Item No.: 06
Site: Potable Water-Microbiologic Sample Reports
Violation: There was no E.Coli test result recorded for the water received by barge on March 11 in Manaus, Brazil. Staff showed an email stating that a sample was negative for that day but it was not specific if the email referred to a pre-chlorination sample or post-chlorination sample. The form used for recording sample results had information recorded for a post-chlorination sample only.
Recommendation: If water samples are collected and analyzed by the vessel for the presence of E. coli, analyze the samples using a method accepted in Standard Methods for the Examination of Water and Wastewater. Ensure test kits, incubators, and associated equipment are operated and maintained in accordance with the manufacturers? specifications.

Site: Galley-Cold Section
Violation: When the inspection team was in the cold area of the main galley, a food employee was observed swatting at flies with his hands. It was explained to the inspectors that fumigation only happens at night and no immediate action was to be taken at this point. Flies were flying and resting on the counter and equipment.
Recommendation: Ensure the supervisor or person in charge of food operations on the vessel monitors that: (1) Food operations are not conducted in a room used as living or sleeping quarters; (2) Food employees are effectively cleaning their hands.

Site: Other-Bridge
Violation: The coffee machine had slotted fasteners in the food splash area.
Recommendation: Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Site: Galley-Deck 4 Dishwasher
Violation: The dishwasher near the front entrance of the galley had a continuous leak underneath the dishwash which created a pool of water on the deck. The dishwasher was not in operation at the time of the inspection. Remediation began immediately.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines.

Site: Galley-Passenger Colonade
Violation: There were approximately 10 wet nested plate covers stacked underneath a storage counter.
Recommendation: After cleaning and sanitizing, ensure equipment and utensils are air dried or adequately drained before contact with food.

Site: Galley-Deck 4 Dishwasher
Violation: The dishwasher near the front entrance of the galley had a continuous leak underneath the dishwash which created a pool of water on the deck. The dishwasher was not in operation at the time of the inspection. Remediation began immediately.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Site: Food Service General-Mutiple Areas
Violation: Flies were observed in several area including three flies near the food digester in the garbage room, one fly in the Colonnade near ice machine, one fly near the potwash machine in the passenger main galley, three near the cold galley in the passenger main galley, and one grasshopper in the patio grill.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Site: Integrated Pest Management-Active Monitoring
Violation: It was unclear how accurate the pest management records were maintained. Active monitoring records from January to March all had zero findings. However, eight flies and a grasshopper were found in the food areas during the inspection. When the inspection team was in the cold area of the main galley, a food employee was observed swatting the flies with his hands. It was explained to the inspectors that fumigation only happens at night and no immediate action was to be taken at this point.
Recommendation: Ensure the vessel has an IPM plan to implement effective monitoring and control strategies for pests aboard the vessel.
