Food debris and residue soiled the curtains and interior walls of the wash tank and rinse tank of the in-use flight-type warewash machine. An accumulation of dust soiled the upper portion of the air drying manifold, located along the clean landing of the flight-type warewash machine.

Recommendation:
Ensure that warewashing machines; the compartments of sinks, basins, or other receptacles used for washing and rinsing equipment, utensils, or raw foods, or laundering wiping cloths; and drainboards are cleaned: (1) Before use; (2) Throughout the day at a frequency necessary to prevent recontamination of equipment and utensils and to ensure that the equipment performs its intended function; and (3) If used, at least every 24 hours.

Recommendation:
Ensure that warewashing equipment is maintained in good repair and proper adjustment including: (1) Being maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; and (2) Maintaining water pressure, and water temperature measuring devices in good repair and ensuring their accuracy within the intended range of use.

Medical-Crew GI Illness

Violation:
A team waiter with onset of gastrointestinal (GI) illness symptoms at 2 pm on 5 September reported to the medical facility at 10:45 pm. He worked that day from 1:50 pm to 4:50 pm and again from 5:40 pm to 10:45 pm. An assistant reefer with onset of GI illness symptoms at 11 am on 26 August reported to the medical facility at 2:36 pm on 27 August. He worked on 26 August from 11:56 am to 4:31 pm, and on 27 August he worked from 7:07 am to 11:38 am, then from 12:55 pm to 5:36 pm. This same type violation was also noted in the April 2011 VSP inspection.

Recommendation:
Ensure that symptomatic non-food employees meeting the case definition for AGE are isolated in cabin or designated, restricted area until symptom-free for a minimum of 24 hours.

The cross-connection control plan did not accurately identify the various chemical dispensers and the backflow devices used in the deck 9 forward housekeeping chemical dispensing station.

Recommendation:
Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.
