Site: Potable Water-Engine Room - Forward Evaporator #1
Violation: The air gap below the reduced pressure principle backflow prevention assembly's relief vent was insufficient. Staff stated they fixed the insufficient air gap after the last inspection; however, additional maintenance occurred, and the drain funnel was moved back up which created the insufficient air gap.
Recommendation: Ensure air gaps are at least twice the diameter of the delivery fixture opening and a minimum of 25 millimeters (1 inch).

Violation: A nonfood employee reported to work at 0700 on 21 September 2023 and continued working while symptomatic with acute gastroenteritis (AGE) symptoms until they reported to medical at 0922.
Recommendation: When nonfood employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 24 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document the date and time of last symptom and clearance to return to work.

Site: Buffet-Deck 5 Officer Mess Beverage Station
Violation: Inside the milk dispenser, the milk's discard date label was marked 10/01/23-10/26/23. Staff stated the dates were incorrect and should be 10/26/23-11/01/23. This was corrected immediately.
Recommendation: Ensure refrigerated, ready-to-eat, potentially hazardous food: (1) Prepared on a vessel and held refrigerated for more than 24 hours is clearly marked at the time of preparation to indicate the date or day by which the food must be consumed (7 calendar days or fewer from the day the food is prepared). The day of preparation is counted as day 1. (2) Prepared and packaged by a food-processing plant and held on the vessel after opening for more than 24 hours must be clearly marked at the time the original container is opened to indicate the date by which the food must be consumed (7 calendar days or fewer after the original container is opened). The day of opening is counted as day 1.

Violation: The cutting board on the preparation counter was heavily scored making it difficult to clean.
Recommendation: Ensure surfaces such as cutting blocks and boards that are subject to scratching and scoring are resurfaced if they no longer can be effectively cleaned and sanitized or discarded if they are not capable of being resurfaced.

Site: Preparation Room-Standing Refrigerators
Violation: All three standing refrigerators had door gaskets that were peeling towards the bottom making it difficult to fully close.
Recommendation: Ensure nonfood contact equipment is maintained in good repair and proper adjustment including: (1) Equipment maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines; (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Violation: The undercounter refrigerator's gasket was peeling and hanging from the door.

Violation: Inside the counter-mounted microwave, the silicone along the bottom was in disrepair making the surface difficult to clean. This was repaired immediately.

Recommendation: Procedures, systems, equipment, technology, processes, or activities that are not identified in the scope of this manual must not be tested or introduced operationally onboard any vessel until the concept is submitted in writing to the VSP Chief for review. If the review determines the concept is within the scope of the VSP Operations Manual, written procedures, control measures, or a complete variance submission may be required.
