Site: Potable Water-STARBORAD BUNKERING STATION
Violation: THE POTABLE WATER BUNKERING HOSES WERE LABELED 'FRESH WATER'. THE VESSEL SANITATION PROGRAM'S NEW OPERATIONS MANUAL REQUIRES THE LINES TO BE LABELED 'POTABLE WATER ONLY'.
Recommendation: Ensure that potable water hoses are identified for use with potable water by labeleing each hose with the words "potable water only" in letters at least 13mm (0.5 inch) at each connecting end.

Violation: THE MOUNTED FINAL RINSE TEMPERATURE THERMOMETER WAS IN EXCESS OF 222 F DURING ACTIVE OPERATION. THE MEASURED FINAL RINSE TEMPERATURE AT THE DISH SURFACE WAS 172 F. IT APPEARS THAT THE MOUNTED FINAL RINSE TEMPERATURE THERMOMETER IS OUT OF CALIBRATION. THERE WAS EVIDENCE OF EXCESSIVE STEAM EXITING THE MACHINE. THE MOUNTED FINAL RINSE PRESSURE GAUGE WAS NOT WORKING.
Recommendation: Ensure that water temperature measuring devices: (1) That are scaled in Celsius or dually scaled in Celsius and Fahrenheit are designed to be accurate to 1.5C in the intended range of use. (2) That are scaled only in Fahrenheit are designed to be accurate to 3F in the intended range of use.

Violation: THE IPM LOG INDICATED ACTIVE MONITORING OF 'FRUIT FLIES' IN VARIOUS PROVISION, GALLEY, AND OTHER AREAS OVER THE 6 MONTH PERIOD REVIEWED, WITH SUBSEQUENT PESTICIDE APPLICATION TO THESE AREAS AT EACH SIGHTING. THE CONTINUAL RETURN OF THESE PESTS TO THE SAME AREAS INDICATES THAT CONTROL STRATEGIES MAY BE INEFFECTIVE.
Recommendation: Ensure that the presence of insects, rodents, and other pests is effectively controlled to minimize their presence in the food storage, preparation, service areas, warewashing, and utensil storage areas aboard a vessel. Ensure that any pests are identified accurately to allow the proper pest management to be performed.
