Site: Medical-Acute Gastroenteritis (AGE) Surveillance Log
Violation: On 27 July, a passenger reported one vomit episode and was deemed a reportable case because the one vomit was above normal to that individual. This case did not meet the reportable case definition.
Recommendation: Ensure a reportable case of AGE is a case reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member and meets the case definition based on the following symptoms: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period or what is above normal for the individual. See section 4.1.2.1.3, illness symptoms); or (2) vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature of greater than or equal to 38C [100.4F]).

Site: Potable Water-Microbiologic Monitoring
Violation: Both color comparators for analyzing results of the potable water testing method (Colilert) for microbiological monitoring expired in July. There was a requisition from June for a new comparator.
Recommendation: Analyze samples using a method accepted in Standard Methods for the Examination of Water and Wastewater. Operate and maintain test kits, incubators, and associated equipment in accordance with the manufacturers' specifications.

Site: Potable Water-Forward Evaporator
Violation: There was no air gap at the air relief vent of the reduced pressure assembly. An inverted plastic bottle with an open bottom was surrounding the vent.
Recommendation: Remove the bottle or lower it so to achieve an air gap that is twice the diameter of the relief vent opening.

Site: Potable Water-Dialysis Machines
Violation: The ship did not have backflow protection for dialysis machines. Staff stated that passengers could bring their own dialysis machines. Because the machines need running water to function, passengers connected the machines to potable water connections in their cabins. Staff stated that passengers could make connections to the sink faucet or the shower hose in the bathroom of the cabins. The faucet connection did not have backflow prevention and the atmospheric vacuum breaker on the shower hose connection did not offer adequate backflow prevention.
Recommendation: Provide reduced pressure assemblies or pressure vacuum breakers for dialysis machine connections.

Site: Provisions-Beverage Storage
Violation: The overwrap on pallets of bottled water stored in the open loading area was very dusty. The open loading area was not finished in accordance with food storage area requirements for deckheads, decks, and bulkheads.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; and (4) At least 15 centimeters (6 inches) above the deck. Do not store foods: (10) In areas not finished in accordance with 7.7.4 and 7.7.5 for food storage areas.

Violation: Lang broiler #25 had a difficult to clean technical space where the broiler height adjusting handle was installed.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Galley-Hot Galley
Violation: Lang broiler #25 had more than a day's accumulation of grease in the difficult to clean technical space where the broiler height adjusting handle was installed. The adjacent broiler's technical space was completely clean.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: A slight buildup of dust was noted in the inverted decorative shells on the front counter food display case.

Site: Galley-Passageway
Violation: To the left of the corridor entrance, along the passageway from the dirty ware drop off near door FSDA4.18 a constant drip was observed from the deckhead seam. Crew thought it was from condensate dripping from the air conditioning ducts above the deckhead. No food was being impacted. This problem was not noted in other areas of this galley.
Recommendation: Repair the drip.

Site: Provisions-Frozen Fish Storage
Violation: An icicle had formed from water leaking from two deckhead panel seams near the back bulkhead in the first row of storage shelves. Another frozen leak was observed from the deckhead/bulkhead seam in the same area. Ice was found on the shelf, not on the cartons of food stored in the area.
Recommendation: Repair the leaks.

Violation: A slight leak was noted above the aft walking area between shelves of beverages stored in carton. The deck was wet from this leak. No food was impacted.

Violation: Pallets of bottled water were found stored in the open loading area that was not finished in accordance with food storage area requirements for deckheads, decks, and bulkheads. When this was noted, crew began to immediately move these pallets to a more appropriate storage space.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, bars, and food and equipment storage areas are constructed and maintained for easy cleaning.

Violation: In the technical compartment of the undercounter refrigerator, there were gaps where pipes penetrated the back bulkhead. There was no back panel in the technical compartment.
Recommendation: Close the gaps.

Site: Galley-Chop Sticks
Violation: A live fly was noted on the bulkhead of the food preparation area.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Violation: Medical staff did not fully understand the AGE case definition. On 27 July, a passenger reported one vomit episode and was deemed a reportable case because the one vomit was above normal to that individual. Staff acknowledged that they understood that above normal vomits also met the definition of an AGE reportable case.
Recommendation: Ensure the supervisor or person in charge of medical operations related to AGE on the vessel demonstrates to VSP-during inspections and on request-knowledge of medical operations related to AGE, is able to demonstrate this knowledge by compliance with Section 4 of the VSP 2011 Operations Manual or by responding correctly to the inspector's questions as they relate to the specific operation, and properly trains employees to comply with Section 4 of the VSP 2011 Operations Manual as it relates to their assigned duties.
