Medical-Acute Gastroenteritis (AGE) Reportable Case Definition

Violation:
Many vomit and fecal incidents were not all followed-up by medical staff for evaluation to determine if they met the AGE case definition and needed to be isolated. Additionally, some medical follow-up phone conversations did not assess the need for isolation. For example: (1) on 2 November, a vomit incident was identified in cabin 11021 and medical follow-up recorded that the person reported feeling better; (2) on 2 November, a vomit incident was identified in cabin 14021 and medical follow-up recorded that the person took personal medication and felt better; (3) on 2 November, a vomit incident was identified in cabin 9031 and medical follow-up recorded that the person reported to be seasick; (4) on 2 November, a fecal incident was identified in cabin 12219 and there was no medical follow-up documented; (5) on 2 November, a vomit incident was identified in cabin 12271 and there was no medical follow-up documented; (6) on 3 November, a vomit incident was identified in cabin 10090 and there was no medical follow-up documented; and (7) on 4 November, a fecal incident was identified in the life jacket locker located in the Deck 7 Humidor Cigar Bar and there was no medical follow-up for the identified individual documented.

Recommendation:
Ensure a reportable case of AGE is a case reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member and meets the case definition based on the following symptoms: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period or what is above normal for the individual. See section 4.1.2.1.3, illness symptoms); or (2) vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature of greater than or equal to 38°C [100.4°F]).

Medical-Acute Gastroenteritis (AGE) Isolation
|
Violation:
The review of the AGE logs disclosed that three crew members had an onset of AGE symptoms and delayed their reporting to the medical center for evaluation. Specifically, (1) a nonfood worker had an AGE symptom onset on 18 October at 2:30pm and reported to medical at 7:58pm the same day; (2) a nonfood worker had an AGE symptom onset on 18 October at 6:00am and reported to medical at 11:35am the same day after working for some time; and (3) a food worker had an AGE symptom onset on 27 October at 6:45am and reported to medical at 10:30am the same day after working for some time. The crew members received disciplinary action because medical care is always available and the crew members knew that AGE symptoms needed to be reported immediately to medical. Food handled by the food worker was also discarded as a result of the food employee assessment.

Recommendation:
When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document date and time of last symptom and clearance to return to work. When nonfood employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 24 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document the date and time of last symptom and clearance to return to work. Ensure the food employee's supervisor or person in charge conducts an assessment of food prepared or served by the food employee while symptomatic and take appropriate corrective actions. Document corrective actions taken as a result of the assessment. Maintain records for 1 year and make them available for review during inspections. Review [when possible] any AGE cases among passengers or crew reported after the ill food employee?s symptom onset for epidemiologic link/connection. Appropriate corrective actions could include discarding ready-to eat food, thoroughly cooking raw food, and disinfecting the food area and equipment.

A small pan of lasagna was measured at 45°F by the inspector's thermometer and 44-47°F by a crew member's thermometer. The cooling log showed the food was properly cooled the day prior to the inspection. All other potentially hazardous foods in the refrigerator were measured at 41°F or below, and the refrigerator's temperature was below 41°F. The lasagna was discarded.

Except during preparation, cooking, or cooling, or when time is used as the public health control, maintain potentially hazardous foods at 57°C (135°F) or above, except that roasts may be held at a temperature of 54°C (130°F); or 5°C (41°F) or less.

Ensure handwash sinks are at least 750 millimeters (30 inches) above the deck so that employees do not have to reach excessively to wash their hands.
