Site: Medical-Acute Gastroenteritis (AGE) Log
Violation: A food worker was not isolated for 48 hours from their last symptom. Specifically, on 28 January 2024, a food worker had AGE symptoms beginning at 2031 and was released from isolation on 29 January at 2242. Additionally, this food worker was listed as having a cabin mate, but the log stated that no close contacts were observed.
Recommendation: When any crew member (food or nonfood employee) meets the case definition for AGE, take the following actions for the asymptomatic cabin mates or immediate contacts of symptomatic crew: (1) restrict exposure to symptomatic crew member(s); (2) undergo a verbal interview with medical or supervisory staff, who will confirm their condition, provide facts and a written fact sheet about hygiene and handwashing, and instruct them to report immediately to medical if they develop illness symptoms; (3) complete a verbal interview daily with medical or supervisory staff until 48 hours after the ill crew members? symptoms began. Ensure that the first verbal interview is conducted within 8 hours from the time the ill crew member initially reported to the medical staff. If the asymptomatic immediate contact or cabin mate is at work, ensure that he or she is contacted by medical or supervisory staff as soon as possible. Document the date and time of verbal interviews. An immediate contact is someone sharing a cabin either full-time or part-time or sharing a cabin?s bathroom facilities. This includes boyfriends, girlfriends, spouses, and significant others. Ensure to document if the symptomatic crew member has or not cabin mate(s) or other immediate contact(s).

Site: Potable Water-Cross-Connection Control Log (Backflow)
Violation: Twelve (12) reduced pressure assemblies listed in the Cross-Connection Control log as being tested did not have the differential pressure values listed in the log. Additionally, no backflow prevention control devices were listed for the Aft window washing system. Staff stated the aft system had not been used since the previous dry dock one year earlier.
Recommendation: Inspect and test backflow prevention devices requiring testing (e.g., reduced pressure backflow prevention devices and pressure vacuum breakers) with a test kit after installation and at least annually. Maintain test results showing the pressure differences on both sides of the valves. Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections.

Recommendation: When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document date and time of last symptom and clearance to return to work. Document corrective actions taken as a result of the assessment. Maintain records for 1 year and make them available for review during inspections.

Site: Buffet-Deck 9 - Rosie's Restaurant Aft Starboard Side Fruit Station
Violation: A small fly was observed on the grapefruit halves. The area was in operation and the grapefruits were discarded.
Recommendation: Protect food from contamination that may result from a physical, chemical, biological origin.

Site: Galley-Deck 9 - Stacked Ovens
Violation: The top oven was missing three fasteners and had three slotted fasteners. Additionally, the second from the top was missing four fasteners. This was corrected before the end of the inspection.
Recommendation: Ensure multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams greater than 0.8 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel. Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Site: Galley-Deck 9 - Potwash
Violation: The in-use hood-type potwash machine's final sanitizing rinse temperature measured at 125F and 127F. The machine was shut down and repairs started immediately. This was corrected before the end of the inspection.
Recommendation: In a mechanical operation, ensure the temperature of the fresh hot water sanitizing rinse as it enters the manifold is not more than 90C (194F) or less than: (1) 74C (165F) for a stationary rack, single-temperature machine; (2) 82C (180F) for all other machines. Ensure the utensil surface temperature is not less than 71C (160F) as measured by an irreversible registering temperature indicator.

Site: Housekeeping-Deck 6 and 7 Pantries
Violation: The water arms for the deck 6 and deck 7 ice machines were heavily soiled with a yellow material. The inspector used an alcohol wipe to remove the residue. The machines were closed for cleaning immediately.
Recommendation: Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized. Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Pantry-Deck 5 - Sports Bar
Violation: The ice machine had a brown residue on the interior surface above the evaporator, The residue was removed when the inspector wiped it with an alcohol wipe. The area was not in operation. The machine was taken out of service to be cleaned and sanitized.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch. Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized.

Violation: Black debris was on the bulkhead tile above the stainless-steel backsplash. The inspector was able to remove it with an alcohol wipe. This area was cleaned and sanitized immediately.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.
