Site: Potable Water-Potable Water Tank Maintenance
Violation: In reviewing the records of potable water tank cleaning, painting and disinfection, the procedures and records appeared to be very comprehensive and complete. The photos of nearly all the potable water tanks taken before this work had several areas of the tanks where a black colored sediment was evident on the tank walls. The most significant of the photos were those including the tank deck, which in some cases had some residual water which also appeared black. Strongly suggest a more involved assessment of the potable water tanks be made over the next year to understand what may be causing this strong discoloration. In some of the center most tanks little of this condition was observed in the photos.

Site: Medical-Crew Gastrointestinal (GI) Illness Reporting
Violation: There were several medical data points reviewed for both of the crew GI cases that made it very difficult to understand what happened. The AGE Surveillance log noted the bartender had symptom onset at 10 am on 28 July, but the meal and activity questionnaire the crew member completed had 2 pm written as onset. The work record was also a problem because it is not clear if the employee actually did return to work after onset, even though that is what's recorded. The sommelier records were worse, in that the illness onset time on the AGE log indicates 1:30 pm on 21 July, but her medical record indicates onset at 12:30 am on 22 July. The work record notes she came to work at 9:02 am on the 22nd of July, but there is no entry beyond this to understand if she left work and reported to medical or never came to work. Lastly, this employee noted on her self-completed meal and activity questionnaire that she went ashore at a port, but failed to name the port. Under activities on the day of illness onset the employee listed 20 July as the onset date and indicated meals in the crew mess that day. The medical staff on board during this inspection had no information except the records, as they only arrived to the ship in the last two cruises.
Recommendation: Ensure that the information documented in the AGE log and other supporting records, such as work history and the self-administered questionnaire are completed clearly and accurately.

Site: Potable Water-Potable Water Production
Violation: During January and February of 2015 this ship was sailing in South America and produced potable water while sailing in polluted rivers as part of those voyages.
Recommendation: Operate a reverse osmosis unit, distillation plant, or other process that supplies water to the vessel?s potable water system only while the vessel is making way, and do not operate in polluted areas, harbors, or at anchor.

Site: Medical-ICU Ward Room - Toilet
Violation: The bath tub faucet was installed with the opening below the level of the tub edge, creating a submerged inlet for that water supply. In addition, there was a flexible shower and shower head attached to the lower edge of the tub faucet, but the backflow preventer installed was attached at the base of the shower head handle, where it could easily be immersed in the tub or other non-potable liquid and again become a submerged inlet. There was no other visible backflow preventer in the supply line before the faucet or flexible spray hose.
Recommendation: Install a backflow preventer either on the potable water hot and cold supply to the shower or on the tub tap connection, if that faucet is raised at least so the bottom of the backflow device under it is at least 25 mm above the flood rim of the tub.

Site: Potable Water-Cabins 6300 and 7114
Violation: In both cabins there were flexible shower hoses that could reach either the deck, the toilet, or both and there was no backflow preventer installed on the flexible shower hose or the potable water supply line to each.
Recommendation: Provide a comprehensive cross-connection control program. Protect the following connections to the potable water system against backflow with air gaps or mechanical backflow prevention devices: (1) RWFs; (2) decorative water features/fountains; (3) cabin shower hoses, toilets, whirlpool tubs, and similar facilities.

Site: Food Service General-Deck and Bar Pantries and Galley Ice Machines
Violation: In nearly alll the ice machines in the food areas of the ship there were potable water labels attached to the plastic wire ties that were fixed to the N9 backflow preventers at the point were the weep holes are located, effectively blocking them.
Recommendation: Ensure the pipe identification tags are not tied against the intermediate vents for the backflow prevention devices.

Site: Galley-Main Galley - Pastry
Violation: The backflow preventer at the bulkhead water supply hose for the large dough mixers had a significant amount of debris clogging the weep holes of the backflow preventer.
Recommendation: Maintain backflow prevention devices in good repair.

Site: Recreational Water Facilities-RWF Maintenance
Violation: The log for hair and lint strainer and hair and lint strainer housing cleaning and disinfection only listed hair and lint strainer in the title. Staff complete this weekly, but the title indicates only the hair and lint strainers are cleaned and disinfected, so that should be changed to note the housing for both are also cleaned and disinfected.
Recommendation: Edit the log to indicate that the hair and lint strainers and hair and lint strainer housings are cleaned and disinfected.

Site: Medical-Crew Gastrointestinal (GI) Illness Reporting
Violation: A bartender with onset of GI illness symptoms at 2 pm on 28 July, 2015 reported to the medical facility at 5 pm. The work record indicated this staff member worked from 11:30 am to 1:53 pm, then had a work break and went back in at 4:35. A Sommelier had GI symptom onset at 12:30 am and reported to the medical facility at 9:45 am on 22 July.
Recommendation: Exclude food employees suspected of, diagnosed with, or exposed to any communicable disease caused by Salmonella typhi, Shigella spp., E. coli O157:H7, hepatitis A virus, norovirus, or other communicable diseases that can be transmitted by food, from working in any food or food related areas or operations, including working with exposed food, warewashing, equipment, utensils, table linens, single-service articles and single-use articles. Do not allow the excluded individual to return to the above duties until they are symptom free for a minimum of 48 hours. When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document date and time of last symptom and clearance to return to work. When nonfood employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 24 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document the date and time of last symptom and clearance to return to work.

Site: Buffet-Terrace Cafe - Waiter Station #2 and Starboard/Aft
Violation: There were several filled water pitchers set at the waiter stations #2 and starboard aft without lids or any covering as they were stored between uses.
Recommendation: Cover the water pitchers with a fixed cover or film to the level just short of the pour spout when not in use.

Site: Galley-Main Galley - Pastry
Violation: Some raised fibers were noted along the dough sheeter belt that was in use during the inspection.
Recommendation: Food contact equipment must be maintained in good repair and proper adjustment: (1) Equipment must be maintained in a state of repair and condition that meets the materials, design, construction, and operating specifications of these guidelines; and (2) Cutting or piercing parts of can openers must be kept sharp to minimize the creation of metal fragments that can contaminate food when the container is opened.

Site: Bar-Terrace Cafe
Violation: The door from the front bar to the pantry had open holes in the frame where plastic caps were not inserted. This leaves openings for pests to access and breed.
Recommendation: Cap holes in the dor frame to prevent insect harborage.

Site: Buffet-Terrace Cafe - Cold Buffet - Starboard/Aft
Violation: There was a heavy dust and grease debris present in the slot at the deckhead, near to the back bulkhead over the cold buffet starboard/aft section.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Site: Galley-Main Galley - Decks
Violation: The decks had some broken or cracked tiles which made cleaning difficult. This was more the case near gutterways and deck sinks.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Site: Dining Room-Grand Dining Room - Waiter Station #2
Violation: The light fixture at the bulkhead over waiter station #2 had two bulbs inside that were not completely covered and which were not shatter-resistant bulbs.
Recommendation: Ensure light bulbs are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, and linens, or unwrapped single-service and single-use articles.

Site: Buffet-Pizzeria - Pantry
Violation: The artificial light level behind the counter model coffee machine was below the minimum 110 lux.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) behind and around mounted equipment, including counter-mounted equipment.

Site: Room Service-Deck 5 Room Service Pantry
Violation: There were two flat top grills with undercounter ovens in this space that were no longer in use, according to staff. These should be removed if not used so other equipment or cabinets that can be used in the large space could be installed.
Recommendation: Ensure only articles necessary for the food service operation are stored in food preparation, food storage, and warewashing areas.

Site: Buffet-Terrace Cafe - Waiter Station #2
Violation: One live fruit fly was observed on the deckhead during active food service above the back counter of waiter station #2.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.
