Site: Medical-Food/Beverage Questionnaire
Violation: The Food/Beverage Questionnaire detailing activities and meal locations the 72 hours before illness onset did not comply with the 2018 VSP Operations Manual. Numerous questionnaires did not designate if the symptomatic patient was a crew member or passenger.
Recommendation: Distribute questionnaires detailing activities and meal locations for the 72 hours before illness onset to all passengers and crew members who are AGE CASES. Ensure that the self-administered questionnaires contain all of the data elements that appear in the questionnaire found in Annex 13.2.2. Ensure that completed questionnaires are maintained with the AGE surveillance log.

Site: Potable Water-Production Records
Violation: The potable water production record, dated 2 April 2019, identified a free halogen and pH level monitoring test occurred at 20:10 on 3 April 2019. This same record identified the subsequent free halogen and pH level monitoring test occurred at 20:05 on 4 April 2019. Staff stated this was a mistake and the actual free halogen and pH level monitoring test occurred at 00:05 on 4 April 2019.
Recommendation: After the free residual halogen level of at least 2.0 mg/L (ppm) and pH level not exceeding 7.8 have been reached, monitor the free residual halogen and pH at least once every 4 hours during the production of potable water. Ensure a test kit is available for testing free halogen levels and pH.

Violation: A roll of garbage bags was stored on top of a case of bottled water. This was corrected.
Recommendation: Protect food from contamination that may result from a physical, chemical, biological origin.

Violation: A plastic container of bread crumbs, removed from the original package, was not identified with the common name of the food.
Recommendation: Ensure containers holding food or food ingredients that are removed from their original packages for use on the vessel, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar are identified with the common name of the food.

Site: Galley-Deck 4 Portside Soda Station
Violation: The bulk milk tube, located in the right milk dispenser, was not cut on the diagonal. This was corrected.
Recommendation: Cut bulk milk dispensing tubes 45 degrees on the diagonal, leaving no more than 25 mm (1 inch) protruding from the chilled dispensing head.

Site: Galley-Deck 4 Pastry
Violation: One new cooling rack, wrapped in plastic and with the manufacturer's sticker still attached, was rusted around the edges on the food-contact surface. This was corrected.
Recommendation: Materials used in the construction of multiuse utensils and food contact surfaces of equipment must be: (1) durable, corrosion resistant, and nonabsorbent; (2) sufficient in weight and thickness to withstand repeated warewashing; (3) finished to have a smooth, easily cleanable surface; and (4) resistant to pitting, chipping, crazing, scratching, scoring, distortion and decomposition. Design and construct food contact surfaces of equipment and utensils to be durable and to retain their characteristic qualities under normal use conditions.

Site: Room Service-Deck 5 Room Service Pizza Area
Violation: There were slotted fasteners located on the food-contact surface of two pizza cutters. These pizza cutters were discarded.
Recommendation: Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Site: Other-Engine Control Room (ECR)
Violation: The coffee machine, located against the back left wall of the engine control room, had a slotted fastener to the left of the dispensing nozzle and a missing fastener to the right of the dispensing nozzle. Both of these difficult-to-clean surfaces were soiled with greater than a day's accumulation of brown residue.
Recommendation: Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Site: Galley-Windjammer Hot Galley
Violation: The rubber handle on the deep-fat fryer basket was cracked and in disrepair. This created a difficult-to-clean surface. Staff stated they would replace the basket.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance. Design and construct nonfood-contact surfaces of equipment and utensils to be durable and to retain their characteristic qualities under normal use conditions. Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Site: Galley-Deck 4 Portside Dishwash
Violation: The coating on one waiter's tray was peeling, creating a difficult-to-clean surface. This nonfood-contact surface was not soiled. This tray was discarded.
Recommendation: Design and construct nonfood-contact surfaces of equipment and utensils to be durable and to retain their characteristic qualities under normal use conditions. Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Galley-Deck 5 Dishwashing Station
Violation: Water was dripping from the bottom of the wash tank. Water was observed pooled on the deck below this leak.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Site: Pantry-Deck 5 Portside Wine Cellar
Violation: One wet cleaning cloth was stored on the side of an empty sanitizing bucket. This was corrected. The area was not in operation during the inspection.
Recommendation: Restrict wiping cloths to the following: (1) Cloths used for wiping food spills are used for no other purpose; (2) Cloths used for wiping food spills are dry and used for wiping food spills from tableware and single service articles or wet and cleaned, stored in a chemical sanitizer, and used for wiping spills from food-contact and nonfood-contact surfaces of equipment; (3) Dry or wet cloths used with raw animal foods are kept separate from cloths used for other purposes. Wet cloths used with raw animal foods must be kept in a separate sanitizing solution; (4) Wet wiping cloths used with a freshly made sanitizing solution and dry wiping cloths are free of food debris and visible soil.

Violation: Five previously cleaned plate covers were soiled with food debris on the inside surface. These items were sent for rewashing.

Site: Pantry-Ice Station Deck 8 FSD - 08-4-006
Violation: The lower portion of the deflector panel for the ice machine, located in the water splash zone directly above the ice bin, was covered in a sticky brown residue with the consistency of a removed sticker from the panel. When the inspector rubbed this residue with an alcohol wipe, pieces flaked off.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Violation: The slotted fastener to the left of the dispensing nozzle and the missing fastener to the right of the dispensing nozzle, located on the coffee machine, were soiled with greater than a day's accumulation of brown residue. Corrections started immediately.

Violation: The countertop, located underneath the mounted coffee machine, was soiled with greater than a day's worth of accumulation of coffee grounds and beans. Approximately nine coffee beans were resting where the cords for the coffee machine protruded into the technical space below. Corrections started immediately.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: The potable water line to the water filter was soiled with more than one day's worth of accumulation of dust and debris. Corrections started immediately.

Violation: Approximately 10 glasses were stored wet and were out for service. Visible dripping was present from the beverage-contact surfaces of these glasses. Corrections started immediately.
Recommendation: After cleaning and sanitizing, ensure equipment and utensils are air dried or adequately drained before contact with food.

Violation: Water was dripping from the deckhead onto the deck next to deep-fat fryer #57112. The source of this water could not be determined.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary. Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Violation: Water pooled on the deck below the dishwasher due to a leak from the wash tank.
Recommendation: Ensure black and gray water is discharged to the vessel's wastewater disposal system and does not pool on the deck.

Violation: Condensate was dripping from the deckhead above convection oven #47101. No food items were impacted.

Site: Integrated Pest Management-Rat Guards
Violation: The rat guard, located on the reverse mooring line at the aft of the ship, was insufficient to preclude the entry of rodents onto the vessel. This rat guard was placed over two different mooring lines, with a large circular opening approximately eight inches in diameter in the center. Additionally, bottom two sides of this rat guard did not meet, creating a large opening surrounding the bottom mooring line. Corrections started immediately.
Recommendation: Protect entry points where pests may enter the food areas. Ensure rat guards are made of a durable and non-chewable material. Use single-line, multiple-line, or conical shape rat guards according to the manufacturer?s specifications. Ensure rat guards are on all lines that go ashore upon arrival and until at least one hour before the ship leaves port. Ensure each line has at least one rat guard placed either as far as practicable from the pier or as far as practicable from the ship while being able to stuff any openings with non-chewable material. Ensure rat guards are not staggered on adjacent lines and a group of lines do not merge onto one rat guard.

Site: Integrated Pest Management-Insect Sighting Log
Violation: The insect sighting log identified the presence of two cockroaches inside a passenger stateroom on 25 January 2019 at 11:21. Roach bait was applied in this stateroom from 11:40 until 12:05. The log identified follow-up was scheduled on 31 January 2019. No follow-up for this insect sighting was documented.
Recommendation: When pests are noted during an inspection, ensure the log includes action taken and follow-up inspection results.
