Item No.: 01
Site: Medical-Acute Gastroenteritis (AGE) Reporting
Violation: A 4-hour update report did not contain a cumulative total count of the reported crew and passengers during the entire cruise, including the additional cases. For the 23-30 November voyage, a 24-hour report was submitted on 29 November at 12:58 am with 35 passenger and 17 crew reportable cases and a subsequent 4-hour report was submitted at 7:31 pm with 31 passenger and 9 crew reportable cases. The medical staff could not explain the reason for these reports.
Recommendation: If the number of cases changes after submission of the initial report, submit an updated report no less than 4 hours before the vessel's arrival at the U.S. port. Ensure that the 4-hour update report contains a cumulative total count of the reported crew and passengers during the entire cruise, including the additional cases.

Site: Medical-Acute Gastroenteritis (AGE) Surveillance Log
Violation: For the last 5 cruises, more than 30 cases were classified as non-reportable cases when they reported symptoms that were above normal for those individuals and met the definition of a reportable case. The posted AGE case definition explained by the medical staff did not consider the number of diarrhea episodes that were above normal to certain individuals. The electronic quality management system of the ship had a different AGE case definition that was consistent with section 4.1.1.1.1 of the 2011 VSP operations Manual. The medical staff was unaware of the definition change and as a result a number of cases were not reported.
Recommendation: Ensure a reportable case of AGE is a case reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member and meets the case definition based on the following symptoms: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period or what is above normal for the individual. See section 4.1.2.1.3, illness symptoms).

Site: Recreational Water Facilities-Swimming Pool Drains
Violation: The letter shown to the inspector from Disney's registered design professional indicating ASME A112.19.8-2007 compliance for the shipyard constructed gravity drain covers for the swimming pools, did not meet all the requirements as specified in section 6.7.1.2.2 of the 2011 VSP Operations Manual. The letter was missing the specifications and dimensions of the drain cover as well as the exact location of the RWFs for which they were approved. The letter was also unclear as it mentioned the main pool drains having suction drains, but according to staff these were gravity drains.
Recommendation: Ensure the testing of manufactured drain covers is by a nationally or internationally recognized testing laboratory. Ensure the following information is stamped on each manufactured antientrapment drain cover: (1) certification standard and year; (2) type of drain use (single or multiple); (3) maximum flow rate (in gallons or liters per minute); (4) type of fitting (suction outlet); (5) life expectancy of cover; (6) mounting orientation (wall, floor, or both); (7) manufacturer's name or trademark; and (8) model designation. Ensure the design of custom/shipyard constructed (field fabricated) drain covers and suction fittings is fully specified by a registered design professional in accordance with ASME A112.19.8-2007. Ensure the specifications fully address cover/grate loadings, durability, hair, finger and limb entrapment issues, cover/grate secondary layer of protection, related sump design, and features specific to the RWF. Provide a letter from the shipyard for each custom/shipyard constructed (field fabricated) drain cover fitting. At a minimum, ensure the letter specifies the shipyard, name of the vessel, specifications and dimensions of the drain cover, as well as the exact location of the RWF for which it was designed. Ensure the name of and contact information for the registered design professional and signature are on the letter.

Site: Galley-Cabana Dishwash
Violation: The in-use flight-type dishwasher was alarming to alert the warewash team of a malfunction in the final rinse temperature. The food employees in the area did not appear to be responsive to the alarm while continuing to put plates into the machine.
Recommendation: Ensure galley dishwash personnel are trained to react to dishwash alarms.

Site: Galley-Cabana Ice Cream Stations
Violation: There were no sneeze guards for the dipper wells which were less than 1 meter (40 inches) from the passengers.
Recommendation: Protect food on display from contamination by the use of packaging; counter, service line, or salad bar food guards; display cases; self-closing hinged lids; or other effective means. Install side protection for sneeze guards if the distance between exposed food and where consumers are expected to stand is less than 1 meter (40 inches).

Site: Pantry-Deck 6 Fwd - Ice Machine
Violation: The handle to the ice scoop was laying in direct contact with the ice.
Recommendation: During pauses in food preparation or dispensing, ensure food preparation and dispensing utensils are stored: (1) In the food with their handles above the top of the food and the container; (2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon.

Violation: Three trolleys of bread were stored without the top food items protected.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected.

Site: Dining Room-Remy
Violation: Many food-contact surfaces of the Simplex deli slicer had chipped paint.
Recommendation: Materials used in the construction of multiuse utensils and food contact surfaces of equipment must be: (1) durable, corrosion resistant, and nonabsorbent; (2) sufficient in weight and thickness to withstand repeated warewashing; (3) finished to have a smooth, easily cleanable surface; and (4) resistant to pitting, chipping, crazing, scratching, scoring, distortion and decomposition.

Site: Dining Room-Remy
Violation: Many non-food contact surfaces of the Simplex deli slicer had chipped paint.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Site: Galley-Deck 2 - Hot Section
Violation: Water was leaking from underneath the front of the left combination oven.
Recommendation: Ensure nonfood contact equipment is maintained in good repair and proper adjustment including: (1) Equipment maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines; (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Site: Galley-Cabana Dishwash
Violation: The in-use flight-type dishwasher had low final rinse pressure and the temperature at the final rinse manifold was measured consistently by the machine below 180F. Most of the time the display was reading 177F.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Site: Galley-Cabana Dishwash
Violation: The final rinse temperature at the plate surface of the in-use flight-type dishwasher was consistently below 160F. On several attempts, the final rinse temperature ranged from 118F to 137F. Maintenance workers started to service the machine immediately.
Recommendation: In a mechanical operation, ensure the temperature of the fresh hot water sanitizing rinse as it enters the manifold is not more than 90C (194F) or less than: (1) 74C (165F) for a stationary rack, single-temperature machine; (2) 82C (180F) for all other machines. Ensure the utensil surface temperature is not less than 71C (160F) as measured by an irreversible registering temperature indicator.

Site: Galley-Deck 2 - Entrance from Buffet
Violation: The temperature at the handwash station was 124F. The user could not adjust the temperature. This was corrected.
Recommendation: Ensure handwashing sinks are equipped to provide water at a temperature of at least 38C (100F) through a mixing valve or combination faucet. For handwash sinks with electronic sensors, and other types of handwash sinks where the user cannot make temperature adjustments, ensure the temperature provided to the user after the mixing valve does not exceed 49C (120F).

Site: Galley-Palo
Violation: One of the deckhead light fixtures above the flat grills was out of repair. As a result, the light intensity over the flat grills was less than 220 lux. This was corrected.
Recommendation: Ensure the light intensity is at least 220 lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.

Violation: To the right of the combination ovens, the light intensity was less than 110 lux.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) behind and around mounted equipment, including counter-mounted equipment.

Violation: Three brooms and dust pans were stored in the area.
Recommendation: Ensure maintenance tools such as mops, brooms, and similar items are stored in a designated locker so they do not contaminate food, food-contact surfaces of utensils, and equipment, linens, and single-service and single-use articles.

Violation: A large hand-held deck scrubber was stored in the same room with boxes of glasses, mugs, and other bar items. The scrubber was removed.
Recommendation: Ensure only articles necessary for the food service operation are stored in food preparation, food storage, and warewashing areas.

Violation: Not all of the pest sightings contained follow-up inspection results.
Recommendation: When pests are noted during an inspection, ensure the log includes action taken and follow-up inspection results.

Recommendation: Ensure the supervisor or person in charge of medical operations related to AGE on the vessel demonstrates to VSP?during inspections and on request?knowledge of medical operations related to AGE, is able to demonstrate this knowledge by compliance with Section 4 of the VSP 2011 Operations Manual or by responding correctly to the inspector?s questions as they relate to the specific operation, and properly trains employees to comply with Section 4 of the VSP 2011 Operations Manual as it relates to their assigned duties.
