Item No.: 02
Site: Medical-Acute Gastroenteritis (AGE) Logs
Violation: For voyage X332, three out of eight AGE cases did not have the underlying condition column completed correctly. The crew were using the underlying column to record if a telephonic consultation occurred.
Recommendation: Ensure the AGE surveillance log entry for each passenger or crew member contain the following information in separate columns: (1) presence of underlying medical conditions that may affect interpretation of AGE. If none, write ?none,? ?not applicable,? ?N/A,? or similar wording. Comments may also be added to the log in this column after the information about underlying illness. Ensure the AGE surveillance log contains the above information in the exact order and is entered in the template in Annex 13.2.2. Ensure that the log data is exported in the exact order as in the example template in Annex 13.2.2 with analyzable formats such as Excel or Access. Ensure that any additional data fields are entered only outside of the form margins when exported to VSP.

Site: Potable Water-Deck 4 Garbage Room
Violation: The air gap between the reduced pressure zone (RPZ) assembly's intermediate vent and the top of the drain catch basin was insufficient.
Recommendation: Ensure air gaps are at least twice the diameter of the delivery fixture opening and a minimum of 25 millimeters (1 inch).

Site: Potable Water-Engine Room
Violation: At the forward evaporator and both reverse osmosis units the air gap between the reduced pressure zone (RPZ) assembly's intermediate vent and the top of the drain catch basin was insufficient.
Recommendation: Ensure air gaps are at least twice the diameter of the delivery fixture opening and a minimum of 25 millimeters (1 inch).

Violation: The water fill level was not to the skim gutter level across 90% of the pool.
Recommendation: For RWFs with skim gutters, ensure that the fill level of the RWF is to the level of the skim gutters.

Violation: Excessive frozen condensate was on the deckhead, shelves, trays, and the top surface od ice cream containers.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination.

Site: Dining Room-Crew Beverage Station
Violation: The ambient temperature gauge for the milk refrigerator displayed 48F and the internal thermometer displayed 49F. However, the milk temperature measured 40.3F when taken with the inspector's and crew's thermometer.
Recommendation: Ensure food temperature measuring devices: (1) scaled only in Celsius or dually scaled in Celsius and Fahrenheit are accurate to within1C (within 2F) in the intended range of use; (2) scaled only in Fahrenheit are accurate to within 2F in the intended range of use. Ensure food temperature measuring devices have a numerical scale, printed record, or digital readout in increments no greater than 1C (2F) in the intended range of use.

Site: Galley-Deck 6 - Portside Forward Glasswash
Violation: The glasswash was out-of-service at the time of the inspection. A work order stated the machine was taken out of service on 8 August 2023 at 0344.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Site: Buffet-Deck 16 - Bistro Guest Portside Beverage Station
Violation: The juice dispenser had more than a day?s worth of food debris on the food splash area above the nozzles. The staff started cleaning this immediately.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Violation: Seven food preparation trays were stored soiled and wet nested in the undercounter clean storage compartment beside the warewash machine. Cleaned and sanitized equipment was stored wrapped in plastic beside the trays.
Recommendation: After cleaning and sanitizing, ensure equipment and utensils are air dried or adequately drained before contact with food.

Violation: Condensate was dripping from the deckhead above the in-use potwash machine and the rinse sink, which was not in use at the time of inspection.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.
