Site: Potable Water-Far Point Analyzer Chart Records
Violation: The 15 November 2024 far point analyzer-chart record had a one hour gap in recording from 0500 until 0600. Additionally, the 16 November 2024 far point analyzer-chart record had a one hour gap in recording from 0400 until 0500. There were no notations on these charts to identify why these gaps in recording occurred.
Recommendation: Change, initial, and date halogen analyzer-chart recorder charts daily. Ensure the charts contain notations of any unusual events in the potable water system. If electronic data loggers are used in lieu of chart recorders, record notations of any unusual events in the potable water system log.

Site: Potable Water-Deck 4 - Medical Center, Ward 5
Violation: The backflow prevention device installed on the potable water line for the shower head was not designed for continuous pressure. The shower head was designed with an on/off valve downstream from the atmospheric backflow prevention device.
Recommendation: Install a continuous pressure-type backflow preventer when a valve is located downstream from the backflow preventer.

Site: Galley-Deck 14 - Swirls
Violation: The ice cream dipper well was not effectively protected from passengers standing less than one meter away.
Recommendation: Protect food on display from contamination by the use of packaging; counter, service line, or salad bar food guards; display cases; self-closing hinged lids; or other effective means. Install side protection for sneeze guards if the distance between exposed food and where consumers are expected to stand is less than 1 meter (40 inches).

Site: Provisions-Deck 3 - Beer and Soda Storage # 3204
Violation: Water dripped from the deckhead onto boxes containing food items stored below. Crew initiated corrective action.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; (4) At least 150 millimeters (6 inches) above the deck; (5) At least 127 millimeters (5 inches) for food in packages and working containers on pallets, skids, and racks; and (6) At least 150 millimeters (6 inches) above the deck on shelving racks. Protect food from contamination that may result from a physical, chemical, biological origin.

Violation: The handles for the self-service utensil were touching the smoked mackerel out for passenger self-service. Crew initiated corrective action.
Recommendation: During pauses in food preparation or dispensing, ensure food preparation and dispensing utensils are stored: (1) In the food with their handles above the top of the food and the container.

Violation: The protective covering was peeling and missing on two heat lamp light bulbs in the food-splash zone above food out for self-service. Crew initiated corrective action.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch. Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; (4) At least 150 millimeters (6 inches) above the deck; (5) At least 127 millimeters (5 inches) for food in packages and working containers on pallets, skids, and racks; and (6) At least 150 millimeters (6 inches) above the deck on shelving racks.

Violation: The counter/bulkhead coving was in poor repair along the left of the coffee machine, creating a difficult-to-clean surface.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: A peeling sticker was attached to the food-contact surface of the soda dispensing gun, creating a difficult-to-clean surface.

Violation: The ice/water dispensing machine had brown residue on the food-contact surface of the ice dispensing chute. Crew initiated corrective action.
Recommendation: Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized. Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Galley-Deck 6 - Hot Galley
Violation: Grease residue accumulated on the previously cleaned exhaust hood overhang. Crew initiated corrective action.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Food Service General-Deck and Deck Tile Maintenance
Violation: The deck grout and deck tiles throughout multiple food service outlets were recessed, missing, or broken. This created difficult-to-clean surfaces on numerous decks. Dust and food debris accumulated inside areas of recess. These areas included: Deck 14 Lido Galley, Bellbox, Deck 5 Main Galley, and Deck 4 Crew Galley.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Violation: Flies were observed in the following food service locations: Deck 14 ? Lotus Bar # 14507: Two small flies were near the ice machine. Deck 4 ? Crew Mess Entrance: A small fly was near the handwashing station. Deck 4 ? Crew Mess Hot Line, Portside: Two small flies were near the handwashing station. Deck 4 ? Officer Mess Pantry: A small fly was inside the pantry.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.
