Violation:
On six occasions, sections of the potable water system were not properly disinfected for a total of 4 hours at 50 ppm per the potable water disinfection records. For example: (1) On 1 June 2024, the potable water supply line to the far point was only disinfected for 3 hours, from 15:00 to 18:00; (2) On 25 April 2024, per written disinfection records, the hot water line replaced in the forward sewage treatment room was disinfected for one hour above 50 ppm from 14:30 to 15:30. However, in the electronic maintenance system, a comment stated the hot water line was disinfected for more than four hours; (3) On 19 April 2024, the potable water line on decks 2 and 3, zone 6, was disinfected for two hours above 50 ppm from 09:00 to 11:00; (4) On 5 April 2024, the potable water line on deck 5, zone 4, was disinfected for three hours above 50 ppm from 08:30 to 11:30; (5) On 2 April 2024, the potable water line on deck 1, zone 4, was disinfected for 2 hours and 45 minutes above 50 ppm from 00:30 to 03:15; and (6) On 31 March 2024, a backflow prevention device was disinfected for 2 hours above 50 ppm from 14:00 - 16:00.

Recommendation:
Disinfect after potential contamination by increasing the free residual halogen to at least 50 mg/L (ppm) throughout the affected area and maintain this concentration for 4 hours or by way of another procedure submitted to and accepted by VSP.

The halogen residual in the wading pool measured an average of 0.86 ppm. The inspector measured 0.65 ppm and 1.03 ppm. The technician measured 0.65 ppm and 1.11 ppm. The analyzer measured 1.04 ppm and the alarm was on. Staff immediately closed the pool.

Recommendation:
Maintain a free residual halogen in recirculated RWFs in the following ranges: wading pools, between 1.0 and 5.0 mg/L (ppm).

Violation:
Two individuals were identified by housekeeping as having a public vomit or public diarrhea incident. Medical received an email about one individual on 25 June 2024, but did not have any records of follow-up to determine if the case needed to be isolated. Medical could not locate an email from the individual identified on 1 July 2024.

Recommendation:
When symptomatic, ill passengers meet the case definition for AGE, ensure the following actions are taken: (1) advise them to remain isolated in their cabins until well for a minimum of 24 hours after symptom resolution, and (2) a follow-up by infirmary personnel is advised.

Dishes were still being run through the machine despite activated alarms.

Ensure the supervisor or person in charge of food operations on the vessel monitors that employees are properly sanitizing cleaned multiuse equipment and utensils before they are reused.
