Item No.: 01
Site: Medical-Acute Gastroenteritis (AGE) 24-Hour Report
Violation: The 24-hour AGE report for 15 September 2023 listed the current crew and passenger counts, not the total crew and passengers at the beginning of the voyage. Additionally, the ship submitted unnecessary AGE reports while the ship remained in the U.S. from 15 - 21 September. These included a special report on 16 September while overnighting in New York and a 24-hour report on 20 September prior to arrival in Bar Harbor on the 21st. The ship did not reach 2%.
Recommendation: If there is an update to the AGE surveillance log after the 4-hour report is submitted, submit an additional 4-hour report if the vessel is still more than 4 hours from arrival in the U.S. port. Ensure the AGE report contains the following: (1) total number of passengers and crew members on the cruise at the beginning of the voyage.

Site: Potable Water-Striping - Recycling Center
Violation: The main potable water line and downstream connections were not striped blue/green/blue or blue to indicate potable water. This was corrected.
Recommendation: Stripe or paint potable water lines either in accordance with ISO 14726 (blue/green/blue) or blue only. Stripe or paint these lines at 5 meters (15 feet) intervals and on each side of partitions, decks, and bulkheads, except where decor would be marred by such markings, including potable water supply lines in technical lockers. Do not stripe or paint potable water lines after reduced pressure assemblies as potable water.

Site: Potable Water-Striping - Engine Room
Violation: Distillate and permeate water lines prior to chlorine injection were not consistently striped blue/gray/blue every 5 meters. Remediation began during the inspection.
Recommendation: Stripe or paint distillate and permeate lines directed to the potable water system in accordance with ISO 14726 (blue/gray/blue). Ensure no other lines have the above color designations. Stripe or paint these lines at 5 meters (15 feet) intervals and on each side of partitions, decks, and bulkheads, except where decor would be marred by such markings, including potable water supply lines in technical lockers.

Site: Recreational Water Facilities-Deck 15 - Thalasso Spa Pool
Violation: The safety sign did not include the required precautions listed in section 6.8.1.1.3.
Recommendation: In addition to the safety sign requirements in section 6.7.1.1.1, install a sign at each whirlpool spa and spa pool entrance listing precautions and risks associated with the use of these facilities. Include, at a minimum, cautions against use by the following: (1) individuals who are immunocompromised; (2) individuals on medication or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure; and (3) pregnant women, elderly persons, and children. Additionally, caution against exceeding 15 minutes of exposure. Those under 16 years of age are considered children for the purpose of whirlpool safety sign requirements.

Violation: The consumer advisory sign for one of the two roast beef stations did not state that the roast beef was served undercooked.
Recommendation: Ensure the consumer advisory disclosure is made by the following method at a buffet: (1) On a sign describing the animal-derived foods (e.g., ?oysters on the half-shell,? ?hamburgers,? ?steaks,? or ?eggs?); AND that they can be cooked to order or may be served raw or undercooked; AND a statement indicating that consuming raw or undercooked meats, seafood, shellfish, eggs, milk, poultry may increase your risk for foodborne illness, especially if you have certain medical conditions.

Violation: A small, plastic container of strawberries had visible white fuzz over approximately 1/4 of the strawberries.
Recommendation: Discard a food that is unsafe or adulterated.

Site: Buffet-Deck 11 Executive Lounge
Violation: There was a small hole on both sides of the coffee machine which led to a void and created a difficult to clean surface.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Room Service-Deck 5
Violation: Two coffee pots that were previously cleaned and sanitized and stored to air dry were soiled on the inside food-contact surface.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Pantry-Deck 6 Martini
Violation: The backflow prevention device for the utility sink's spray arm was heavily soiled with a bright green substance. The backflow prevention device was immediately cleaned.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Galley-Deck 5 Roasting Station
Violation: Condensate collected on the deckhead above combination oven 13. Staff stated the ventilation was operating at full strength.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Site: Preparation Room-Deck 4
Violation: The bulkhead panel above the cleaning materials locker, where the fire door attaches to the magnet, was missing the top two fasteners which led to a void space and created a gap between the metal panels.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Site: Galley-Deck 5 Roasting Station
Violation: Condensate collected on the deckhead above combination oven 13. Staff stated the ventilation was operating at full strength.
Recommendation: Ensure ventilation hood systems and devices are sufficient in number and capacity to prevent grease or condensation from collecting on bulkheads and deckheads. Ensure all food preparation, warewashing, and toilet rooms have sufficient ventilation to keep them free of excessive heat, steam, condensation, vapors, obnoxious odors, smoke, and fumes.

Site: Galley-Deck 14 Bakery
Violation: A large fly was near the handwashing station.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Site: Housekeeping-Crew Toilet Facility Near Crew Mess
Violation: No hand towels were available in the crew toilet facility nor was the automated hand dryer functioning. Additionally, the room was not equipped so persons exiting the toilet room have a bare-hands-free exit.
Recommendation: Equip passenger and crew public toilet facilities so persons exiting the toilet room are not required to touch the door handle with bare hands. Where toilet stalls include handwashing facilities, ensure the bare-hands-free contact begins in the toilet stall. Ensure that toilet facilities with multiple exits, such as spa dressing rooms, have bare-hands-free contact at each exit. Provide the passenger and crew public toilets with a handwashing station that includes: (1) Hot and cold running water; (2) Soap; (3) A method to dry hands; (4) A sign advising users to wash hands.
