Site: Medical-MEDICAL - CLINICAL SPECIMEN CONTAINERS AND PACKAGING
Violation: THERE WERE ONLY 5 PROPER CONTAINERS FOR SPECIMENS COLLECTION FOR VIRAL PROCESSING. SUBSTITUTE CONTAINERS WERE AVAILABLE, BUT THE SEAL FOR EACH WAS WEAK. THE SHIP DID NOT HAVE THE REQUIRED PACKAGING BOXES AND LABELS FOR SHIPPING AS PRESCRIBED IN THE 2005 OPERATIONS MANUAL AND DETAILED AT THE VSP WEBSITE.
Recommendation: Ensure that at least 10 clinical specimen collection containers for both bacterial and viral agents (10 each), as well as proper shipping containers and labels are maintained onboard the vessel.

Recommendation: Ensure that backflow preventers are installed when air gaps are impractical or when water under pressure is required. Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Recommendation: Ensure that documentation of tank disinfection includes the concentration of halogenation used for disinfection. When using the alternative potable water tank cleaning and disinfection procedure, verify that the chlorine level is 5 ppm (mg/L) or less before placing the tank back into service.

THE LABEL AFFIXED TO EACH TRAY LISTED 7 AUGUST FOR THE PREPARATION DATE AND 13 AUGUST FOR DISCARD. STAFF STATED THESE EGGS WERE ON TIME ONLY AS A PUBLIC HEALTH CONTROL AND WOULD BE DISCARDED AT 10 AM. THERE WAS A LOG ENTRY AT 6 AM FOR WHEN THE EGGS WERE PLACED INSIDE THE UNIT, BUT THE TIME AS A PUBLIC HEALTH CONTROL PLAN STATES THAT COLD POTENTIALLY HAZARDOUS FOODS STORED IN REACH-IN REFRIGERATORS ARE ON TEMPERATURE CONTROL.
Recommendation: Ensure that the crew is familiar with and follows the time as a public health control plan.

Violation: THE 7-DAY DISCARD LABELS WERE LEFT ON CONTAINERS THAT WERE ON TIME CONTROL FOR USE AS BACK-UP ON THE BUFFET. THE DISCARD DATE FOR THESE ITEMS WAS 08/13/2005 AND NOT 4 HOURS AFTER THE FOODS WERE PLACED ON TIME CONTROL.
Recommendation: Remove the 7-day discard date labels from foods when placed on time control. If the service time is greater than 4 hours, label the food containers with the discard time.

THE WRITTEN TIME ONLY AS A PUBLIC HEALTH CONTROL PLAN STATES THAT ALL COLD POTENTIALLY HAZARDOUS FOODS IN REACH-IN REFRIGERATORS IN GALLEYS 3, 4, AND 5 USE TEMPERATURE CONTROL.
Recommendation: Ensure that the crew is familiar with and follows the temperature, and time as a public health control plans.

Violation: UPRIGHT REACH-IN REFRIGERATOR 4811.053.4009 HAD 4 SHELVES FILLED WITH HALF-PINT MILK CONTAINERS AND 3 SHELVES FILLED WITH INDIVIDUAL YOGURT CONTAINERS ON SHEET PANS. THE STAFF STATED THAT THESE PRODUCTS WERE STORED IN THE REFRIGERATOR EARLIER IN THE MORNING AND OVER 2 HOURS AGO. THE INTERNAL TEMPERATURE OF THE YOGURT WAS 47 F AND THE MILK WAS 45 F. THE AMBIENT AIR TEMPERATURE OF THE REFRIGERATOR WAS VERIFIED TO BE 38 F. THE STAFF DESCRIBED THE PROCESS AND TRANSPORT OF THESE PRODUCTS AND IT WAS CLEAR THAT THEY WERE NOT BLAST-CHILLED OR THE TEMPERATURE CHECKED PRIOR TO STORAGE IN ROOM SERVICE. ALL THE TRAYS WERE REMOVED IMMEDIATELY AND TAKEN TO THE BLAST CHILLER.
Recommendation: Ensure that potentially hazardous foods are transported in a timely manner to maintain proper internal temperatures. Ensure that potentially hazardous food is maintained: (1) At 60C (140F) [roasts 54C (130F)] or above, or (2) At 5C (41F) or less.

THE ACTUATING LEVERS FOR BOTH THE ICE AND THE SEPARATE WATER SIDE WERE DESIGNED SO THAT CUPS WILL CONTACT THE LIP SURFACE WHEN FILLED. THE STAFF STATED THAT CREW MEMBERS DO REFILL DRINKS AT THE STATION.
Recommendation: Ensure that procedures for second portions and refills are as follows: (1) Except for refilling a consumer's drinking cup or container without contact between the pouring utensil and the lip-contact area of the drinking cup or container, food employees may not use tableware, including single-service articles, soiled by the consumer, to provide second portions or refills. (2) Except as specified below, self-service consumers may not be allowed to use soiled tableware, including single-service articles, to obtain additional food from the display and serving equipment.

Violation: THE ICE MACHINES THAT WERE IN USE THROUGHOUT THE GALLEY, BUFFET, PANTRIES, SPECIALTY RESTAURANTS, AND BARS HAD SLOTTED FASTENERS IN THE ICE/WATER CONTACT AREA OF THE UPPER COMPARTMENT, MAKING CLEANING DIFFICULT.
Recommendation: Ensure that multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches. Replace these fasteners with smooth, low profile, non-corroding fasteners.

Ensure that utensils are maintained in a state of repair or condition that meets the materials, design and construction specifications of these guidelines or are discarded.
