The log listed only one backflow prevention device for a shower in the medical ADA toilet room; however, there were three additional showers with backflow prevention devices. Additionally, the atmospheric vacuum breaker in the garbage wash room and two backflow prevention devices on the potable water lines above a sorting table were not listed on the log.

Recommendation:
Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing.
