The AGE log from the December 26-January 2 voyage was missing some information in the underlying illness column. Two passengers experienced AGE symptoms, one patient with 3 episodes of diarrhea and the other patient with 6 episodes of diarrhea, and were classified as nonreportable. When reviewing the passengers' medical records, it was discovered both were lactose intolerant and these AGE symptoms were normal for them.

Recommendation:
Ensure the AGE surveillance log entry for each passenger or crew member contain the following information in separate columns: (16) presence of underlying medical conditions that may affect interpretation of AGE.

The log indicated that continuous pressure backflow prevention devices were installed on the showers in the infirmary, and non-continuous devices were observed on the showers.

Recommendation:
Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections.

The records for the annual test results for two reduced pressure backflow prevention devices, installed on the distillate water tank filling from the evaporator and installed for the filling of the Deck 14 Forward Serenity Whirlpool Starboard, were not available.

Recommendation:
Inspect and test backflow prevention devices requiring testing with a test kit after installation and at least annually. Maintain the test results showing the pressure differences on both sides of the valves for each device.

The free halogen residual was not documented for the disinfection of potable water pipes on 10/16, 10/19, 11/4, 12/23, 12/31, 1/10, and 1/11. Potable water pipe disinfection records could not be located for installation before 10/16/2015.

Recommendation:
Document the free halogen residual level. Maintain documentation of all inspections, maintenance, cleaning, and disinfection for 12 months and ensure it is available for review during inspections. Ensure records include a method of disinfection, concentration and contact time of the disinfectant, and a halogen value of less than or equal to 5 ppm before the tank is put back into service.
