Violation:
There were three entries in the log that documented 50ppm disinfection start and end times of only three hours, but the documented work description indicated disinfection time was for four hours. All three entries were during the ship's dry dock in March 2023. On 20 March 2023, the 50ppm start disinfection time was '4:00' and the end time was '7:00' for pipe replacement on deck 1 forward fire zone 2. On 18 March 2023, the 50ppm start disinfection time was '4:30' and the end time was '7:30' for replacing water valves on deck 10 forward fire zone 1. On 15 March 2023, the 50ppm start disinfection time was '4:30' and the end time was '7:30' for a pipe replacement on deck 10 fire zone 1. The work descriptions for all three entries correctly described disinfecting with 50ppm for four hours. Staff explained the recorded times were clerical errors.

Recommendation:
Disinfect after potential contamination by increasing the free residual halogen to at least 50 mg/L (ppm) throughout the affected area and maintain this concentration for 4 hours or by way of another procedure submitted to and accepted by VSP.

A food employee had an onset of AGE symptoms on 20 May 2023 at 2000, but did not report to medical until 21 May 2023 at 1035. The employee's timecard indicated they had worked while symptomatic on 20 May 2023 from 0630 to 2247. A food risk assessment was completed, disciplinary action was taken, and the crew member received additional training.

Recommendation:
When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document date and time of last symptom and clearance to return to work. Ensure food employees who have conditions or symptoms of boils, open sores, infected wounds, diarrhea, jaundice, fever, vomiting, sore throat with fever, or discharges from the nose or mouth report these conditions or symptoms to the vessel's medical staff and are restricted from working with exposed food, warewashing, clean equipment, utensils, and linens, and unwrapped single-service and single-use articles.
