year old boy's and girl's toilet rooms.
Recommendation: Conveniently locate an airtight, washable waste receptacle to dispose of excrement, soiled sanitary wipes, and soiled gloves. Remove waste materials from the child activity center each day.
