Site: Medical-Acute Gastroenteritis (AGE) Log
Violation: The passenger and crew totals for the voyage on 19-25 January 2024 did not match the number of passengers and crew from the beginning of the voyage.
Recommendation: Ensure the AGE surveillance log includes a header containing the following information about the voyage total number of passengers and total number of crew must be the totals at the beginning of the voyage (i.e., totals on ?date from?).

Site: Medical-Reportable Case
Violation: A passenger reported to medical on 18 January 2024 asking for antidiarrheal medication for the voyage home after the cruise ended on 19 January 2024. Medical staff questioned the passenger who reported he experienced a total of five episodes of diarrhea beginning on 11 January 2024, the day the voyage began, and persisted on and off until 14 January 2024. The passenger indicated the symptoms were above normal. Medical staff administered the medication, however, did not mark the case as reportable.
Recommendation: Ensure a reportable case of AGE is a case reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member and meets the case definition based on the following symptoms: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period or what is above normal for the individual. See section 4.1.2.1.3, illness symptoms); or (2) vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature of greater than or equal to 38C [100.4F]).

Site: Potable Water-Air Gap
Violation: Air gaps below the atmospheric vents of several reduced pressure zone (RPZ) backflow assemblies in the engine room were not twice the diameter of the supply pipe. These locations included: both overboard lines for the evaporators and the technical filling line in the aft evaporator room. These were all corrected by the end of the inspection.
Recommendation: Ensure air gaps are at least twice the diameter of the delivery fixture opening and a minimum of 25 millimeters (1 inch).

Site: Pantry-Deck 5 - Astern
Violation: The drainage tubes on the relief vents of the soda carbonator backflow prevention devices were not positioned to properly drain. One relief tube was sloped upward and another was twisted in a knot.
Recommendation: Maintain backflow prevention devices in good repair. Slope drain tubes to adequately drain.

Site: Galley-Deck 5 - Anthology
Violation: The menu was missing the asterisk for the consumer advisory on the swami beef item that can be ordered undercooked.
Recommendation: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish that is raw, undercooked, or not otherwise processed to eliminate pathogens is offered in a ready-to-eat form or as a raw ingredient in another ready-to-eat food, ensure the consumer is informed by way of disclosure using menu advisories, placards, or other easily visible written means of the significantly increased risk to certain especially vulnerable consumers eating such foods in raw or undercooked form. Locate the advisory at the outlets where these types of food are served.

Site: Provisions-Deck 3 - Vegetable Room 12 030130
Violation: Water dripped from the deckhead onto a box of potatoes stored near the back of the room. The pallet of potatoes was relocated.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; (4) At least 150 millimeters (6 inches) above the deck; (5) At least 127 millimeters (5 inches) for food in packages and working containers on pallets, skids, and racks; and (6) At least 150 millimeters (6 inches) above the deck on shelving racks.

Violation: Water pooled on top of a box of liquor that was stored underneath an air supply vent.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; (4) At least 150 millimeters (6 inches) above the deck; (5) At least 127 millimeters (5 inches) for food in packages and working containers on pallets, skids, and racks; and (6) At least 150 millimeters (6 inches) above the deck on shelving racks.

Violation: The sealant around the cold well on the worker side was missing and/or peeling.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: The third from the right heat lamp was soiled with food debris.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Violation: The drip compartment for the grooved grill had food debris in the bottom left. The compartment had previously been cleaned and sanitized. The area was not in operation.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: Water dripped from the deckhead onto the deck in two separate locations along the center of the room. No food was impacted.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: The deck in the area had concrete droplets in front of the back counter making the area rough and difficult to clean.

Violation: Grease residue was on the exhaust hood overhang of the center cooking area on the far left. This was cleaned before the end of the inspection.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: Excess green cable was on the deck inside the technical compartment beneath the handwashing station.
Recommendation: Install exposed utility service lines and pipes, including lines for fire detection and protection systems, so they do not obstruct or prevent cleaning.

Violation: A small fly was observed near the ice machine. This area was not in operation at the time.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Site: Housekeeping-Acute Gastroenteritis (AGE) Disinfection Log
Violation: The disinfection log identified the cabin number of a crew member that was isolated due to AGE on 19 January 2024, however, the log did not indicate it was disinfected at the end of isolation. The log was initialed by the supervisor without documentation of the work being completed.
Recommendation: Ensure cabins that house passengers or crew with AGE are cleaned and disinfected daily while the occupants are ill.

Site: Housekeeping-Engine Control Room
Violation: The crew toilet room did not have a sign or pictogram advising users to use a hand towel, paper towel, or tissue to open the door.
Recommendation: Post a sign advising users of toilet facilities to use hand towel, paper towel, or tissue to open the door unless the exit is hands free.
