Item No.: 01
Site: Medical-24-Hour Report
Violation: On the 24-hour reports, the total number of passengers and the total number of crew were not the totals at the beginning of the voyage. This included the records for the following voyages: (1) V504 (26 May to 9 June 2023) the total number of passengers was 88 on the 24-hour report and 236 on the AGE surveillance log; the total number of crew was 193 on the 24-hour report and 195 on the AGE surveillance log; (2) V506 (20 June to 1 July 2023) the total number of passengers was 271 on the 24-hour report and 292 on the AGE surveillance log; the total number of crew was 191 on the 24-hour report and 204 on the AGE surveillance log; and (3) V508 (13 July to 20 July 2023) the total number of passengers was 218 on the 24-hour report and 292 on the AGE surveillance log; the total number of crew was 197 on the 24-hour report and 204 on the AGE surveillance log. Medical staff advised that the 24-hour report reflected the total number of passengers and total number of crew at the time of the report submission while the acute gastroenteritis surveillance log reflected the totals at the beginning of the voyage.
Recommendation: Ensure the 24-hour and 4-hour reports contains the following: total number of passengers and crew members on the cruise. This is to be the number of passengers and crew members at the start of the voyage. Also, ensure these numbers match the header of the acute gastroenteritis surveillance log.

Site: Potable Water-Pipe Disinfection Records
Violation: On 25 April 2023, two backflow prevention devices in the deck 3 main galley were replaced, but the start and stop times for the disinfection with more than 50ppm was less than 4 hours. The first entry start time was 10:15 and end time was 12:30 for a total of 2 hours and 15 minutes. The second entry start time was 10:15 and end time was 12:19 for a total of 2 hours and 4 minutes.
Recommendation: Disinfect after potential contamination by increasing the free residual halogen to at least 50 mg/L (ppm) throughout the affected area and maintain this concentration for 4 hours or by way of another procedure submitted to and accepted by VSP.

Site: Potable Water-Cross-Connection Control Log
Violation: The log did not have a complete listing of backflow prevention devices. Crew advised that the ship was lengthened during a major dry dock from 2019 to 2020. Verification and visually inspection of all backflow prevention devices was an ongoing project.
Recommendation: Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections.

Site: Recreational Water Facilities-Antientrapment Drain
Violation: There was no documentation verifying the installation date for the antientrapment drain covers for the pool and two whirlpools.
Recommendation: Confirm when drain covers were installed so they do not exceed their life expectancy.

Site: Recreational Water Facilities-Whirlpool Temperature
Violation: The midship whirlpool water temperature exceeded 40C (104F) as measured by the inspector using two different thermometers. The regulator reading was 37.9C and the set point was 38C. Crew stated they use the regulator reading to check the whirlpool water temperature. The inspector and crew discussed using a thermometer to directly test the whirlpool water temperature and to verify the regulator reading.
Recommendation: Provide a temperature-control mechanism to prevent the temperature from exceeding 40C (104F) on whirlpool spas and spa pools. Measure the water temperature within the whirlpool spa or spa pool tub itself.

Site: Recreational Water Facilities-Hair and Lint Strainer
Violation: The weekly cleaning and disinfection was documented for the most recent week prior to the inspection, but not for other weeks since arriving into US ports starting in June.
Recommendation: Clean, rinse, and disinfect the hair and lint strainer and hair and lint strainer housing on all RWFs weekly. Ensure disinfection is accomplished with an appropriate halogen-based disinfectant. At a minimum, use a 50-ppm solution for 1 minute, or equivalent CT value. Maintain records on all inspection and cleaning procedures. Ensure that if there is not a hair and lint strainer but there is a filter before the pump, this filter must be cleaned, rinsed, and disinfected weekly.

Site: Recreational Water Facilities-Cartridge Filter Inspection and Filter Change
Violation: The weekly inspection for the whirlpool spas cartridges was documented for the most recent week prior to the inspection, but not for other weeks since arriving into US ports in June.
Recommendation: Clean, rinse, and disinfect the filter housing before the new filter media is placed in it. Ensure disinfection is accomplished with an appropriate halogen-based disinfectant. At a minimum, use a 50-ppm solution for 1 minute, or equivalent CT value. Maintain records on all inspection and cleaning procedures. Ensure the filter housing must be cleaned, rinsed, and disinfected each time the filter media-including cartridge filter-is changed.

Site: Pantry-Deck 8 Yacht Club Cafe
Violation: The water hose for the coffee machine was draped along the counter, making cleaning difficult.
Recommendation: Elevate the hose above the counter.

Site: Galley-Deck 7 Veranda
Violation: Ceramic pans were overlapping on the racks and loaded into the hood-type dishwash machine in a position that did not allow all areas of the pans to be exposed to the spray nozzles.
Recommendation: Ensure soiled items to be cleaned in a warewashing machine are loaded into racks, trays, or baskets or onto conveyors in a position that (1) exposes the items to the unobstructed spray from all cycles; (2) allows the items to drain.

Site: Galley-Deck 7 Veranda
Violation: In the potwash area, the minimum water temperature of the handwash station measured 126F. It was determined the cold water valve was turned off. This was immediately corrected.
Recommendation: Ensure handwashing sinks are equipped to provide water at a temperature of at least 38C (100F) through a mixing valve or combination faucet. For handwash sinks with electronic sensors, and other types of handwash sinks where the user cannot make temperature adjustments, ensure the temperature provided to the user after the mixing valve does not exceed 49C (120F).
