Site: Medical-Acute Gastroenteritis (AGE) Food/Beverage Questionnaire
Violation: On two separate 72-hour food and beverage questionnaires, the food employees stated that they went to the crew mess after isolation began. Staff did not believe the crew members broke isolation, but it was an error in recorded information.
Recommendation: Distribute questionnaires detailing activities and meal locations for the 72 hours before illness onset to all passengers and crew members who are AGE CASES. Ensure that the self-administered questionnaires contain all of the data elements that appear in the questionnaire found in Annex 13.2.2. Ensure that completed questionnaires are maintained with the AGE surveillance log.

Site: Potable Water-Medical Bathroom #4
Violation: There was no backflow prevention device on the shower hose and the shower head was able to reach the deck. This was corrected before the end of the inspection.
Recommendation: Provide a comprehensive cross-connection control program. Protect the following connections to the potable water system against backflow with air gaps or mechanical backflow prevention devices: (1) cabin shower hoses, toilets, whirlpool tubs, and similar facilities.

Site: Potable Water-Evaporator Port and Starboard Side
Violation: The air relief vent on the reduced pressure zone assembly had an insufficient air gap. This was corrected before the end of the inspection.
Recommendation: Ensure air gaps are at least twice the diameter of the delivery fixture opening and a minimum of 25 millimeters (1 inch).

Site: Medical-Acute Gastroenteritis (AGE) Surveillance Log
Violation: A food employee was released from isolation before they were symptom free for 48 hours. The AGE surveillance log stated that a food employee had AGE symptom onset at 1220 on 4 January 2024. Their last symptom occurred at 1000 on 5 January 2024. They were released at 1915 on 6 January 2024.
Recommendation: When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document date and time of last symptom and clearance to return to work.

Site: Buffet-Deck 14 Horizon Court
Violation: The consumer advisory signs for smoked white fish and the omelet station did not include the specific statement that the animal food was cooked to order or may be served raw or undercooked.
Recommendation: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish that is raw, undercooked, or not otherwise processed to eliminate pathogens is offered in a ready-to-eat form or as a raw ingredient in another ready-to-eat food, ensure the consumer is informed by way of disclosure using menu advisories, placards, or other easily visible written means of the significantly increased risk to certain especially vulnerable consumers eating such foods in raw or undercooked form. Ensure that disclosure is made: (1) On a sign describing the animal-derived foods (e.g., ?oysters on the half-shell,? ?hamburgers,? ?steaks,? or ?eggs?); AND that they can be cooked to order or may be served raw or undercooked; AND a statement indicating that consuming raw or undercooked meats, seafood, shellfish, eggs, milk, poultry may increase your risk for foodborne illness, especially if you have certain medical conditions.

Violation: The sealant was peeling on the front bar countertop by the dispatch counter which created a rough and difficult to clean surface.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material. Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: Electrical cables for the coffee grinder and the coffee brew were draped on the portside coffee preparation countertop.

Violation: The ambient air temperature measuring device in undercounter refrigerator #5 measured 60F. The sliced melons measured with an infrared thermometer were 40F. Staff immediately removed the ambient air temperature measuring device for replacement.

Violation: The hood-type potwash machine was out of order since the morning of 9 January 2024 due to high temperatures. Staff were using the three-compartment sink.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Violation: The ice and water dispensing machine was heavily soiled with black debris on the inside of the ice dispensing chute and around the black fasteners below the chute. Staff immediately shut down the machine and started cleaning.
Recommendation: Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized. Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Violation: The ice machine's tubing that connects to the pump was soiled with a black residue. The water pump was removed from the lower level of the ice machine where it was soiled. Only the top level of the ice machine was in operation; therefore, the ice was not impacted.
Recommendation: Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized. Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Violation: Condensate collected on the inside of the ventilation hood to the right of the conveyer type warewash machine's clean side. The excessive heat was from the exhaust vents on the side of the warewash machine. The area was in operation, but kitchenware was not impacted.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: Silicone was missing from the bulkhead/equipment juncture behind the starboard side flat grill which created a seam and difficult to clean surface.

Violation: The shutter control switch box cover was missing in the center gazebo which created a rough surface that was not easy to clean. Additionally, the bulkhead and wires inside of the shutter control switch box were soiled with grease residue.

Violation: Condensate collected on the bulkhead and ventilation overhang behind the right tilting pan. Staff stated the ventilation was in energy efficiency mode and operating at 25% capacity.
