For example, for voyage 26421 (12-18 October) the number of crew members onboard was 855, but the logs stated 3721; for voyage 26422 (19-25 October) the number of crew members onboard was 875, but the logs stated 3665; for voyage 26423 (26 October - 1 November) the number of crew members onboard was 867, but the logs stated 4383.

Violation:
A Stateroom Attendant had an onset of AGE symptoms on 22 October at 00:30, but did not report to the medical center until 16:17. His time card stated he worked 12:01-13:02. He received a written warning for late reporting.

Recommendation:
When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document date and time of last symptom and clearance to return to work. Exclude food employees suspected of, diagnosed with, or exposed to any communicable disease caused by Salmonella typhi, Shigella spp., E. coli O157:H7, hepatitis A virus, norovirus, or other communicable diseases that can be transmitted by food, from working in any food or food related areas or operations, including working with exposed food, warewashing, equipment, utensils, table linens, single-service articles and single-use articles. Do not allow the excluded individual to return to the above duties until they are symptom free for a minimum of 48 hours.

Mango mousse was stored in the walk-in refrigerator without any covering. Also in this walk-in refrigerator there was sponge cake, a tray of individual gelatins, berry cobbler, apple pies, and individual cheese cakes all uncovered on the top shelf of their individual storage racks. All items were covered immediately during the inspection.

Violation:
During operation of the flight-type conveyor dishwash machine, the final sanitizing rinse spray pattern was not producing a proper pattern and the far right upper nozzle was spraying and stopping frequently. In addition, there was a significant scaling and soiling visible on the stainless steel panel just after the final sanitizing spray arm. Water made contact with this panel and dripped heavily down to the surfaces, including dishware passing below. The build-up on the upper and lower sections of the panel was beyond one day's accumulation.

Recommendation:
Ensure warewashing machines, drainboards, and the compartments of sinks, basins, or other receptacles used for washing and rinsing equipment, utensils, or raw foods, or laundering wiping cloths are cleaned: (1) before use; (2) throughout the day at a frequency necessary to prevent recontamination of equipment and utensils and to ensure that the equipment performs its intended function; (3) at least every 24 hours (if used). Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

The deckhead throughout this pantry had what appeared to be an oily liquid on the panels and droplets forming in most areas of the space. Although technicians arrived and checked the overhead hatches to identify a source, none was found. When wiped, the liquid was light green in color and bar staff stated they made no cleaning using a product like this.

Housekeeping-Outbreak Prevention and Response Plan

Violation:
The ship's Outbreak Prevention Plan (OPP) stated in section 2.2 that when applying chemicals, the crew should ensure guests were not exposed to those chemicals. This was a statement rather than a procedure. The staff also presented a standard operating procedure (SOP) on the fogging process, but that procedure offered no information on protecting passengers from exposure to the fogging chemicals. In the previous VSP inspection it was noted that there was no procedure in the outbreak plan to protect passengers for respiratory or dermal exposure to the disinfectant products used, particularly during spraying and fogging applications.

Recommendation:
Update the OPP to state that when any spraying or fogging operations occur, first remove passengers and crew members from the area and use methods to prevent passengers and crew members from entering the area both during and after the operation.

The medical staff were not paying particular attention to the number of crew members and the voyage dates on the AGE surveillance logs. AGE surveillance logs contained over four times the number of crew members onboard and incorrect voyage dates. For example, the logs indicated the voyages started and ended on a Friday or started and ended on a Saturday. Medical staff showed the itineraries which indicated voyages started on a Saturday and ended on a Friday.

Recommendation:
Ensure the supervisor or person in charge of medical operations related to AGE on the vessel demonstrates to VSP-during inspections and on request-knowledge of medical operations related to AGE, is able to demonstrate this knowledge by compliance with Section 4 of the VSP 2011 Operations Manual or by responding correctly to the inspector's questions as they relate to the specific operation, and properly trains employees to comply with Section 4 of the VSP 2011 Operations Manual as it relates to their assigned duties.
